Two-Year Outcomes in HeartMate 3 versus HeartWare HVAD Patients Implanted as Destination Therapy
The ENDURANCE trial demonstrated noninferiority of the intrapericardial centrifugal-flow HeartWare HVAD compared to the axial-flow HeartMate II left ventricular assist device (LVAD) regarding survival in patients with advanced heart failure ineligible for heart transplantation, but stroke was more common. Most recently, the MOMENTUM 3 trial demonstrated superiority of the intrathoracic, fully magnetically levitated centrifugal-flow HeartMate 3 (HM3) LVAD compared to the HeartMate II. A clinical trial evaluating the HM3 and HVAD has not been performed, therefore the purpose of this study was to compare outcomes between these two devices.

Pulmonary Artery Pulsatility Index (PAPi) is a Predictor of Right Ventricular Assist Device (RVAD) Use Following HeartMate 3 LVAD Implantation
Pulmonary artery pulsatility index (PAPi) recently emerged as a predictor of right ventricular failure after Heartmate II LVAD Implantation. Clinical utility of this marker in Heartmate 3 patients is unknown. We hypothesized that PAPi is a stronger predictor of RV failure compared to central venous pressure to pulmonary artery capillary wedge pressure ratio (CVP/PCWP) or RV stroke work index (RVSWI) in Heartmate 3 population.

Increased Mean Platelet Volume is Associated with Decreased Survival in Patients Supported with Impella
Increased mean platelet volume (MPV) has been associated with elevated risk of stroke and myocardial infarction in the general population and stroke/pump thrombosis in Heartmate II recipients. It has been proposed that contact with foreign surfaces and centrifugation promote prothrombotic states with platelet activation and increased MPV. An association between MPV and outcomes of patients treated with percutaneous short-term circulatory devices has not been investigated.

Natural Antibodies and Left Ventricular Assist Device Complications
Left ventricular assist devices (LVAD) are widely used as a support strategy for advanced heart failure. Complications such as thrombosis and bleeding have been linked to LVAD. We observed that LVAD implantation was followed by a sharp increase in serum levels of IgG natural antibodies (Nabs) recognizing oxidation-specific epitopes (OSE) and apoptotic cells. Nabs have been implicated in inflammatory reactions related to atherosclerosis, ischemic stroke and primary graft dysfunction following heart transplantation.

The development of durable left ventricular assist systems (LVASs) has been critical in providing an option for management of end-stage heart disease and addressing the increasing gap between demand and supply of donor cardiac allografts for management of end-stage heart disease. The technology also provides an alternative to heart transplantation in many patients. Unfortunately, the long-term use of LVASs has been associated with significant morbidities, the most devastating of which may relate to the thrombogenicity of the artificial devices, leading to ischemic stroke or pump thrombosis.

The Impact of Statin Therapy on Neurological Events Following Left Ventricular Assist System Implantation in Advanced Heart Failure
Stroke is a leading cause of disability and death in advanced heart failure patients supported with continuous-flow left ventricular assist systems (CF-LVAS). Statins (HMG-CoA Reductase Inhibitors) reduce the risk of major cardiovascular and neurological events, such as stroke, but their impact has not been evaluated in patients implanted with CF-LVAS. We sought to explore the association between use of statin therapy and subsequent occurrence of neurological events, particularly stroke, following CF-LVAS implantation.

The utilization and duration of left ventricular assist device (LVAD) support has increased steadily over the last decade and recent changes in the UNOS heart allocation system, implemented in October 2018, portend a further increase in the duration of mechanical circulatory support for many patients (1). Though four-year survival rates have improved to 60% following LVAD implantation, the leading cause of death in these patients is neurological injury, specifically ischemic and hemorrhagic stroke (spontaneous subarachnoid hemorrhage or intraparenchymal hemorrhage [IPH]) (2).
