EuroSCORE II and N-terminal pro-B-type natriuretic peptide for risk evaluation: an observational longitudinal study in patients undergoing coronary artery bypass graft surgery
Conclusions Combining EuroSCORE II and preoperative NT-proBNP appears to improve risk prediction with regard to severe circulatory failure after isolated CABG for ACS. NT-proBNP may be particularly useful in patients at intermediate risk according to EuroSCORE II. Clinical trial registration NCT00489827.

Who might benefit from early aspirin after coronary artery surgery?
A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was whether early administration of aspirin might optimize vein graft patency. More than 250 papers were found using the reported search, of which 4 new papers in addition to the previous 7 represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers are tabulated. Early postoperative aspirin administered within 6 h following coronary artery bypass grafting (CABG) has been show...

A specialized post anaesthetic care unit improves fast-track management in cardiac surgery: a prospective randomized trial
IntroductionFast-track treatment in cardiac surgery has become the global standard of care. We compared the efficacy and safety of a specialised post-anaesthetic care unit (PACU) to a conventional intensive care unit (ICU) in achieving defined fast-track end-points in adult patients after elective cardiac surgery. Methods: In a prospective, single blinded, randomized study, 200 adult patients undergoing elective cardiac surgery (coronary artery bypass graft (CABG), valve surgery or combined CABG and valve surgery), were selected to receive their postoperative treatment either in the ICU (n?=?100), or in the PACU (n?=?100)....

126 * extra-anatomic revascularisation for distal occlusion of the left or right common carotid artery in acute type a aortic dissection with cerebral malperfusion
Conclusion: Extra-anatomic bypass for distal LCCA or RCCA occlusion presents a valuable bail-out technique to restore cerebral perfusion during AADA repair, potentially reducing the increased incidence of neurological complications in patients with cerebral malperfusion.

264 * results of total arterial versus conventional versus hybrid myocardial revascularisation: a propensity match analysis of long-term follow-up
Conclusion: Total arterial revascularisation provides improved outcomes at mid-term follow-up compared with conventional or hybrid revascularisation. The latter technique is particularly associated with a significantly higher incidence of late myocardial infarction and repeat revascularisation.

Results of “elephant trunk” total aortic arch replacement using a multi-branched, collared graft prosthesis
We report on our experience with a simplified elephant trunk (ET) procedure with a multi-branched prosthesis (Vascutek® Siena™ Collared Graft). It consists of a proximal portion (20 cm) with prefabricated side branches, a collar and a distal portion (30 cm). The collar, which can be trimmed into any desired diameter, constitutes the suture portion to the descending aorta. Radiopaque markers in the distal portion indicate the landing zone. Between January 2011 and June 2013, 20 consecutive patients (10 women; mean age, 66 ± 9.3 years) underwent ET procedure, including 6 re-do cases. Underlying aortic diseases were ...

The safety of deep hypothermic circulatory arrest in aortic valve replacement with unclampable aorta in non-octogenarians
CONCLUSIONS Postoperative neurological events and operative mortality were, respectively, 3- and 4-fold higher in octogenarians undergoing AVR using DHCA. Such patients may represent suitable candidates for TAVR if favourable outcomes are demonstrated in patients with atherosclerotic aortas. Surgical AVR remains the standard treatment option with excellent outcomes for patients <80 years old with unclampable aortas.

Operative Mortality and Complication Risk Model for All Major Cardiovascular Operations in Japan
Conclusions These risk models increased the discriminatory power of former models. Thus, our models can be said to reflect the current state of Japan. With respect to major complications, useful feedback can now be provided through the Japan Cardiovascular Surgery Database Web-based system.

Outcome of Early Revascularization Surgery in Patients with ST‐Elevation Myocardial Infarction
ConclusionsIn patients with ST‐elevation myocardial infarction who required emergency coronary artery bypass surgery, there was no difference in procedure complications or mortality between early (within 24 hours) or later (more than 24 hours). That was noted at one month and one year after the index myocardial infarction.

The impact of anesthesiologists on coronary artery bypass graft surgery outcomes.
CONCLUSIONS: The rate of death or major complications among patients undergoing coronary artery bypass graft surgery varies markedly across anesthesiologists. These findings suggest that there may be opportunities to improve perioperative management to improve outcomes among high-risk surgical patients. PMID: 25695571 [PubMed - in process]

Should we consider beating-heart on-pump coronary artery bypass grafting over conventional cardioplegic arrest to improve postoperative outcomes in selected patients?
A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was whether beating-heart on-pump coronary artery bypass grafting (BH-ONCAB) offered superior mortality and morbidity outcomes when compared with conventional on-pump coronary artery bypass grafting (C-ONCAB). Morbidity outcomes consisted of renal failure, stroke (transient or permanent), myocardial infarction, angina, congestive cardiac failure, reintervention and arrhythmias. Best evidence papers investigating BH-ONCAB versus C-ONCAB were considered. Where data were duplicated, the more credible evidence-based ...

Off-pump or on-pump beating heart: which technique offers better outcomes following coronary revascularization?
A best evidence topic was written according to a structured protocol. The question addressed was whether on-pump beating heart coronary artery bypass (BH-ONCAB) surgery has a different outcome profile in comparison to off-pump coronary artery bypass (OPCAB). A total of 205 papers were found by systematic search of which 7 provided the largest and most recent outcome analysis comparing BH-ONCAB with OPCAB, and represented the best evidence to answer the clinical question. The authors, date, journal, study type, population, main outcome measures and results were tabulated. Reported outcome measures included mortality, stroke...

Epidemiology of coronary artery bypass grafting at the Hospital Beneficência Portuguesa, São Paulo
Conclusion: The profile of patients undergoing coronary artery bypass graft surgery in this study is similar to other published studies.

Immediate coronary artery bypass graft surgery for acute coronary syndrome – Outcomes and trends over the past eight years
Conclusions The analysis of our patient file shows a decreasing trend in the number of patients undergoing emergency surgery due to ACS in our centre in the course of the last eight years. Between 2010 and 2013, the proportional representation of patients undergoing surgery due to UA decreased, while the percentage of patients undergoing surgery due to AMI and CS increased. Within the population of patients with ACS, we were also able to determine an increased frequency of some risk factors and increased thirty-day mortality among patients undergoing surgery.

Outcome Reporting in Cardiac Surgery Trials: Systematic Review and Critical Appraisal Cardiovascular Surgery
Conclusions Outcome reporting in the cardiac surgery literature is heterogeneous, and efforts should be made to standardize the outcomes reported and the definitions used to ascertain them. The development of standardizing outcome reporting is an essential step toward strengthening the process of evidence-based care in cardiac surgery.
