Clinical Correlates and Prognostic Value of Elevated Right Atrial Pressure in Patients With Hypertrophic Cardiomyopathy.
CONCLUSIONS: Elevated RAP in patients with HCM is associated with left-sided heart failure and is an independent predictor of all-cause mortality and new-onset AF. PMID: 29526913 [PubMed - as supplied by publisher]

Levitronix bilateral ventricular assist device, a bridge to recovery in a patient with acute fulminant myocarditis and concomitant cerebellar infarction.
We report on the case of a 27-year-old male who presented to our emergency room with chest tightness, dyspnoea and cold sweats. The 12-lead electrocardiogram showed diffuse ventricular tachycardia with wide QRS complexes. Troponin-I level was elevated to 100 ng/ml. The coronary angiogram showed good patency of all three coronary vessels, and acute fulminant myocarditis was suspected. The patient underwent cardiopulmonary resuscitation in the catheter room and high-dose inotropic support was initiated to stabilise his haemodynamic status. After resuscitation, the patient was in a coma and acute stroke was highly suspected. ...

Long-term outcome of catheter ablation and other form of therapy for electrical storm in patients with implantable cardioverter-defibrillators
ConclusionsIn patients treated with RFCA for ES, all-cause mortality was significantly lower compared to the group treated with other methods.

Diagnostic yield of asymptomatic arrhythmias detected by mobile cardiac outpatient telemetry and autotrigger looping event cardiac monitors
ConclusionsIn patients with conventional diagnostic monitoring indications, MCOT™ had significantly higher diagnostic yields for five asymptomatic arrhythmias compared to the AT‐LER.This article is protected by copyright. All rights reserved

A congenital diverticulum of the left ventricular apex manifested by stroke and recurrent ventricular tachycardia
Left ventricular (LV) outpouchings include acquired pathologies comprising ventricular aneurysm or pseudoaneurysm and congenital abnormality ventricular diverticulum (CVD) [1 –3]. CVD is a rare cardiac malformation consisting of a localized protrusion from the free wall of the LV. Although it is often associated with other cardiac and extracardiac congenital anomalies, it may also present alone, as an incidental finding in adult patients. Although very rare, CVD may be associated with significant morbidity and even mortality.

Clinical significance of platelet reactivity during prasugrel therapy in patients with acute myocardial infarction.
CONCLUSIONS: It is feasible to treat AMI patients with prasugrel. HTPR is a significant independent risk factor for adverse events in AMI patients receiving prasugrel after primary PCI. PMID: 27836373 [PubMed - as supplied by publisher]

Gender Discrepancy in Diabetic Patients Hospitalized With Heart Failure: Does Age Matter?
Conclusions: In DHF, female gender is characterized by having a high prevalence of metabolic syndrome components. Also, females are more likely to have better Left ventricular ejection fraction but less likely to receive cardiovascular evidence based medications. There is no significant difference in the overall hospital mortality between both genders, however, in the younger age; males have a significantly higher mortality.

A novel cross-sector telemedical approach to detect arrhythmia in primary care patients with palpitations using a patient-activated event recorder.
CONCLUSIONS: The investigated cross-sector telemetric network is a feasible approach to detect arrhythmia in patients with palpitations and may have high impact on further treatment, notably in those at risk for stroke due to AF. PMID: 27320955 [PubMed - as supplied by publisher]

Ventricular Tachycardia Ablation in the Presence of Left Ventricular Thrombus: Safety and Efficacy
Conclusion: Initial data suggest ablation of VT in the presence of intracavitary thrombus is feasible, is associated with a similar success rate to historical studies in patients without thrombus, and has an acceptable risk of complications given the high risk nature of patients with ES. Further data is needed; however, the presence of a laminated thrombus should not necessarily preclude ablation in patients who have failed medical therapy for VT in whom ablation is otherwise indicated. This article is protected by copyright. Dabigatran for left ventricular thrombus.
Abstract Male patient in dilated phase of hypertrophic cardiomyopathy had multiple hospitalizations during the past 2 years either due to congestive heart failure, stroke, scar epilepsy, or atrial fibrillation and ventricular tachycardia. Medication included evidence based therapy for heart failure, cordarone and warfarin. Anticoagulation had to be discontinued due to marked fluctuations in INR. Transthoracic Echocardiography (TTE) revealed a mobile mass in the left ventricle. He was treated with Dabigatran 110mg twice a day for 4 months without any bleeding or embolic episode and complete resolution of thrombus. ...

Interruption of Pacing Following Nonsustained Ventricular Tachycardia in an AAI Programmed Implantable Cardioverter Defibrillator
ConclusionsUnintended consequences can occur when complex VT detection parameters interact with specific pacing modes. At times, nonconventional programming can avoid these interactions while still achieving effective AAI(R) pacing.

A decade has passed since the previous survey of the epidemiological and clinical characteristics of Japanese HCM patients. The Aichi Hypertrophic Cardiomyopathy (AHC) Registry is based on a prospective multicenter observational study of HCM patients. The clinical characteristics of 42 ambulant HCM patients followed up for up to 5...
