Zap-X and SRS
I know it was discussed a while back, but it has been a while and I wonder if anyone has had recent experience using Zap-X for brain radiosurgery. Specifically, are people treating more than 2-3 brain mets, and how is the quality of the treatment planning? Would appreciate any thoughts/feedback! (Source: Student Doctor Network)

USUHS vs HPSP specialty options
Question for the group. When it comes to non primary care, selective medical specialities such as Ortho, ENT, optho, etc, is it easier to match into the specialty of your choice if you demonstrate your full throated embrace of military medicine by attending USUHS vs a more reluctant approach to the military if you do HPSP? It seems I notice more people getting competive residencies such as orthopedics, neurosurgery, etc if they went USUHS vs HPSP, but I don't know for certain and so I... Hiring Non-Residency Trained FMG's Instead of Mid-Levels...Implications for Pain?
More States Cut Training Requirements for Some International Medical Graduates Alternative pathways to licensing for IMGs spreads beyond Tennessee www.medpagetoday.com Does anyone have experience with this? I'm aware of one pain group that hired a foreign neurosurgeon to be their in-house pain surgeon/first assist at their ASC. "Bryan Carmody, MD, MPH, a pediatric nephrologist at Eastern Virginia Medical School in Norfolk, who has long studied data on residency and the Match process, said that the influx of labor would be good for academic me...

Applying to Advanced Neurology Position (pgy2) After Transitional Year?
Went unmatched but got a TY. The acgme website says that in order to be eligible for a neuro residency your pgy1 year must include: "Eight months in internal medicine with primary responsibility in patient care; or, Six months in internal medicine with primary responsibility in patient care and a period of at least two months comprising one or more months of emergency medicine, family medicine, internal medicine, or pediatrics" I think I can schedule my TY year to meet either of those... How much would low board scores from med school impact application for sub-specialties?
Let's say you're an IM trying to get into competitive specialties like Cards. Or let's say you're a Derm wanting to apply for Moh's. Or a Radiologist wanting to do Neuro fellowship. EDIT: I meant, Steps 1 and 2 (because they were in medical school). (Source: Student Doctor Network)

Basic vs. clinical research
Hi, I am a medical student currently interested in either neurology or neurosurgery. I understand that research is very important for applying to neurosurgery. I have a background in basic neuroscience research from before I came to medical school, and I am now getting involved in clinical research. Is it suggested to pursue a balance of both clinical and basic science research, or does it matter? Is one more respected over another? I see lots of people saying to just do clinical research... VA occlusion
Have a patient with moderate cervical stenosis and clear radicular pain. Had planned to do a CESI. His c spine MRI and later CT angiogram revealed a major blockage of the entire left VA as it passes through the neck. He didn’t have any CN deficits when I saw him last month. Thoughts on him ? No acute neurological deficits so I was thinking of sending him to neurology but they have a 2 month wait. Maybe just send him to ER in major city so he can get a same day neurology consult? Re-SRS in a patient who has had WBRT?
All, wondering about precedence for this. I haven't treated this patient before, but to sum up, Extensive stage small cell initially with 2 brain mets. 30 Gy in 10 fx WBRT in March of 2019. Left cerebellar progression in May 2020 for which he got 18 Gy x 1. Since that time, he's been stable until recently when he had evidence of progression in the left cerebellum. Not rushing into anything, as I'm not entirely convinced this represents recurrent disease, but mostly curious if anyone has... Read more (Source: Student Doctor Network)

Leg cramps
Have a patient with moderate lumbosacral stenosis that I did bilateral S1 tfesi about 6 weeks ago with good relief of radic symptoms but consistent cramping throughout legs after the procedure. Encouraged hydration and after two weeks symptoms persisted. EMG was normal, started low dose neurontin without much benefit. Any thoughts on this? More time? Referral to neurology? Is it difficult to match back to Chicago for Neuro/IM?
I grew up in Chicago and have regional ties there but my fiancé is back in Chicago as a DPT and I would love to match back there! I currently attend WCUCOM and we have matched back in Chicago/midwest but not for Neuro/IM. I know I will have to do well on boards and do auditions back home 4th year but is Chicago a realistic goal attending a DO school in the south? I would be fine with MI and WI as well! Thank you! Being 2nd Quartile (Bottom Half)
I think Im smack down in the middle, maybe a little bit to the lower half. I know preclinical grades don’t matter, but assuming I end up in the second quartile (bottom half), is there any specialty that would view this as a negative? I’m a DO, and I’m interested in academic IM, neuro, anesthesia, and maybe gen surg. Thanks. (Source: Student Doctor Network)
