Does this sound accurate? Hierarchy of single doctor private offices.
From top to bottom: Owner dentist Office manager Front desk staff Hygienist Assistant Janitor The dog Dirt Crud Gum on the back of the shoe Cigarette butt on pavement in the parking lot The grime in the sink Mold in ceiling Turd in the toilet bowl Associate dentist And after a few years, the associate can work his/her way up above "Dirt" . "Dog" will always be favoured over associate. (Source: Student Doctor Network)

Nasal vestibular carcinoma
This is a case that checks disparate boxes and is rare. Thus it makes it a little confusing. 59yo male in excellent health. Non smoker. No drug use. Perhaps 40yo ago had some sort of injury and broke nose and cartilage was exposed or something like that. Never quite healed for a while, in vestibular region. About 6 mos ago had a right neck node suddenly appear. Asymptomatic. Had a simple but complete excisional biopsy of that by community ENT. Was SCC p16+. N1, level 2, maybe 3. No adverse... Another purely hypothetical scenario
60ish male needs cysto w/ biopsies for staging of bladder cancer. 6.4cm ascending thoracic aortic aneurysm found incidentally on CT as part of work-up. 40 pack year smoker, no features that scream Marfan, ehlers danlos, etc. Patient seen by medicine who says “patient is optimized for surgery”. Seen by thoracic who says, patient can proceed under anesthesia and will consider repair if prognosis from cancer is reasonable based on staging, biopsies, etc. You’re assigned the case and despite... Another Cardiac Case
I always enjoy the case presentations around here and it's been a while since I contributed, so here was a fun case 54 y/o male underwent Sternotomy for CABG/MVR/MAZE/LLAA 2 months ago after presenting with an NSTEMI. BMI 42, recent hx of Meth and Marijuana use, current everyday smoker with questionable COPD (PFTs were fine), OSA on BiPAP while at hospital (non-compliant at home), and AFib on Elliquis. Surgery went uneventfully, had a Mitral Valve Repair (simple ring), good flow in... Another Cardiac Case (Source: Student Doctor Network)

Waiver Question
I'm planning to join the military (Army) and would appreciate any insight into whether I would need a waiver/what I can do to improve my chances of being able to join. I smoked marijuana approximately 5 times over 10 years ago. At the time, my parents found out and had me meet with a drug counselor. There was no diagnosis of a substance use disorder but the office/clinic where I met with the counselor no longer has any records on file due to length of time (and I have an official letter from... The Happy Happy Joy Joy Thread
OK we've gotten a little negative and cranky on here lately, myself (especially?) included. So I'm starting this thread where we can post happy things happening in our lives, share joyous anecdotes, trade recipes, and possibly reference Ren and Stimpy repeatedly. I'll start. Just bought a new grill/ smoker which I'm having a fantastic time using. Was able to get my hands on some good cuts of sockeye salmon and, after adding sea salt, pepper, paprika, and a smidge of olive oil, smoked... Case discussion
Had a weird case about a month ago, curious to see what others would do, and differential diagnosis, investigative workup etc. Identifying details have been changed to protect confidentiality. I will start the post with all the details when I took over her care. I can post the autopsy results at the end. This was a 77 year old woman, admitted with acute abdominal pain. PMHx of hypertension and gout, appy/chole. Living independently, married, non smoker, reasonably healthy lady. CT scan of... Case discussion (Source: Student Doctor Network)

UWorld Q 7601 Respiratory physiology
Pt evaluated for worsening SOB on exertion, 40-pack-year smoking hit. Lung are hyperresonant on percussion and there is diffuse wheezing on auscultation. CXR reveals a flattened diaphragm with a narrow mediastinum, there are decreased bronchovascular markings in the peripheral lung region. Why this is COPD? how to tell it is not restrictive lung diseases ? (Source: Student Doctor Network)
