Assessment of clinical severity in patients with infective endocarditis with cardio-embolic strokes
We sincerely thank Biteker et al. for their interest in our study to suggest a possible association of stroke severity with clinical outcomes in patients with infective endocarditis (IE) with cerebral complications regardless of whether early surgery or conventional treatment. They recommended us to use National Institutes of Health Stroke Scale (NIHSS) score to evaluate this issue as recently reported by Murai et al. [1], while a necessity of patient's answer to several inquiries for a measurement of NIHSS score had imposed major limitations on analyzing our data retrospectively.

Revision of Echocardiographic Indications and Findings in Neurologically Ill Patients.
CONCLUSION: Care should be taken to visualize the left ventricular apical regions when investigating patients referred from a neurologic department in order not to overlook LVHT and thrombi within the left ventricular apex. PMID: 28400861 [PubMed - in process]

Streptococcus pneumoniae meningitis and intracranial vasculopathy: Clinical correlation with improving transcranial Doppler hemodynamics (P1.322)
Conclusions:Antibiotic treatment for S. pneumoniae meningitis correlated with improvement in clinical status and TCD hemodynamics. Serial TCDs may be a potentially useful strategy in the management of bacterial meningitis.Disclosure: Dr. Idris has nothing to disclose. Dr. Tai has nothing to disclose. The Characteristics of Infectious Intracranial Aneurysm and Indication of Cerebral Angiogram in Infective Endocarditis (P2.296)
Conclusions:IIAs were found in approximately one fifth of IE persons who underwent DSA. Imaging characteristics such as SWI hemorrhage in sulci or parenchyma and contrast enhancement appear to correlate with the presence of IIA. DSA should be performed when they are present.Disclosure: Dr. Cho has nothing to disclose. Dr. Marquardt has nothing to disclose. Dr. Zhang has nothing to disclose. Surgical Ablation of Atrial Fibrillation in the United States: Trends and Propensity Matched Outcomes
Conclusions Contemporary utilization of SA is increasing across all operative categories. Performance of SA is accompanied by a 30-day reduction in mortality and stroke. These findings further refine our understanding of the role of SA in the treatment of AF.

A systematic review and meta-analysis of mechanical vs biological composite aortic root replacement, early and 1-year results
ConclusionsComposite mechanical root offers no superiority to composite biological root. There is a significant increase in the perioperative bleeding amongst composite mechanical root cohort. There is a need for further randomized control trail to assess the efficacy of either methods.

Human serum albumin in cardiovascular diseases.
Abstract Cardiovascular diseases are the leading cause of death worldwide. Endothelial dysfunction, inflammation and oxidative stress are at the forefront in the onset and development of atherosclerosis and many cardiovascular diseases. Epidemiological evidence is that low serum albumin levels are linked to incident ischemic heart disease, heart failure, atrial fibrillation, stroke and venous thromboembolism, independent of risk factors, body mass index and inflammation. Hypoalbuminemia has also emerged as an independent prognosticator in many cardiovascular diseases, such as coronary artery disease, heart failure...

Hemodynamic and clinical performance of Solo stentless bioprosthetic aortic valves
ConclusionsSolo stentless aortic valves are safe to implant, with promising clinical outcomes in short- and medium-term assessment. Moreover, they show an excellent hemodynamic performance: low transvalvular gradients, large effective orifice areas and low incidence of patient-prosthesis mismatch.

Mitral valve prolapse.
This article reviews the pathophysiology, genetics, clinical features, diagnostic imaging, complications, long term outcomes and indications for intervention in mitral valve prolapse. Expert commentary: Several key dilemmas in the management of mitral valve prolapse remain. Factors which influence progression of mitral regurgitation are unclear and therefore we have no therapeutic targets to prevent progression. Evidence-based methods to reduce the risk of sudden death, stroke and endocarditis have not been identified. In symptomatic patients with severe mitral regurgitation valve surgery is recommended. In asymptomatic pa...

Determining Which Prosthetic to Use During Aortic Valve Replacement in Patients Aged Younger than 70 Years: A Systematic Review of the Literature.
CONCLUSIONS: Published literature does not preclude the use of bioprostheses for AVR in younger patients. As new valves are developed, the use of bioprosthetic aortic valves in younger patients will likely continue to expand. Clinical trials are needed to provide surgeons with more accurate guidelines. Long-term outcomes with mechanical versus biological aortic valve prosthesis in patients >70 years
ConclusionsMechanical aortic valve prosthesis is associated with worse long-term survival and increased bleeding after SAVR in patients aged>70 years. Our results suggest caution when considering mechanical aortic valve prostheses in elderly patients.
