Operative Technique and Experience of One Referral Center With Vaginal Cesarean Delivery.
CONCLUSION: Vaginal cesarean delivery is a valuable technique for rapid fetal extraction of deceased or nonviable fetuses for severe maternal indications. PMID: 31923057 [PubMed - as supplied by publisher]

142: Migraines in pregnancy and select adverse pregnancy outcomes
The objective of this study was to assess risk factors associated with maternal migraine, and the risk of maternal stroke, postpartum hemorrhage, and preterm birth in a large California birth cohort.

1137: At what blood pressures do hypertensive women develop severe maternal morbidity: the  clinical inflection point
Sustained severe hypertension (systolic blood pressure, SBP,>160 mmHg and/or diastolic blood pressure, DBP,>110 mmHg for 15 minutes) is a risk factor for severe maternal morbidities (SMM), as defined by the Centers for Disease Control (CDC). However, evidence for morbidity at these pressures is based on limited studies of peripartum stroke. We aimed to determine whether specific hypertensive parameters exist above which SMM increases.

Temporal Summation in Chronic Pelvic Pain.
CONCLUSION: Women with chronic pelvic pain and allodynia show significant worsening of pain with successive strokes of a cotton-tip applicator, thus indicates that wind-up and central sensitization are present in women with chronic pelvic pain and allodynia. Identification of summation is further evidence of neuroplasticity, which is helpful in innovative therapies for chronic pelvic pain. PMID: 31882288 [PubMed - as supplied by publisher]

Treatment of severe hypertension during pregnancy: we still do not know what the best option is.
Authors: Wertaschnigg D, Wang R, Reddy M, Costa FDS, Mol BWJ, Rolnik DL Abstract Intracranial hemorrhage and stroke are primary causes of maternal mortality in pregnancies affected by hypertensive disorders. As such antihypertensive therapy plays a crucial role in the management of severe hypertension. However, the target level to achieve the best outcome for both - mother and fetus - is still unclear. Moreover, given the lack of well-designed randomized controlled trials with standardized key outcomes, the current choice of antihypertensive medications depends rather on clinicians' preference. Furthermore, data on...

Expectant Management of Hypertensive Disorders of Pregnancy and Future Cardiovascular Morbidity.
CONCLUSIONS: Prolonged expectant management of preterm hypertensive disorders of pregnancy is associated with an increased risk of maternal cardiac disease in the ensuing years. PMID: 31809423 [PubMed - as supplied by publisher]

Development of a comprehensive health-risk prediction tool for postmenopausal women
Conclusion: This study represents the first large-scale study to develop a risk prediction calculator that yields health risk prediction for several outcomes simultaneously. Development of this tool is a first step toward enabling women to prioritize interventions that may decrease these risks. Video Summary:http://links.lww.com/MENO/A463.

Promoting Cardiovascular Health in Midlife Women
AbstractPurpose of ReviewCardiovascular disease (CVD) remains the number one cause of death in the USA. In women, coronary heart disease (CHD) accounts for 22% of deaths with an additional 6.2% of deaths secondary to stroke. The prevalence of CVD increases as women age; after the age of 75, the incidence will exceed that reported for men. The risk for CVD in women is systematically underestimated. Both healthcare providers and women need a better understanding of the mechanism and role of traditional CVD risk factors (CVDRF) in women, and the newer identified non-traditional CVDRF which are unique to women and the resultin...

A review of stroke in pregnancy: incidence, investigations and management
AbstractKey contentThe incidence of stroke in young and middle ‐aged adults is increasing, with pregnancy‐related strokes occurring in 30 in 100 000 pregnancies; strokes are three times more common among pregnant than among nonpregnant individuals aged 15–44 years.The investigation and management of stroke is changing because of the time ‐sensitive benefits of thrombolysis and thrombectomy in the acute management of ischaemic stroke. The approach to clinical assessment and investigation of a pregnant woman presenting with suspected stroke is not different from that of a non ‐pregnant patient:; timely brain imagi...

Scientific pathophysiology of intrapartum fetal hypoxemia and cardiotocography pattern recognition –realignment is a basic prerequisite!
Turner et  al1 highlight knowledge accrued since the 1960s that 75% cases of fetal hypoxemia result from contraction-induced reduction in uteroplacental perfusion by up to 60%. Following anomalies oblige focused explicit clarification by the authors. The review maintains a misconception that the chemoreflex would increase the cardiac output.1 However, with a significant fetal heart rate (FHR) deceleration intended to reduce cardiac workload and oxygen consumption, the stroke-volume could increase, but the cardiac output per minute would drop albeit preferentially redistributed to the brain.

Combined hormonal contraception and migraine: are we being too strict?
Purpose of review Combined hormonal contraception has been contraindicated in migraines, especially in migraines with aura, because of ischemic stroke risk. Newer formulations are now available and physicians may unnecessarily be limiting access to contraceptive and medical therapeutic options for patients with migraines. This review summarizes the available data regarding ischemic stroke risk of modern combined hormonal contraception in the setting of migraines. Recent findings Limited data exists on current formulations of combined hormonal contraception and outcomes in migraine patients. Studies indicate ischemic s...

Lipid profile differences during menopause: a review with meta-analysis
Conclusions: These findings are important as they provide precise estimates of lipid differences in women around menopause. Furthermore the results suggest that the unfavorable lipid profile that develops in postmenopausal women puts them at higher risk of cardiovascular disease such as heart disease and stroke if appropriate lifestyle/pharmacological interventions are not implemented.
