Middle-age men in Northern Kentucky who hope to regain their youthful vitality might soon be able to get testosterone injections at a Low T Center without having to pop across the Ohio River. Mike Sisk, the president of the rapidly expanding national chain that operates the Low T Center in Mason, tells me that he is “actively seeking locations … on the Kentucky side of Cincinnati.” It would be the first Low T Center in the Bluegrass State. Clinical Management of Youth with Gender Dysphoria in Vancouver
Conclusion: Treatment with gonadotropin-releasing hormone analog and/or cross-sex hormones, in collaboration with transgender-competent mental health professionals, is an intervention that appears to be appropriate in carefully selected youth with gender dysphoria. Cyclosporine and tacrolimus have inhibitory effects on toll‐like receptor signaling after liver transplantation
In conclusion, patients on calcineurin inhibitors had impaired inflammatory cytokine production in response to TLR2, TLR4, and TLR7/8 stimulation compared comparison with healthy controls. Importantly, TAC and CYA appear to have different effects on TLR signaling. Impaired TLR function has important repercussions for risk of infection, graft rejection, and disease recurrence after transplantation, and the different immunosuppressive profiles of CYA and TAC may guide the choice of therapy to improve disease outcomes. We thank Dr Lin for the helpful comments. EGD and capsule endoscopy (CE) identified the etiology in 62 of 83 patients (75%) in our cohort. Of the remaining patients, 2 of 21 (9.5%) had an underlying colorectal cancer, 1 of 21 patients (4.8%) were subsequently found to have gastric angiectasia, and 18 of 21 patients (85.7%) had no cause identified. Post–Liver Transplant Leptin Results in Resolution of Severe Recurrence of Lipodystrophy‐Associated Nonalcoholic Steatohepatitis
We describe the first case of a patient undergoing orthoptic liver transplantation for acquired generalized lipodystrophy‐related nonalcoholic steatohepatitis who developed severe recurrence of nonalcoholic fatty liver disease in the first few months posttransplant but responded rapidly to the administration of exogenous leptin. Electronic estimations of renal function (eGFR) are inaccurate in solid-organ transplant recipients and can result in significant under-dosing of prophylactic valganciclovir.
Abstract In a prospective study of solid-organ transplant recipients (n=22; 15 hepatic/7 renal) receiving valganciclovir for cytomegalovirus (CMV) prophylaxis, electronic estimation of glomerular filtration rate (eGFR) underestimated the true GFR (24 hr urine creatinine clearance) by >20% in 14/22 (63.6%). Its use was associated with inappropriate under-dosing of valganciclovir while the Cockroft-Gault equation was accurate in 21/22 (95.4%).
