Comparison Between Healthcare-Associated and Community-Acquired Infective Endocarditis at Tertiary Care Hospitals in Japan.
CONCLUSIONS: HAIE has different clinical characteristics and causative microorganisms, as well as worse prognosis, than CIE. Preventive strategies, and the prompt and appropriate identification of HAIE may improve the outcome of infective endocarditis. PMID: 32132310 [PubMed - as supplied by publisher]

Temporal sustainability of guideline based door-to-needle times for intravenous thrombolysis for acute ischemic stroke
ConclusionIt is possible to sustainably reduce DTN time for IVT in AIS to well below the current 60 min benchmark with hospital system training and teamwork.

Clinical features and efficacy of reperfusion therapy in minor ischemic stroke patients with atrial fibrillation
AbstractThe efficacy of reperfusion therapy (RT) using intravenous infusion of recombinant tissue plasminogen activator and/or endovascular therapy for minor ischemic stroke (MIS) has not yet been established. The present study aimed to elucidate the clinical features of MIS patients with atrial fibrillation (AF) and examine whether they could be potential candidates for RT. Data of MIS patients, defined as those with a score  ≤ 5 on the National Institute of Health Stroke Scale, were extracted from patients admitted to our hospital between 2006 and 2018, and clinical characteristics were compared between the AF and ...

Relationship of White Matter Lesions with Intracerebral Hemorrhage Expansion and Functional Outcome: MISTIE II and CLEAR III
ConclusionsConcomitant WML does not increase the odds for HE in patients with ICH but increases the odds for poor functional outcome.Clinical Trial Registrationhttp://www.clinicaltrials.gov trial-identifiers: NCT00224770 and NCT00784134.

Recanalization is the Key for Better Outcome of Thrombectomy in Basilar Artery Occlusion
ConclusionReperfusion is the major predictor of functional outcome in BAO in clinical practice. Failed recanalization resulted in a  13-fold increase of the risk of poor outcome. Successful recanalization is crucial to achieve a better functional outcome in BAO.

Plasma Neurofilament Light Chain Is Associated with Poor Functional Outcome and Mortality Rate After Spontaneous Subarachnoid Hemorrhage
AbstractThe initial clinical status after subarachnoid hemorrhage (SAH) is an important outcome predictor, but the mechanisms behind the early brain injury (EBI) remains incompletely understood. Elevated neurofilament levels in the cerebrospinal fluid at protracted stages after SAH are associated with poor outcome, but the potential association between plasma neurofilament (pNfL) levels during EBI, disease severity on admission, and poor outcome remains unaddressed. Plasma NfL (pNfL) was measured by single molecule array in 44 SAH patients on admission and 24  h after ictus, as well as in 44 controls. Disease severity on ...

Collateral Scores in Acute Ischemic Stroke
ConclusionAlthough collaterals are known to correlate with clinical outcome, none of the analyzed collateral scores sufficiently predicted outcome as a  standalone parameter.

Invasiveness and Clinical Outcomes of Off-Hour Admissions in Patients with Intracerebral Hemorrhage
Background: Whether time of hospital admission —during or outside regular working hours—affects functional outcome in intracerebral hemorrhage (ICH) is unestablished as previous analyses have focused on mortality only. We here investigate whether on- versus off-hour hospital admission in ICH is associated with levels of invasiveness and clin ical outcomes. Methods: Based on the UKER registry (NCT03183167) we grouped ICH-patients according to on- versus off-hour hospital admission. Primary outcome measures was functional outcome after 3 months using the modified Rankin scale (mRS) dichotomized into favorable (mRS = ...

Direct Admission vs. Secondary Transfer to a  Comprehensive Stroke Center for Thrombectomy
ConclusionThese results confirm prior studies stating that DA to a  comprehensive stroke center leads to better outcome compared to ST in stroke patients undergoing thrombectomy.

Efficacy of ADAPT with large-bore reperfusion catheter in anterior circulation acute ischemic stroke: a multicentric Italian experience
ConclusionsADAPT is a valid technique with respect to the rates of TICI 2b/3 recanalization and 90-day mRS scores. In this series, an attempt at recanalization with ADAPT with larger-bore reperfusion catheter may be warranted prior to stentriever thrombectomy.

Decompressive craniectomy in malignant middle cerebral artery infarction: family perception, outcome and prognostic factors.
CONCLUSIONS: We present a 21 patients group where the best outcome was achieved in patients ≤60 years old. The severe neurological sequelae in patients with malignant infarction subjected to decompressive hemicraniectomy are tolerated and accepted by most families to the benefit of survival. We must not let this family satisfaction hide the prognosis, having to contextualize it within the real ambulatory situation of the patients. PMID: 31445797 [PubMed - as supplied by publisher]
