Detection of inflow obstruction in left ventricular assist devices by accelerometer: A porcine model study
Left ventricular assist devices (LVAD) provide circulatory blood pump support for severe heart failure patients. Pump inflow obstructions may lead to stroke and pump malfunction. We aimed to verify in vivo that gradual inflow obstructions, representing prepump thrombosis, are detectable by a pump-attached accelerometer, where the routine use of pump power (PLVAD) is deficient.

Elective Cardiopulmonary Bypass for Lung Transplantation: Is It Safer Than We Think
The use of cardiopulmonary bypass (CPB) during lung transplantation is controversial. CPB can offer improved visualization, better control of central vasculature, and possibly faster operations. Opponents argue the use of CPB results in higher transfusion requirements, increased rates of primary graft dysfunction (PGD), and more complications such as stroke, acute kidney injury (AKI) and renal failure requiring hemodialysis. In the fall of 2020 our program transitioned to performing all lung transplants on CPB.

Platelet Function and Sildenafil Use During Left Ventricular Assist Device Support
Clinical outcome studies have shown that sildenafil is associated with reduced mortality and thrombotic events such as ischemic stroke during contemporary left ventricular assist device (LVAD) support. Sildenafil has anti-platelet properties but its relation to platelet function during LVAD support is uncertain.

A Retrospective Study on Gender, LAA Morphology and Stroke Risk
This was a retrospective study to investigate whether gender and LAA size/morphology was a predictor of cerebral vascular events in patients undergoing cardiac surgery. We studied the LAA morphology, and measurements for women and men in an attempt to correlate the different morphologies/sizes with the patient's risk of stroke.

Impact of Perioperative Sarcopenia and Dysphagia on LVAD Related Complications
Sarcopenia, as defined by decline in muscle quantity/quality, is prevalent in heart failure and associated with high morbidity and mortality. Sarcopenic dysphagia is caused by sarcopenia in the whole body and swallowing-related muscles. We investigated the impact of perioperative sarcopenia and dysphagia on postoperative pneumonia (PNA), stroke, gastrointestinal bleeding (GIB) and 30-d readmission after LVAD.

Stroke and Survival Outcomes in Left Ventricular Device Patients
Left ventricular assist devices (LVADs) are artificial pumps used for hemodynamic support in patients with end stage heart failure. They can function effectively as a bridge to transplant as well as extend and improve the quality of life of patients. However, these patients are still at a higher risk of stroke, death, and other complications than the average population.

Right heart failure after left ventricular assist device implantation – from prediction to action
In this issue of the Journal, Read and colleagues1 report the use of peak stroke volume index (SVI) during pre-procedure vasodilatory testing as a predictor of early right heart failure (RHF) after left ventricular assist device (LVAD) implantation. They retrospectively examine 70 patients from three institutions who underwent pre-implant nitroprusside vasodilator testing. Indications for pre-operative vasodilator testing were elevated pulmonary vascular resistance, pulmonary arterial wedge pressure, or systolic pulmonary artery pressure.

Carotid Endarterectomy Prior to Ventricular Assist Device Placement in a Patient with Cardiogenic Shock
Atherosclerotic disease in the cervical vessels may be a contraindication for implantation of left ventricular assist devices (LVADs). The occurrence of ischemic stroke represents an obstacle to rehabilitation, and its incidence seems to be higher in this population. Carotid endarterectomy (CEA) is a preferable surgical treatment in patients with high bleeding risk. This case report suggests that CEA followed by implantation of LVADs can be feasible.

Assessing Cardiac Recovery for Decommissioning of LVAD Support: Structure versus Function
Recovery of left ventricular (LV) function during LVAD support occurs in only 1-3% of patients. Assessment for recovery and LVAD removal is desirable given the risks of infection, stroke, and device malfunction. This case of a patient with an LVAD, with recovered LV systolic function but abnormal exercise hemodynamics, illustrates the potential utility of right heart catheterization studies for clinical decisions.
