Not allowed to attend orientation?
Posting this for a friend... Signed a contract, contract stated something to the effect of this being a tobacco-free program, and evidence of the contrary would result in termination. So, urine gets dipped and is positive for nicotine and metabolites. The program director is notified and informs the orientee to not attend orientation until further notice. What's your guys' take? I think the orientee should wait until further notice (reportedly at noon the next day) since it's a hassle... Not allowed to attend orientation? (Source: Student Doctor Network)

Mentioning smoking in essay

Question about proceeding w case.
Just curious if you guys would proceed w the case or cancel. Case is lap radical nephrectomy for cystic neoplasm. 60s years old avg sized, has mild copd , htn , smoker/drinker, had SOB 3 months ago found to have large anterior MI on EKG . tte per cardiologist note said 31% ef and lv aneurysm, etc and cleared him. According to patient he said he had stress test sometime after the MI and he thinks it was normal.. Labs today normal. Does get SOB w some exertion. Got a Aicd placed couple week... Modern Medicine does little to extend life
Elsevier: Article Locator Great article discussing how life is really only extended through smoking cessation, physical activity, stress reduction, weight control and good sleep. Dear collegues, Your opinion is needed plz. 70 yo male, smoker may 2016: radiochemotherapy for T3N1 supraglottic squamous cell carcinoma He received 70Gy in 35 fractions to the GTV,60Gy to the intermediate risk regions and 50 Gy to the low risk PTV, 3DRT +IGRT ( could not afford IMRT. In my country VMAT is not covered) He kept smoking and drinking Good response to CRT March 2017: he relapsed with 2cm transglottic lesion and underwent total laryngectomy with bilateral neck dissection... Attendings... were you absolutely smoked after residency?
The flogging has finally caught up to me. It freaks me out bc I'm about to graduate and have zero energy. Between working my ass off as a resident, research, studying for boards, and moonlighting to pay off the mountain of debt I have little in the tank left. Now, I am trying to get motivated to hit the ground running as an attending and it's a little intimidating to say the least. You guys catch your second wind after you transition to attendinghood?
