Recurrent coronary vasospasm-induced acute coronary syndrome complicated by cardiac arrest
We report a case of a 62-year-old male patient who was transferred to the emergency department after a successfully resuscitated cardiac arrest. He presented with persistent retrosternal chest pain. In the electrocardiogram ST-segment elevations and negative T waves in precardial leads were found (Fig. 1). The patient was current smoker, with a history of previous stroke and arterial hypertension. During emergency angiography, critical stenosis within the proximal left anterior descending coronary artery was found (Fig. 2A).

The appearance of a small thrombus in the pulmonary vein indicates the existence of a smaller pulmonary vein
Publication date: Available online 22 April 2015 Source:IJC Heart & Vasculature Author(s): Hidekazu Takeuchi Left atrial appendage thrombi and left atrial thrombi are known to cause ischemic stroke. Pulmonary vein thrombosis manifests with either atypical or subtle symptoms and is believed to be rare. However, this condition is life threatening and necessitates the development of tools and protocols to recognize and diagnose the presence of thrombi. Angiography via 64-slice multidetector CT (64-MDCT) is a procedure used to assess coronary artery plaques and identify the presence of LAA thrombi. Additionally, sinc...

My Approach to Detecting and Managing Peripheral Artery Disease
Atherosclerotic peripheral artery disease (PAD) is a prevalent disorder, affecting approximately 8 million adults in the United States. As a manifestation of systemic atherosclerosis, PAD is associated with an increased risk for myocardial infarction, stroke, and cardiovascular death. Patients with PAD may be asymptomatic, but many have impaired walking capacity, symptoms of intermittent claudication, pain and numbness of the feet, skin ulcers, and necrosis threatening limb viability.

Cardiovascular Risk Assessment and Effects on Behavior in Switzerland The Swiss Heart Foundation HerzCheck(®)/Cardio-Test(®).
CONCLUSION: The overall cardiovascular risk of individuals participating in a pharmacy based risk assessment program seems to be low. CardioTest (®) provided risk stratification with respect to future cardio-vascular events. CardioTest (®) seems to have impact on individual behavior and lifestyle modification. Other settings and locations for screening might be considered to reach higher risk individuals at an earlier stage. PMID: 25834654 [PubMed]

Combination refusion therapy for a hemodynamically unstable patient with acute myocardial infarction complicated by acute ischemic stroke within a time window
A 60-year-old man was admitted to the emergency department with 3hours history of chest pain and left-sided hemiparesis. The physical examination showed a heart rate of 42 beats per minute, blood pressure of 70/40mmHg. Neurological examination revealed left hemiplegia with a Grade 3 motor power in the upper and lower limbs, deep tendon reflexes were not exaggerated with negative Babinski sign. ECG presented elevated ST-segment in inferior leads with three-degree atrioventricular block and junctional escape rhythm.

Use of carbonated water in reduction of adjacent gastric activity in 456 consecutive technetium-99m myocardial perfusion imaging studies
Conclusion: The use of soda water prior to technetium-99m MPI was associated with lower rates of adjacent gastric activity only in patients undergoing ETT stress but not rest or VD stress. This differs from previously published data.

Ischaemic stroke after ST-elevation myocardial infarction
Clinical introduction A 67-year-old Hispanic man presented with 5-h history of typical chest pain and an ECG consistent with inferior ST-elevation myocardial infarction (STEMI). Coronary angiography revealed an occluded proximal right coronary artery, and a percutaneous intervention with drug eluting stent was performed. He was started on aspirin and clopidogrel. Two weeks later, he was admitted with loss of consciousness, tonic-clonic seizure and urinary incontinence. On physical examination, he was unresponsive to tactile stimuli. Cardiac examination revealed a prominent left ventricular heave. Neurological examination s...

Abstract 145: Differences in Presenting Symptoms of Fibromuscular Dysplasia Patients With Family History of Fibromuscular Dysplasia, Arterial Aneurysm, Dissection, or Sudden Death: A Report from the United States Registry for Fibromuscular Dysplasia Session Title: Abstract Poster Session I and Reception
Conclusions: Patients with a family history of FMD, arterial aneurysm (AA), dissection (AD), or sudden death (SD) more commonly had extracranial arterial bed complications, and experienced symptoms related to this vascular territory at time of diagnosis. These results suggest that FMD may present differently in those with this family history.

Correlates of pre-hospital morphine use in ST-elevation myocardial infarction patients and its association with in-hospital outcomes and long-term mortality: the FAST-MI (French Registry of Acute ST-elevation and non-ST-elevation Myocardial Infarction) programme
Conclusion In two independent everyday-life cohorts, pre-hospital morphine use in STEMI patients was not associated with worse in-hospital complications and 1-year mortality. Clinical trial registration Clinicaltrials.gov identifier: NCT00673036 (FAST-MI 2005); NCT01237418 (FAST-MI 2010).

Case Report: Acute Cerebellar Thrombosis in an Adult Patient with IgM Nephropathy.
We describe a patient who presented with nonspecific symptoms of epigastric pain, nausea, and early satiety. Abdominal imaging and endoscopies were unremarkable. She was found to have significant proteinuria (6.4 g/24 hours), hyperlipidemia, and edema consistent with a diagnosis of nephrotic syndrome. Kidney biopsy was performed and confirmed an IgM nephropathy. Less than 2 weeks after her diagnosis of IgM nephropathy, she presented with an acute cerebellar stroke. Thrombophilia is a well-known complication of nephrotic syndrome, but a review of the literature failed to show an association between IgM nephropathy and acute...

Endovascular Therapy for Chronic Mesenteric Ischemia
Opinion statement Chronic mesenteric ischemia (CMI) most commonly occurs as a consequence of multivessel atherosclerotic disease of the mesenteric vasculature. Risk factors include smoking, hypertension, dyslipidemia, and advanced age, and women are more commonly affected than men. The clinical presentation of CMI is characterized by postprandial abdominal pain and weight loss. Left untreated, patients often develop severe malnutrition. Current consensus guidelines recommend secondary prevention medications such as statins and aspirin for all patients with known atherosclerosis to reduce the risk of stroke and MI...

Low Homoarginine Levels in the Prognosis of Patients With Acute Chest Pain Coronary Heart Disease
Conclusion Low plasma homoarginine was identified as a risk marker for incident MACEs in patients with acute chest pain, in particular, in those with elevated hsTnI. Impaired homoarginine was associated with prevalent AF. Further studies are needed to investigate the link to AF and evaluate homoarginine as a therapeutic option for these patients.

Cohort of renal infarction during 2years at Grenoble teaching hospital.
CONCLUSION: Delay of diagnosis is a real problem for renal infarction, and need to be evocated every flank pain. LDH elevation may help clinician to suggest renal infarction and lead to CT scan. Association of delayed inflammatory syndrome, hypertension and hypokalemia after flank pain strongly suggest renal infarction. PMID: 27180563 [PubMed - as supplied by publisher]

0417 : Thrombolisis with tenecteplase (metalyse) in myocardial infarction an always topical issue
Conclusion Thrombolysis by Tenecteplase of STEMI admitted to ICCU was successfull in 89% of cases, which proves its effectiveness compared with other thrombolytic agents. The reperfusion rate was inversely proportional to time limit chest pain – arrival at ER. Patient education to call the ambulance (190) in case of chest pain should improve this delay and increase the reperfusion rate. The author hereby declares no conflict of interest
