Dynamic changes of carotid atherosclerosis and their relations with stroke recurrence in patients with stroke or transient ischemic attack
CONCLUSION: Plaque progression occurred in 46% of stroke or TIA patients. Plaque progression was significantly associated with clinical stroke recurrence. Plaque regressed in 6.5% of patients. P-016 Relationship between stroke recurrence, infarct pattern, and vascular distribution in patients with symptomatic intracranial stenosis
Conclusion We determined that basilar artery stenosis was most likely to present as a perforator stroke. As expected, patients discharged with suboptimal medical therapy were twice as likely to have a recurrent stroke (29% versus 13%). Among patients with optimal medical therapy, no recurrent strokes were seen in patients with embolic infarct pattern, while 57% recurrence rate was seen in patients with a watershed infarct pattern, suggesting that an embolic infarct pattern on initial presentation is possibly related to underlying plaque instability that is modifiable with optimal medical therapy. The PAr index, an indicator reflecting altered vitamin B-6 homeostasis, is associated with long-term risk of stroke in the general population: the Hordaland Health Study (HUSK).
Conclusions: Higher plasma PAr was independently associated with increased risk of incident stroke in all participants and across all subgroups stratified by conventional risk predictors. Our novel findings point to and expand the range of inflammation and immune activation processes that may be relevant for the pathogenesis and prevention of stroke.
