Postpartum cardiomyopathy with cardioembolic stroke (p3.298)
Conclusions:Peripartum cardiomyopathy is a rare cause of stroke. Though seldom described, it should be considered in appropriate female patients presenting with neurological deficits. Definitive therapy with anticoagulation should be considered to prevent further complications.Disclosure: Dr. Kumbham has nothing to disclose. Dr. Sharma has nothing to disclose. Special Considerations for Primary and Secondary Stroke Prevention in Women
Semin Neurol 2017; 37: 376-382 DOI: 10.1055/s-0037-1603684There are gender differences in stroke in several areas. Overall, the incidence of stroke is greater in women because of the increased risk of stroke with increasing age and the longer life expectancy of women. However, gender-specific risk factors for stroke include hormonal changes during pregnancy and exogenous hormonal therapies. Also, cerebral venous thrombosis, thrombophilia, and migraine are more prevalent in women. These gender differences are the subject of this review. Stroke in Pregnancy
Semin Neurol 2017; 37: 669-678 DOI: 10.1055/s-0037-1608940Pregnancy and the postpartum period confer an increased risk of stroke. The risk appears to be increased for all strokes during the puerperium and for hemorrhagic strokes during pregnancy. During pregnancy, physiologic systems are under increased stress to accommodate the growing fetus, which results in substantial hemodynamic, cardiovascular, hematologic, immunologic, and structural connective tissue changes that account for much of this increased risk. In addition, preeclampsia–eclampsia has major effects on the vascular system, and this disorder, unique to preg...

Pregnancy ‐Associated Stroke
This article reviews assessment and initial treatment of pregnant and postpartum women experiencing stroke and provides guidance for subsequent maternity and primary care to assist front‐line perinatal care providers who may be the first to treat affected women or may resume primary care after diagnosis.

The Risk of Cardiovascular Disease, End-Stage Renal Disease, and Stroke in Post-Partum Women and their Fetuses after a Hypertensive Pregnancy.
Abstract Women with hypertensive pregnancy complications are at greater risk of developing cardiovascular disease (CVD), metabolic diseases, stroke, and end-stage renal disease later in life. Pregnancy complications not only effect the mother's long-term health, but also the health of the fetus immediately after delivery and into adulthood. The health of the fetus until adulthood can be influenced by developmental programming, in which the fetus is exposed to insults that will ultimately affect the growth of the offspring and increase the offspring's risk of developing hypertension, coronary heart disease, metabol...

The Impact of Pregnancy on Hemorrhagic Stroke in Young Women
Conclusions: Women with pregnancy-associated HS had fewer cerebrovascular risk factors, lower odds of having underlying vascular lesions, and higher proportion of migraine and RCVS compared with similar-aged non-pregnant women. Pregnancy-associated HS appears to represent a unique pathophysiological process, requiring targeted study.Cerebrovasc Dis 2018;46:10 –15

Acute Ischemic Stroke in Pregnancy
Stroke is an uncommon but serious potential complication of pregnancy. The management of acute ischemic stroke in pregnant women remains a complex challenge that extends beyond the limits of clinical trial evidence. Patient 1 was a 29-year-old woman 27 weeks into her first pregnancy, without remarkable past medical history or vascular risk factors. She was admitted 1 h after sudden onset of a left total anterior circulation syndrome (National Institute of Health Stroke Scale [NIHSS] score of 23). CT and angio-CT scans were normal. Thrombolysis was performed, with mild clinical improvement. Brain MRI showed multi-territoria...

Acute Management of Ischemic Stroke During Pregnancy.
Abstract Acute stroke in pregnancy can be devastating. Although neurologists will at some point be involved in the management, most of these patients are likely to first be evaluated by an obstetric care provider. It is, therefore, important for obstetric care providers to have an understanding of the presentation and management of stroke, particularly in the initial period when the window of opportunity for therapy is critical. Once suspected, a head computed tomography (CT) without contrast media should be performed without delay to rule out a hemorrhagic component. Patients presenting within 4.5 hours of sympto...

Maternal Stroke Associated With Pregnancy
This article summarizes current knowledge of the epidemiology, pathophysiology, prevention, and treatment of cerebrovascular disease in pregnant and postpartum women. RECENT FINDINGS Stroke is a leading cause of maternal morbidity and mortality, and most fatal strokes are preventable. Adaptive physiologic changes of pregnancy, including hemodynamic changes, venous stasis, hypercoagulability, and immunomodulation, contribute to increased maternal stroke risk. The highest-risk time period for maternal stroke is the immediate postpartum period. Migraine and hypertensive disorders of pregnancy, including gestational hyp...

Prenatal alcohol exposure exacerbates acute sensorimotor deficits and impedes long-term behavioral recovery resulting from adult-onset cerebrovascular ischemic stroke
CONCLUSIONS: Our data indicate that PAE contributes to adverse effects on brain health and to decreased resiliency to a common adult-onset neurovascular disease.PMID:36203340 | DOI:10.1111/acer.14952
