Transcatheter and Surgical Aortic Valve Replacement in Patients with Recent Acute Heart Failure.
CONCLUSIONS: Recent AHF is associated with increased risk of mortality and morbidity after SAVR and TAVR. These findings suggest that AS patients should be referred for invasive treatment before the development of clinically evident heart failure. PMID: 31288017 [PubMed - as supplied by publisher]

Common Postcardiothoracic Surgery Arrhythmias
Cardiac arrhythmias are common after cardiac surgery and have profound sequelae. Bradycardias are typically transient and have reversible causes; however, persistent atrioventricular block is an indicator for permanent pacemaker implantation after valvular surgery. Transcatheter aortic valve surgery is associated with even higher rates of permanent pacemaker implantation. Atrial fibrillation, the most common postoperative arrhythmia, is associated with ischemic stroke, myocardial infarction, congestive heart failure, and short-term mortality. Ventricular arrhythmias have extremely high in-hospital mortality, as well as lon...

Mitral valve surgery with or without coronary bypass grafting: eight-year cohort study.
CONCLUSION: MVR+CABG compared to MVR patients had more comorbidities and greater operative risk, and were independently associated with higher operative mortality and composite morbidity, but not independently associated with higher long-term mortality. PMID: 31415499 [PubMed - in process]

Predictors of Unfavourable Outcomes in Children and Adolescents Submitted to Surgical Mitral Valvuloplasty Secondary to Chronic Rheumatic Heart Disease
Conclusion: The duration of vasoactive drug use is an independent predictor for unfavourable outcomes in the immed iate and late postoperative period, while residual mitral regurgitation was associated with reoperation, and both tricuspid regurgitation and pulmonary hypertension were associated with unfavourable outcomes.

Current Role of the Total Artificial Heart in the Management of Advanced Heart Failure
AbstractThe total artificial heart (TAH) is a form of mechanical circulatory support that involves resection of the native ventricles followed by placement of a device that can restore total pulmonary and systemic flow. Given the increasing burden of congestive heart failure and cardiovascular disease, the number of people in need of cardiac replacement therapy will continue to grow. Despite aggressive efforts to expand the donor pool, the number of heart transplants in the United States (US) has plateaued at less than 3000 per year. In addition, there is increasing recognition of the long-term complications of current gen...

Predictors of Cumulative Health Care Costs Associated with Transcatheter Aortic Valve Replacement in Severe Aortic Stenosis
ConclusionsThis analysis shows that there are 3 distinct phases of cost accumulation from referral to post-TAVR with some potentially modifiable cost drivers in each phase.

Light ‐chain and transthyretin cardiac amyloidosis in severe aortic stenosis: prevalence, screening possibilities, and outcome
ConclusionBoth TTR ‐ and AL‐CA can accompany severe AS. Parameters solely based on ECG and echocardiography allow for the identification of the majority of CA‐AS. In the present cohort, CA did not significantly worsen prognosis 15.3 months after TAVR.

Right ventricular mechanical pattern in patients undergoing mitral valve surgery: a predictor of post ‐operative dysfunction?
ConclusionsMVR induces a significant shift in the RV mechanical pattern. Advanced indices of RV mechanics are associated with invasively measured parameters of RV contractility and may predict post ‐operative RVD.

Impact of left atrial diameter on outcome in patients undergoing edge ‐to‐edge mitral valve repair: results from the German TRAnscatheter Mitral valve Interventions (TRAMI) registry
ConclusionLeft atrial enlargement is a strong and independent predictor of adverse long ‐term outcome after transcatheter mitral valve repair. Further study is warranted to examine whether timely intervention may have the potential to modify outcome.

Long-term echocardiographic evaluation of asymptomatic patients undergoing minimally invasive valve repair for severe primary mitral regurgitation.
CONCLUSIONS: Minimally invasive mitral valve repair is a safe and efficient method of correction in asymptomatic patients. The intervention should be performed regardless of ventricular and atrial dimensions. PMID: 32267134 [PubMed - as supplied by publisher]

Commentary: Transcending acceptable, moving toward optimal: Standardizing surgical configurations of ventricular assist device therapy
The number of patients with end-stage heart failure potentially eligible for ventricular assist device (VAD) therapy continues to increase.1 Meanwhile, VAD outcomes with current centrifugal devices have improved significantly and in the first few years after implantation are approaching those of transplantation, yet at the same time they remain encumbered by adverse events such as stroke and hemodynamic complications like aortic regurgitation (AR).2 Our understanding of the mechanisms of VAD complications has expanded from basic engineering of the devices themselves to the patient −pump interface (comorbidities, thrombog...
