Is this too controversial for a secondary?
In one of the shelters I volunteered at, we gave out cigarettes to clients to encourage harm reduction and ensure that clients facing addiction didn't put themselves in more dangerous/harmful situations. We also pointed them to resources to get medical help (which we ourselves were unqualified to provide). I know that some adcoms tend to be pretty politically conservative, and I wasn't sure whether this would be considered too controversial to include in my secondary. Thanks in advance! Case: Esophageal Cancer with weird nodes
Hello! I have a case for you and looking forward to hearing your input. Male patient in his sixties, known history of tobacco use. Now presents with swallowing difficulties and is diagnosed with a cT3 G3 SCC of the thoracic esophagus. No immediate regional nodes affected, but two findings on PET-CT: one node in the uppermost mediastinum (almost SCV) and a few other nodes grouped together in the coeliac region. The first biopsy of the tumor was negative and compliance was an issue, so it... Read more (Source: Student Doctor Network)

Drug offense applying to elite schools
Hi all, I have been charged with 2 petty offenses at a National park - procession and under the influence of a controlled substance (weed). Long story short, I was smoking with a couple friends in the woods and a ranger caught us. There was no arrest, just issues fines. It’s a petty offense and I was given the option to fight it in court but have given up on that option, and the 2 petty offenses will be on my federal record for 5-7 years. Is this something that the AMCAS background check... Read more (Source: Student Doctor Network)

Non-oropharyngeal HNSCC and HPV
I was wondering what your take is concerning HPV in non-oropharyngeal HNSCC. Data seem to point out towards better prognosis, but I have not found any deescalation trials looking at non-oropharyngeal HNSCC. I saw a new patient with an oral cavity HNSCC post surgery who has never smoked. HPV status had not been determined so far and the pathologist suggested to determine it. I am wondering if this result would influence my management. The indication for RT is clear in this patient due to... Read more (Source: Student Doctor Network)

Clozapine dosing
I was just curious on how others are dosing clozapine. It has a pretty wide ranging half life, smoking can affect it, all kinds of other variables, etc. I know prescription recommendations are to dose bid, but I almost never do this. My own practice has been to start at bedtime and titrate upward only at bedtime. Rarely, if they are struggling with daytime psychosis I'll add a smaller dose in the mornings, but usually only 25 or 50 mg, with the bulk being at bedtime. When I've tried to... Long-Term NRT
Are there issues with maintaining patients on NRT long term? Obviously there are very many reasons to get patients off tobacco, and probably good reasons to get off vaping, but what about patients on nicotine patch/gum? Are there significant psychiatric or medical problems from long-term use of nicotine itself? It's been hard to find good information because much of the literature conflates tobacco and nicotine. Also, has anyone seen a case or have literature regarding prolonged/complex... Read more (Source: Student Doctor Network)

(Spoiler) AAMC sociology q13
I sort of understand why the answer is B, but AAMC answer explanation says answer choice A is conflict theory, C is rational choice theory, and D is functionalist theory. Can someone please understand how these theories match the answer choices?
