Stroke Associated Pneumonia: The University Hospitals-Cleveland Medical Center experience (P4.301)
Conclusions:In our series, most cases of SAP occurred in patients with severe stroke who were more likely to be ‘found down’ on presentation, urgently intubated and NPO. This data supports that impacting prevention and outcomes will require new strategies focusing on patients that require urgent or prolonged intubation rather than revising the current dysphagia assessment process.Disclosure: Dr. Attar has nothing to disclose. Complex interplay of multiple biological systems that contribute to post-stroke infections.
Abstract Stroke is a leading contributor of death and disability around the world. Despite its recognised debilitating neurological deficits, a devastating clinical complication of surviving stroke patients that needs more attention is infection. Up to half of the patients develop infections after stroke, and a high proportion of them will die as a direct consequence. Major clinical trials that examined preventive antibiotic therapy in stroke patients have demonstrated this method of prevention is not effective as it does not reduce incidence of post-stroke pneumonia or improve patient outcome. Additionally, retro...

Update on the Preventive Antibiotics in Stroke Study (PASS): a randomised controlled phase 3 clinical trial
Conclusion: The data from PASS will establish whether preventive antibiotic therapy in acute stroke improves functional outcome by preventing infection. In this update, we changed our primary outcome analysis from dichotomisation to ordinal regression analysis.Trial registration: Current controlled trials; ISRCTN66140176. Date of registration: 6 April 2010.

Antibiotic therapy for preventing infections in people with acute stroke.
CONCLUSIONS: Preventive antibiotics had no effect on functional outcome or mortality, but significantly reduced the risk of 'overall' infections. This reduction was driven mainly by prevention of urinary tract infection; no effect for pneumonia was found. PMID: 29355906 [PubMed - as supplied by publisher]

Brain abscess - A rare complication of endovascular treatment for acute ischemic stroke.
Conclusion: To the best of our knowledge, this is the first reported UK case of brain abscess following mechanical thrombectomy for stroke. Endovascular interventions can lead to increased incidence of ischemia-reperfusion injury in stroke with increased blood-brain barrier damage and risk of microbial seeding. This case highlights the need for rigorous asepsis and proactive treatment of systemic infections in the acute phase following endovascular treatment and consideration of brain abscess in all patients who present with new-onset confusion and unexplained fever following stroke. Comment Preventive antibiotic therapy in stroke: PASSed away?
Stroke not only impairs neurological function but also leads to severe medical complications,1 of which infections, especially pneumonia, are the most important.2 In the past 10 years, epidemiological data have consistently shown that post-stroke infections are associated with increased mortality and poor neurological outcomes.3–5 Dysphagia and a stroke-induced immunodepressive state are risk factors for post-stroke infections.6 Despite general measures for infection prevention, pneumonia remains a common and severe clinical challenge, even for patients treated in specialised stroke units5,7 that are known to reduce the ...

Antibiotic Class and Outcome in Post-stroke Infections: An Individual Participant Data Pooled Analysis of VISTA-Acute
Introduction: Antibiotics used to treat post-stroke infections have differing antimicrobial and anti-inflammatory effects. Our aim was to investigate whether antibiotic class was associated with outcome after post-stroke infection. Methods: We analysed pooled individual participant data from the Virtual International Stroke Trials Archive (VISTA)-Acute. Patients with ischaemic stroke and with an infection treated with systemic antibiotic therapy during the first two weeks after stroke onset were eligible. Antibiotics were grouped into eight classes, according to antimicrobial mechanism and prevalence. The primary analy...

Hypersensitive C ‐reactive protein‐albumin ratio is associated with stroke‐associated pneumonia and early clinical outcomes in patients with acute ischemic stroke
ConclusionsPatients with AIS with a high CAR at admission are more likely to develop SAP during hospitalization and have poor short-term clinical outcomes. These findings might help to timely identify patients at high risk of SAP and provide a basis for further research on prophylactic antibiotic therapy.
