Successful valve-in-valve replacement using intravascular lithotripsy for femoral access in an elderly patient with critical aortic stenosis and extensive vascular calcification
J Invasive Cardiol. 2024 May 20. doi: 10.25270/jic/24.00123. Online ahead of print.ABSTRACTAn 87-year-old man with a history of heart failure and a modified bio-Bentall Mitroflow 23-mm bioprosthesis (Sorin) was referred to our institution for critical aortic stenosis, which was confirmed by transesophageal echocardiography. Pre-procedural computed tomography revealed slightly calcified bioprosthesis leaflets, adequate coronary ostia height, and a slightly angled Bentall tube graft measuring 29 mm in diameter at the aortic anastomosis .PMID:38776474 | DOI:10.25270/jic/24.00123 (Source: The Journal of Invasive Cardiology)

Right coronary artery obstruction following transcatheter aortic valve replacement by aortic valve mass
J Invasive Cardiol. 2024 May 21. doi: 10.25270/jic/24.00147. Online ahead of print.ABSTRACTA 93-year-old woman with symptomatic severe aortic stenosis and normal biventricular function was referred for transcatheter aortic valve replacement (TAVR) evaluation. Cardiac computed tomography revealed safe coronary heights and multiple large calcified mobile mass-like structures attached to the aortic valve (AV), confirmed also by transesophageal echocardiography, which were thought to be prominent Lambl's excrescences.PMID:38776477 | DOI:10.25270/jic/24.00147 (Source: The Journal of Invasive Cardiology)

Transcatheter aortic valve replacement in a patient with pre-existing left ventricular pseudoaneurysm: the importance of pre- and intra-procedural imaging
J Invasive Cardiol. 2024 May 20. doi: 10.25270/jic/24.00143. Online ahead of print.ABSTRACTAn 85-year-old man with a history of myocardial infarction and coronary bypass surgery presented with severe symptomatic aortic stenosis. Echocardiography revealed pressure gradients of 73/40 mm Hg across the aortic valve, an aortic valve area of 0.74 cm2, and a 35% ejection fraction.PMID:38776475 | DOI:10.25270/jic/24.00143 (Source: The Journal of Invasive Cardiology)

Repeat Mitral Valve Interventions After Transcatheter Edge-to-Edge Repair:The COAPT Trial
The frequency and effectiveness of repeat mitral valve interventions (RMVI) after transcatheter edge-to-edge repair (TEER) for secondary mitral regurgitation (MR) are unknown. We aimed to examine the rate of and outcomes following RMVI after TEER in the COAPT trial. The COAPT tria randomized heart failure (HF) patients with severe secondary MR to TEER with the MitraClipTM device plus guideline-directed medical therapy (GDMT) versus GDMT alone. We evaluated the characteristics and outcomes of patients who had a RMVI during 4-year follow-up. (Source: The American Journal of Cardiology)

Cardiac computed tomography post-transcatheter aortic valve replacement
Transcatheter aortic valve replacement (TAVR) is performed to treat aortic stenosis and is increasingly being utilised in the low-to-intermediate-risk population. Currently, attention has shifted towards long-term outcomes, complications and lifelong maintenance of the bioprosthesis. Some patients with TAVR in-situ may develop significant coronary artery disease over time requiring invasive coronary angiography, which may be problematic with the TAVR bioprosthesis in close proximity to the coronary ostia. The frequency and effectiveness of repeat mitral valve interventions (RMVI) after transcatheter edge-to-edge repair (TEER) for secondary mitral regurgitation (MR) are unknown. We aimed to examine the rate of and outcomes after RMVI after TEER in the Cardiovascular Outcomes Assessment of the MitraClip Percutaneous Therapy for Heart Failure Patients with Functional Mitral Regurgitation (COAPT) trial. Only 3.9% of COAPT trial patients required a repeat mitral valve intervention during 4-year follow-up which was successful in 90% of cases but was associated with an increased rate of heart failure (HF) hospitalizations (HFH). Aortic valve replacement with rapid-deployment bioprosthesis in case of infective endocarditis: a literature review
ConclusionAlthough there are few anecdotal experiences, surgical aortic valve replacement with the use of RD represents an emerging strategy in case of aortic valve endocarditis. Its advantages, pros, and cons are under debate, and robust clinical trials are needed to demonstrate its safety and efficacy. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Is TAVR the Solution for Women with Small Annuli?
The current referenced post hoc analysis by Modine and others pools results from the Evolut Low Risk and SURTAVI intermediate risk clinical trials to compare 2-year clinical and hemodynamic outcomes in women with small annuli undergoing either transcatheter aortic valve replacement (TAVR) or bioprosthetic surgical aortic valve replacement (SAVR).1 The study highlights similar rates of all-cause mortality or stroke at 1 and 2-year follow-up, less atrial fibrillation and acute kidney injury in the TAVR arm, and superior hemodynamic performance of the self-expanding valve largely due to its decreased rates of patient prosthes...
