Valve-in-valve implantation to seal post-dilation-induced annular rupture in transcatheter aortic valve replacement
J Invasive Cardiol. 2024 May 13. doi: 10.25270/jic/24.00092. Online ahead of print.ABSTRACTAn 85-year-old woman was admitted to our hospital with severe symptomatic aortic stenosis. Preoperative computed tomography and transesophageal echocardiography (TEE) revealed a type I bicuspid aortic valve.PMID:38754081 | DOI:10.25270/jic/24.00092 (Source: The Journal of Invasive Cardiology)

Transcatheter aortic valve replacement for aortic regurgitation: a systematic review and meta ‐analysis
AbstractThe efficacy and safety of new-generation devices (NGDs) for severe aortic regurgitation (AR) have mostly been based on single-arm studies with limited sample sizes. Our goal was to summarize the current evidence on NGDs and compare the safety and efficacy of ‘off-label’ and ‘on-label’ devices in NGDs. We searched MEDLINE, Embase, Cochrane Library, and Scopus for articles on transcatheter aortic valve replacement in patients with AR. A total of 31 studies that included 1851 patients were identified through April 2023. Among these, 1067 (57.6%) pa tients received treatment with ‘on-label’ devices (JenaVa...

Imaging in severe aortic stenosis: Looking at the future
In the present issue of the journal, Lee et al., in an interesting study and manuscript, analyze the prognostic value of left ventricular and left atrial strain imaging in an Asian population with moderate to severe aortic stenosis demonstrating also the effect of aortic valve replacement on these parameters [1]. Is There an Alternative Oral Anticoagulation to Vitamin-K-Antagonists for Patients with Mechanical Aortic Valve Replacement? – A Literature Review
AbstractCurrent guidelines exclusively recommend vitamin-K-antagonists (VKA) as anticoagulation for patients after mechanical aortic valve replacement due to the increased postoperative risk of valve thrombosis and thrombo-embolism. Strict and regular assessments are mandatory during VKA therapy to ensure a potent anticoagulatory effect within the desired range. From the patients ’ perspective, VKA are associated with relevant interactions and side effects reducing the quality of life and contributing to a high number of patients not achieving the optimal therapeutic target. Direct oral anticoagulants (DOAC) have replace...

Comparison of apixaban versus aspirin for the prevention of latent bioprosthetic aortic valve thrombosis: study protocol for a prospective randomized trial
The optimal antithrombotic strategy early after aortic valve replacement surgery with a biological valve remains controversial due to lack of high-quality evidence. Either oral anticoagulants or acetylsalicyli... (Source: Trials)

Impact of osteoporotic risk in men undergoing transcatheter aortic valve replacement: a report from the LAPLACE-TAVI registry
In conclusions, in men as same as women, osteoporotic risk assessed by OST values was overlapped with increased frailty. The simple OST formula was useful for predicting all-cause mortality in patients undergoing TAVR in large registry datasets.Graphical Abstract (Source: Cardiovascular Intervention and Therapeutics)

Three cases of Cutibacterium avidum prosthetic valve infective endocarditis at a single heart center
The widespread use of implantable prosthetic material has increased the incidence of infective endocarditis (IE) and modified the spectrum of causative microorganisms. Foreign body surfaces constitute a distinct target for adherence, and Cutibacterium acnes now accounts for 6-10% of prosthetic IE cases [1]. C. acnes is a recognized agent of various prosthetic infections, but reports of Cutibacterium avidum infections are not frequent [2]. A comprehensive list of C. avidum case reports up to 2018 is available [3], but only three cases of IE are included, all preceded by aortic valve replacement. (Source: International Journ...

Contemporary TAVI counterindications and space for staged procedures
Transcatheter aortic valve replacement (TAVR) was considered only in patients with prohibitive/extreme surgical risk, at first. In the last two decade, it has been greatly demonstrated how this approach is safe and an effective therapy, so it has been evaluated in randomized clinical trials also for patients with intermediate and low surgical risk [1]. As a result, the most recent American and European heart valve guidelines recommended TAVR respectively for patients older than 65 and 75  years, irrespective of surgical risk. Early and mid-term outcomes of tricuspid valve surgery in patients with functional tricuspid regurgitation induced by atrial fibrillation
CONCLUSIONS: TV surgery for TR induced by AF showed low surgical mortality and favorable mid-term outcomes. For these patients, early surgery after progress to severe TR can be helpful to decrease the occurrence of TVRE.PMID:38738253 | PMC:PMC11087639 | DOI:10.21037/jtd-23-1776 (Source: Journal of Thoracic Disease)

Blood flow assessment technology in aortic surgery: a narrative review
CONCLUSIONS: Blood flow assessment and computational modeling can evaluate hemodynamics and flow patterns by visualizing blood flow and calculating biomechanical forces in patients with aortic disease. We anticipate that blood flow assessment will become an essential tool in the treatment planning and understanding of the progression of aortic disease.PMID:38738252 | PMC:PMC11087597 | DOI:10.21037/jtd-23-1795 (Source: Journal of Thoracic Disease)
