Is getting people off weight loss medications the right move?
By RICHARD FRANK Demand for GLP-1 medications soared last year and shows no signs of stopping in 2024. Employers and health plans are understandably anxious about how long they should expect to pay for these pricey drugs. They’re itching for an easy off-ramp. Some solutions are cropping up to pave the way. Many of them claim they can help patients reap the benefits of GLP-1s within a short time frame, and get them off the drugs within 12 months to save costs. But the data doesn’t support that promise. In fact, studies suggest some patients may need to stay on the drugs indefinitely to sustain outcomes while other...

In conclusion, we found that elevated central aortic stiffness is associated with a greater decline in kidney function in old age. Since aPWV and cSBP both appear to be predictors of eGFR decline, it might be of interest to identify older individuals with elevated aortic stiffness. In this specific population, intensive blood pressure reduction might be justified in order to slow down the process of vascular aging and prevent kidney function decline. « Back to Top Cellular Senescence in Neurodegenerative Conditions https://www.fightaging.org/archives/2024/04/cellular-senescence-in-neurodegenera...

Juventology as an Complementary Alternative to Gerontology
There are many different ways to conceptualize programs of research and development aimed at the treatment of aging. The Strategies for Engineered Negligible Senescence (SENS) is focused on aging as damage accumulation, and treatment is thus damage repair: remove senescent cells, restore mitochondrial function, clear out harmful protein aggregates, and so forth. Programmed aging viewpoints instead focus on ways to alter what are suspected to be evolved programs that drive aging, with this line of thought most often centered around the reversal of epigenetic changes that are observed to occur with age. In today's ope...

Lean Digital: General and Long-Term Considerations for Obesity
This series of articles has covered the ways that digital technologies contribute to weight loss. To round out the series, this final article covers general topics applying to the field. Maintaining Success Both surgery and medications help some people lose weight, but a lot of patients gradually gain it back. Some of the technology solutions I cover in this article can be used either by themselves in behavioral changes for weight loss, or to maintain a desired weight after other medical interventions. Virta Health is an example. A recent study showed that it can help people go off of GLP-1 drugs and maintain the weight ...

Lean Digital: How Tracking Feeds Into Behavior Change for Weight Loss
This article is part of a series on today’s philosophies regarding weight change and how digital interventions can help. In this article, we’ll look at the contribution of tracking. The article looks at standards, making sure data meets a useful need, and some particular applications of tracking to metabolic control and diet. Finding Meaning in Data Device manufacturers are finding more and more things to track. While some people love measurements such as steps taken through the day, and there’s even a site called Quantified Self exploiting this fad, most people don’t care about data unless they s...

Lean Digital: Behavior Change for Weight and Obesity
This article, part of a series about obesity, interviews a number of experts to explore the philosophy and practice of behavioral therapy for obesity. Medical and Behavioral Interactions Dr. Luke Twelves, medical director of the contract research organization Lindus Health, says, “Obesity is a multifactor disease.” He believes that technology helps people take control of their own treatment. For instance, a continuous glucose monitor (CGM) allows patients to decide what food or medication they need at each moment in the day. A similar viewpoint was cited by Eric Bormel, a director in the Healthcare Group at S...

Lean Digital: How AI Drives Treatment Recommendations for Weight Loss
The first article in this series laid out what we know about body weight and obesity today. The rest of the Lean Digital series will look at some contributions that digital technologies are making toward solutions. The best contribution that information can make to weight management is to find more effective, personalized treatments: to match the right person to the right treatment, whether it be surgery, drugs, diet, exercise, or some combination. AI is turning up intriguing successes in this endeavor. I talked to Dr. Ronald Razmi, co-founder and managing director at Zoi Capital and author of the book AI Doctor: The Rise ...

Should you take medications for high blood pressure lifelong?
The answer depends on what caused your high blood pressure. If it is a reversible cause which can be removed, then of course medications can be stopped. But if a reversible cause has not been found, you should be cautious about stopping medications. What are the reversible causes of high blood pressure? A transient drop in kidney function due to some illness (acute kidney injury) can raise the blood pressure. But when the kidney problem settles, blood pressure normalizes, and it may never recur. In this situation blood pressure medication can certainly be stopped. There are certain rare tumours which secrete hormones which...

In conclusion, we assigned stemness scores to human samples and show evidence of a pan-tissue loss of stemness during human aging, which adds weight to the idea that stem cell deterioration may contribute to human aging. « Back to Top The Role of Immune Aging in Neurodegenerative Conditions https://www.fightaging.org/archives/2024/04/the-role-of-immune-aging-in-neurodegenerative-conditions/ The research community has come to see chronic inflammation and other age-related immune system dysfunctions as an important aspect of neurodegenerative conditions. Calorie Restriction and Fasting Benefit the Aging Heart
In conclusion, fasting-induced autophagy is beneficial for ensuring cardiac function, preventing disease, and improving longevity. However, additional studies in vivo in animal models of cardiac aging are needed to determine the specific molecular mechanisms involved in normalizing autophagy by fasting. In addition, large-scale studies on humans are needed. Importantly, further in vitro research should be directed toward human cardiac tissues to better understand the molecular mechanisms of fasting-induced autophagy and its beneficial effects on longevity pathways and prevention of cardiovascular disease. Reviewing Approaches to Improving Aged Stem Cell Function
A variety of approaches show some promise in improving the function of stem cells in aged tissues. Stem cell populations support their tissue by providing a supply of daughter somatic cells to replace losses. This supply diminishes over time as stem cells reduce their activity for reasons that descend from the known root causes of aging, but which are not fully understood in detail. To the degree that reduced stem cell function is a response to the aged environment rather than a consequence of damage inherent to these cells, then it is useful to find ways to force stem cells to be more active. Whether this is the case may ...

Ozempic And Other Weight Loss Drugs Will Only Work With Digital Health
Weight loss injections are now widely used as miracle drugs for effortless skinniness. But without digital health, they just won’t work in the long run. The U.S. Food and Drug Administration, the FDA, approved Wegovy, a semaglutide injection for chronic weight management in adults with obesity or overweight with at least one weight-related condition for use in addition to a reduced calorie diet and increased physical activity in 2021.  Semaglutide was originally used for the treatment of type 2 diabetes, and was found to have a major lucky side effect: being very effective in assisting weight loss. Semaglutide inject...
