Future of medicine... Virtual??? What do we think/know?
When I keep seeing Walmart and Amazon hiring NP/PA providers to care for patients, it makes me wonder where the future of medicine is going. All virtual, no physicians, quick, and disingenuous. I work with doctors and other providers and just about every single one of them HATE virtual visits. People are going to do a virtual visit for ear infections, strep throat, COVID, flu... all without anyway to test for it right there. POC Glucose for diabetics in outpatient setting
Currently practice in ASC where, by policy, all diabetics get a FS glucose check and generally canceled if FS >250. Moving to outpatient hospital procedure suite. How many of you are routinely checking FS glucose in diabetics? Are you only checking in the setting of steroid injection? What's your cutoff number to cancel the case? Which class should I take or should I even take a class?
I am finishing up a do-it-yourself post-bacc, and I would also like to take at least one more science class before MCAT and applying. I was thinking about taking either an endocrinology or introductory immunology course. Osa out of hospital endoscopy
Hi Ive been approached by an out of hospital premises i work at to help develop an osa protocol. Does anyone know of any guidelines for this that are recent and practical? Also should i ask to be renumerated for this work? Ive already done the diabetic procotol and updated their anesthesia pathway and documentation for free... Poly pharmacy and falls
Long story short, I had an older patient on sedating meds which I had been working to minimize. Last time I saw her she was having poor PO intake and I focused on enhancing that and getting her to be medically evaluated in the ER because she is also diabetic on insulin (which she did not follow through on, and it was telemedicine so I could not arrange direct transport). Blood glucose level to cancel cataract surgery
Thoughts on this? What would be your BG range and cut-offs? Recently I cancelled one whose BG was 400-500 mg/dL. Pt claimed to be compliant with all her diabetic medications. Talked to her PCP and found out she was lying and that she actually ran out of her humalog a month ago. No recent A1c.. given the hx I can assume her BG in the 400-500's range for the past month.
