Comparing Velocity and Mean Arterial Pressure to Assess Pulsatility in Patients with Continuous Flow Left Ventricular Assist Devices
An increasing number of end-stage heart failure patients are supported by continuous-flow Left Ventricular Assist Devices (cfLVADs). In these patients, accurate assessment of arterial pulsatility remains a challenge because of diminished pulse pressure. Limited pulsatility in modern cfLVADs itself can be associated with adverse events such as stroke. Our study investigates the value of simultaneous arterial pressure and arterial blood velocity assessment in cfLVAD patients.

Optimization of Left Ventricle Size After VAD Implantation Prevents Incidence of Cerebral Infarction
Stroke after ventricular assist device (VAD) implantation is a critical complication. Previous studies have noted the distance between the inflow cannula and the left ventricular (LV) septum, as well as an angulation of the inflow cannula from the apical axis, related to the incidence of stroke. In the early phase after VAD implantation, diuresis is caused by improved renal blood flow, furthermore, rapid volume reduction of LV greatly changes circulating plasma volume, resulting in increased stroke risk; however, little is known about an appropriate indicator for the prevention of this phenomenon.

A Prospective Randomized Trial Comparing Stroke Volume Measurements by Esophageal Doppler and Pulse-Contour Analysis During a Fluid Resuscitation Protocol in Brain-Dead Organ Donors
Maximizing stroke volume (SV) is the optimal method of fluid resuscitation. Both the esophageal doppler (EDM) and pulse contour analysis (PCA) are validated methods of measuring SV in critical patients, but neither have not been well-studied in the brain-dead (BD) donor. We have previously published our fluid resuscitation protocol in BD donors demonstrating a significant reduction in time on vasopressors from 16 hours in the control group to 2.9 hours in the protocol group. We performed a prospective randomized trial comparing the effectiveness of the EDM and PCA in this fluid resuscitation protocol in BD donors.

Stroke Risk in the Contemporary Era of Left Ventricular Assist Device Support

Who is at Risk for Seizures After Heart Transplantation?
Seizures may occur following heart transplantation (HTx), especially since tacrolimus can lower a seizure threshold. HTx recipients are also at high stroke risk given many patients have existing cerebrovascular disease. Association of Outflow Cannula Angulation with Adverse Neurological Events in Patients with CF-LVAD
Adverse neurological outcomes are the Achilles heel of the current day LVAD technology and are associated with worse survival. Our previous data suggests that outflow cannula (OFC) angles measured using cardiac CT may correlate with stroke in continuous-flow HeartMate II patients. Therefore, the aim of our study was to assess the relationship of inflow cannula (IFC) and outflow cannula (OFC) position with neurological outcomes centrifugal flow pumps (HM3 and HVAD).

Bacteriophage Therapy and the Biofilm: An Old Solution for a Persistent and Desperate Problem?
With the adoption of use of continuous-flow pumps with centrifugal design and complete magnetic levitation (i.e., HeartMate 3, Abbott, Chicago, IL), outcomes following durable left ventricular assist device (LVAD) have continuously improved over the last five years (1). Improved hemocompatibility associated with this device design has translated into a significant reduction in major adverse events, primarily pump thrombosis, stroke, and to a lesser extent, reduction in non-surgical bleeding (1).
