Stroke and mortality rates after off-pump vs pump-assisted/no-clamp coronary artery bypass grafting
CONCLUSIONS: A hybrid strategy incorporating off-pump, pump-assisted, and combined offpump/pump-assisted techniques achieved very low stroke rates in patients undergoing coronary revascularization. Perioperative mortality was similar for all three techniques. Avoiding aortic clamping may be crucial for decreasing CABG-related stroke rates. Off-pump/no-bypass surgery had no significant advantage over the pump-assisted/no-clamp or combined techniques in reducing the stroke rate after coronary artery bypass grafting surgery.PMID:36168952 | DOI:10.23736/S0021-9509.22.12337-2

Clampless off-pump surgery reduces stroke in patients with left main disease
Conclusions: This study shows the superiority of OPCAB for patients with LMD with regards to risk-adjusted outcomes other than mortality. A “clampless OPCAB strategy”, effectively reduces stroke yielding similar early outcomes as PCI.

Aspirin Instead of Oral Anticoagulant Prescription in Atrial Fibrillation Patients at Risk for Stroke
ConclusionsIn a large, real-world cardiac outpatient population of AF patients with a moderate to high risk of stroke, more than 1 in 3 were treated with aspirin alone without OAC. Specific patient characteristics predicted prescription of aspirin therapy over OAC.

Off-Pump Coronary Artery Bypass Reduces Early Stroke in Octogenarians: A Meta-Analysis of 18,000 Patients.
CONCLUSIONS: Coronary artery bypass in octogenarians can be performed safely with low early mortality. Although off-pump operations reduce the risk of early stroke, all other adverse events are comparable in on- and off-pump coronary artery bypass operations. Influencing Factors for Early Acute Cerebrovascular Accidents in Patients with Stroke History following Off-pump Coronary Artery Bypass Grafting
Conclusions: This study indicated that patients with severe bilateral carotid stenosis, the left ventricular ejection fraction ≤35%, the postoperative acute myocardial infarction, postoperative atrial fibrillation and postoperative hypotension were more likely to suffer from early acute cerebrovascular accidents when they received OPCAB. Application of Enclose® II proximal anastomotic device may decrease the incidence of early acute cerebrovascular accidents during OPCAB.

Conclusion: Coronary artery bypass in octogenarians can be performed with low early mortality. Number of grafts is lesser in the OPCABG cohort. While stroke rates are higher with conventional surgery, all other adverse events are comparable. Future randomised trials are needed to define the role of off-pump surgery in this high-risk cohort.
