“No Turn Back Approach” to Reduce Treatment Time for Endovascular Treatment of Acute Ischemic Stroke
Conclusions: The no turn back approach appeared to be feasible and reduced the time interval between ED arrival and microcatheter placement in acute ischemic stroke patients undergoing endovascular treatment.

Circulating Atrial Natriuretic Peptide Genetic Association Study Identifies a Novel Gene Cluster Associated With Stroke in Whites Original Articles
Conclusions— This is the first large-scale genetic association study of circulating NT-proANP levels performed with replication and functional assessment that identified genetic variants in the MTHFR-CLCN6-NPPA-NPPB cluster to be significantly associated with NT-proANP levels. The clinical significance of this variation is related to lower NT-proANP levels, higher blood pressures, and an increased risk of stroke in the general community.

Sex Differences in Outcomes and Associated Risk Factors After Acute Ischemic Stroke in Elderly Patients: A Prospective Follow-up Study
Stroke has a greater effect on women. However, sex differences in outcome and factors associated with outcome among elderly patients are unknown. From January 2009 to December 2011, 810 patients with acute ischemic stroke aged 75 years or older were recruited in China. Clinical profile and risk factors were recorded. Outcomes and associated risk factors at 12 and 36 months after stroke were assessed by sex. Hypertension, diabetes mellitus, dyslipidemias, and obesity prevalence rates were higher in women than in men; opposite trends were found for smoking and alcohol consumption.

Residual Risk of Stroke and Death in Anticoagulated Patients According to the Type of Atrial Fibrillation: AMADEUS Trial Clinical Sciences
Conclusions— The risk of cardiovascular death, stroke, or systemic embolism is higher in anticoagulated patients with permanent AF than in those with nonpermanent AF, regardless of preexisting heart failure.

It is well established that diabetes mellitus type 2 (T2DM) is associated with increased risk of cardiovascular disease, including ischemic stroke. Among past antidiabetic agents, only pioglitazone and glucagon-like peptide 1 receptor agonists (GLP-1RA) have been shown to decrease the risk of stroke.1-5 Similarly, sodium-glucose cotransporter-2 inhibitors (SGLT2i) were expected to reduce the risk of stroke, considering their blood pressure lowering and neuroprotective effects demonstrated by experimental studies.

Effects of Sodium/Glucose Cotransporter Inhibitors on Atrial Fibrillation and Stroke: A Meta-Analysis
Globally, the number of people estimated to have diabetes mellitus (DM) in 2019 was 463 million, and the estimated number is expected to rise to 578 million people by 2030.1 DM is also associated with an increased risk of atrial fibrillation (AF), 2 and has shown to be an independent risk factor for stroke in patients with AF.3 A meta-analysis of 102 studies further showed that patients with diabetes have a higher risk of all subtypes of stroke (ischemic, hemorrhagic and unclassified stroke).4 This has generated great interest in anti-hyperglycemic agents and their ability to reduce the cardiovascular complications of diabetes.

Conclusion: A substantial percentage of patients deemed "too good" for IV tPA were unable to be discharged home. Factors such as advanced age and higher NIHSS should be considered in tPA decision-making to optimize outcomes. Large, multi-center prospective studies are underway to study the predictors of poor outcomes in this group.
