Reconstruction of Full-Thickness Lateral Defect of the Lower Lip and Vermilion with our Modification of a Fan Flap — Technique and Results
AbstractSuccessfully restoring lateral lip defects, while ensuring proper mouth opening, oral competence, and the creation of an aesthetically pleasing vermilion, has consistently posed a challenging undertaking. In a prospective study conducted from 2006 to 2022, we employed our modified version of the McGregor fan flap technique to reconstruct post-oncosurgical pure lateral lower lip and complete lower lip defects. The study excluded cases involving a central lip defect or accompanying buccal mucosa involvement. A total of 126 flap procedures were performed on 122 patients, encompassing lower lip defects ranging from 30 ...

Outcomes Following Pedestal Cup Reconstruction of (Impending) Pathological Fractures of the Acetabulum due to Metastatic Bone Disease
AbstractManagement of periacetabular metastatic bone disease (MBD) is challenging, specifically if associated with bone loss or fracture. The aim of this study was to evaluate the complications and outcomes after undergoing peri-acetabular reconstruction using an ‘ice-cream cone’ pedestal cup endoprostheses for the most severe cases of (impending) pathological acetabular fractures. Fifty cases with severe periacetabular disease were identified. Acetabular defects were classified using the Metastatic Acetabular Classification (MAC). Pre- and post-operativ e mobility was assessed using the Eastern Cooperative Oncology Gr...

Prospective Study to Evaluate Efficacy of Single Versus Double Drains in Breast Cancer Patients Undergoing Surgery
AbstractSeroma formation is a common sequel following modified radical mastectomy (MRM), which hinders healing, may prolong hospital stay, and cause a delay in adjuvant treatment. Closed suction drains have been used to prevent formation of seroma; however, the use of a single drain in the axilla along with draining the mastectomy flaps and axilla separately remains a topic of debate. This prospective randomized dual-arm study was conducted in the Department of Endocrine Surgery. All female patients with carcinoma breast diagnosed on core tissue biopsy, undergoing modified radical mastectomy, upfront or post neoadjuvant sy...

A Rare Presentation of Recurrent Malignant Peripheral Nerve Sheath Tumor with Glandular Differentiation —A Case Report
We report a case of recurrent MPNST with glandular differentiation arising in the background of neurofibroma in a 20-year-old lady. By immunohistochemistry, MPNST showed focal positive staining for S100 and negative staining for SOX10 while adjacent neurofibroma showed diffuse positivity for S100 and SOX10. The glandular tumor cells showed positive staining for CDX2, Cam5.2, CK19, and CK7 (focal), while negative for SOX10 and S100. MPNST with glandular differentiation is quite rare which may pose a diagnostic challenge. The glandular differentiation in MPNST should be excluded from the metastasis from second primary with t...

A Novel Technique for Hand-Assisted Laparoscopy in Difficult Nephrectomies —Is One Hand Better Than Two? A Single-Centre, Single-Surgeon Series
AbstractLaparoscopic radical nephrectomy is the standard of care for T1 renal tumors and nowadays being used for T2 or higher tumors, resulting in higher the conversion rates. To bridge this gap, the hand-assisted laparoscopy (HAL) method was introduced. Even now, in the robotic era, this HAL approach continues to find importance in urology, especially in the most challenging cases, albeit, with a relatively low usage rate due to the cost involved and availability of hand port devices. Here, we report a case series using a novel modification of the HAL nephrectomy (HALN) technique when open conversion is needed. From a pro...

Pseudotumour of the Testis —Awareness Prevents Mismanagement
We present a case of a 30-year-old gentleman who presented with a hard painless testicular mass. Ultrasound scrotum revealed a diffuse paratesticular swelling arising from the tunica vaginalis with a normal-appearing testis. Germ cell tumour markers were normal. Exploration through an inguinal approach confirmed it to be arising from the tunica vaginalis. Wide excision of the tumour along with a cuff of adherent tunica albuginea was done. Final histopathology revealed a diffuse fibrous pseudotumour of the tunica vaginalis confirming its benign nature. (Source: Indian Journal of Surgical Oncology)

Systemic Therapy for Pancreatic Neuroendocrine Tumors
AbstractPancreatic neuroendocrine tumors (PanNETs) account for approximately 2% of all pancreatic malignancies. Several systemic treatment options have been developed over the last four decades, ranging from cytotoxic chemotherapy and octreotide to newer targeted therapies like sunitinib, cabozantinib, and lenvatinib. Although surgery or liver-directed therapy remains cornerstone for management of metastatic PanNETs, however, they remain unfeasible in majority of cases. PanNETs behave differently than SI-NETs (small intestinal NET); the former is more aggressive and less responsive to somatostatin-based therapies. The opti...
