Thrombus Aspiration in ST Elevation Myocardial Infarction: An Individual Patient Meta-analysis.
CONCLUSIONS: -Routine thrombus aspiration during STEMI PCI did not improve clinical outcomes. In the high thrombus burden subgroup the trends toward reduced CV death and increased stroke or TIA provide a rationale for future trials of improved thrombus aspiration technologies in this high-risk subgroup. Predictors of long ‐term outcomes after bypass grafting versus drug‐eluting stent implantation for left main or multivessel coronary artery disease
Conclusions: Simple clinical variables and SYNTAX score differentially predict long‐term outcomes after CABG versus those after PCI with DES for left main or multivessel CAD. Those predictors might help to guide the choice of revascularization strategy. © 2017 Wiley Periodicals, Inc.

Coronary bypass surgery versus stenting in multivessel disease involving the proximal left anterior descending coronary artery
Conclusions In patients with multivessel disease with proximal LAD involvement, CABG is associated with lower rates of the safety composite endpoint of death, MI or stroke as compared with PCI with drug-eluting stents at 5 years of follow-up (number needed to treat=21). Trial registration number PRECOMBAT clinicaltrials.gov NCT00997828; SYNTAX: clinicaltrials.gov identifier: NCT00114972 NCT00114972.

Worse clinical outcomes following percutaneous coronary intervention with a high SYNTAX score: A systematic review and meta-analysis
Conclusions: Compared with CABG, worse clinical outcomes were observed following PCI in patients with a high SYNTAX score, confirming with evidence, published clinical literatures. Therefore, CABG should be recommended to CAD patients who have been allotted a high SYNTAX score.

Utilization of Anticoagulant and Antiplatelet Agents Among Patients With Atrial Fibrillation Undergoing Percutaneous Coronary Intervention - Retrospective Cohort Study Using a Nationwide Claims Database in Japan.
CONCLUSIONS: Among Japanese AF patients who underwent PCI, prescriptions for antiplatelet agents were more common than those for anticoagulant agents. The majority of study participants were classified as high risk, suggesting a need for a new risk classification that reflects the risk profiles of Japanese patients. Antithrombotic Therapy for Atrial Fibrillation and Coronary Disease de-Mystified
Publication date: Available online 20 August 2018Source: Canadian Journal of CardiologyAuthor(s): Jason G. Andrade, Marc W. Deyell, Graham C. Wong, Laurent MacleABSTRACTAtrial fibrillation (AF) is a progressive chronic disease characterized by exacerbations and periods of remission. It is estimated that up to 20-30% of those with AF also have coronary artery disease (CAD), and 5-15% will require percutaneous coronary intervention (PCI). In patients with concomitant AF and CAD, management remains challenging and requires a careful and balanced assessment of the risk of bleeding against the anticipated impact on ischemic out...

Clopidogrel Versus Aspirin for the Treatment of Acute Coronary Syndrome After a 12-Month Dual Antiplatelet Therapy: A Cost-effectiveness Analysis From China Payer's Perspective.
Abstract PURPOSE: Monotherapy with either aspirin or clopidogrel is recommended for long-term use after discontinuation of dual-antiplatelet therapy (DAPT) for acute coronary syndrome (ACS) management after percutaneous coronary intervention (PCI). The present study is to evaluate the cost-effectiveness of clopidogrel versus aspirin after 12-month DAPT for patients with ACS who underwent PCI in China. METHODS: A 2-part model was developed to estimate the cost-effectiveness of clopidogrel compared with aspirin. The short-term part was a decision tree that included health states such as myocardial infarction (M...

C-reactive protein and prognosis after percutaneous coronary intervention and bypass graft surgery for left Main coronary artery disease: Analysis from the EXCEL trial
Conclusions: In patients with LMCAD undergoing revascularization, elevated baseline CRP levels were strongly associated with subsequent death, MI and stroke at 3 years, irrespective of the mode of revascularization. Further studies are warranted to determine whether anti-inflammatory therapies may improve the prognosis of high-risk patients with LMCAD following revascularization.

Antithrombotic treatment in acute coronary syndrome and atrial fibrillation.
Authors: Darius H, Görge G, Spiecker M, Schinzel H Abstract The number of patients with atrial fibrillation (AF) is increasing due to the aging of the population. In addition, the number of patients with AF and indications for oral anticoagulation (OAC) for the prevention of stroke, who need dual antiplatelet treatment (DAPT) with acetylsalicylic acid (ASA) plus a P2Y12 inhibitor because of an acute coronary syndrome (ACS) and/or percutaneous coronary intervention (PCI) is also increasing. In the past these patients received a triple therapy (TT) for 3-12 months. This TT has never been studied for efficacy; how...

Society of Thoracic Surgeons Risk Scores Performance in Patients with Left Main Coronary Artery Disease Undergoing Revascularization in the EXCEL trial.
CONCLUSIONS: In selected patients with LMCAD from the EXCEL trial, STS risk models showed good predictive performance for CABG yet lacked predictive performance for PCI for perioperative mortality and renal failure. The STS stroke risk model was surprisingly more discriminating in PCI compared to CABG EXCEL patients. Improved and procedure-specific risk-prediction instruments are needed to accurately estimate adverse events after LMCAD revascularization by CABG and PCI. Coronary Artery Bypass Grafting Versus Percutaneous Coronary Intervention in Patients with Left Ventricular Systolic Dysfunction
ConclusionCABG was associated with lower rates of long-term mortality and revascularization but higher rate of upfront stroke in patients with LVD. However, the data included consisted predominantly of observational studies, highlighting the paucity and need for randomized trials.

Cardiovascular risk and mortality in rheumatoid arthritis compared with diabetes mellitus and the general population
ConclusionThis study demonstrates that RA is associated with an increased risk of HF, MI, stroke and coronary revascularization than found in the GP but without reaching the risk levels observed in DM patients.

Percutaneous Coronary Intervention Versus Medical Therapy in the Treatment of Stable Coronary Artery Disease: An Updated Meta-Analysis of Contemporary Randomized Controlled Trials
CONCLUSIONS: In patients with stable CAD, overall, short-term and intermediate-term risks of all-cause mortality are not significantly different between PCI and MT. However, PCI may reduce the overall and intermediate-term risk of the combined outcome of all-cause mortality, MI, revascularizations, rehospitalizations, or stroke.PMID:34338654
