B vitamins and cognition in subjects with small vessel disease: A Substudy of VITATOPS, a randomized, placebo-controlled trial
VITAmins TO Prevent Stroke (VITATOPS) study, a randomized, double-blind international multi-centre trial involving 8164 subjects in studying effect of lowering homocysteine level by daily administration of B-vitamins (2mg folic acid, 25mg vitamin B6, and 0.5mg vitamin B12) failed to demonstrate a positive effect of B-vitamins in reducing incidence of major vascular events and global cognition decline as measured by mini-mental state examination (MMSE) score [1,2]. However, assessment of cognition using MMSE alone has its limitations and a full neuropsychological assessment of different cognitive domains will provide more robust evidence.

Meta-analysis of folic acid efficacy trials in stroke prevention: Insight into effect modifiers
Conclusions: Folic acid supplementation could reduce the stroke risk in regions without folic acid fortification, particularly in trials using a relatively low dosage of folic acid and with low vitamin B12 levels.

Longitudinal association between fasting blood glucose concentrations and first stroke in hypertensive adults in China: effect of folic acid intervention Cardiovascular disease risk
Conclusions: In Chinese hypertensive adults, an FBG concentration ≥7.0 mmol/L or diabetes is associated with an increased risk of first stroke; this increased risk is reduced by 34% with folic acid treatment. Medical cost savings in Sakado City and worldwide achieved by preventing disease by folic acid fortification.
Abstract The introduction of mandatory fortification of grains with folate in 1998 in the United States resulted in 767 fewer spina bifida cases annually and a cost savings of $603 million per year. However, far more significant medical cost savings result from preventing common diseases, including myocardial infarction, stroke, dementia and osteoporosis. A cost‐effectiveness analysis showed a gain of 266,649 quality‐adjusted life‐years and $3.6 billion saved annually, mainly due to the reduction of cardiac infarction. Folic acid supplementation and chronic kidney disease progression
In contrast to prior studies demonstrating no benefit or even increased harm from B vitamin supplementation in patients with chronic kidney disease, a large randomized trial from China recently demonstrated small but statistically significant reductions in the risk of first stroke and chronic kidney disease progression with the addition of folic acid to enalapril in adults with hypertension. Differences in the study population and study intervention may explain these discordant results.

Folic acid (FA) deficiency is not only associated with an increased risk of ischemic stroke, but also with increased oxidative DNA damage and brain injury after cerebral ischemia –reperfusion. However, the cellular and molecular mechanisms underlying FA deficiency-associated neuropathogenesis are not completely understood. In the present study, we tested the hypothesis that neuronal autophagy in focal cerebral ischemia rats may be involved in the mechanisms of FA deficienc y -induced injury to neuronal cells.
