A Randomized Pilot Study of Aortic Waveform Guided Therapy in Chronic Heart Failure Heart Failure
Conclusions Maximization of goal-directed medical therapy in heart failure patients may enhance afterload reduction and lead to reverse remodeling, while additional medicine titration based upon aortic pressure data improves exercise capacity in patients with heart failure.

Early Clopidogrel Versus Prasugrel Use Among Contemporary STEMI and NSTEMI Patients in the US: Insights From the National Cardiovascular Data Registry Coronary Heart Disease
Conclusions With prasugrel as an antiplatelet treatment option, contemporary practice shows low uptake of prasugrel and delays in P2Y12 antagonist initiation among NSTEMI patients. We also note concerning evidence of inappropriate use of prasugrel, and inadequate targeting of this more potent therapy to maximize the benefit/risk ratio.

Long-term Cardiovascular Risks Associated With an Elevated Heart Rate: The Framingham Heart Study Epidemiology
Conclusions Individuals with a higher heart rate are at elevated long-term risk for cardiovascular events, in particular, heart failure, and all-cause death. On the other hand, a higher heart rate is associated with a lower risk of future permanent pacemaker implantation.

B-type Natriuretic Peptides for the Prediction of Cardiovascular Events in Patients With Stable Coronary Heart Disease: The Heart and Soul Study Coronary Heart Disease
Conclusions Both BNP and NT-proBNP were significant predictors of MACE in stable coronary heart disease; however, NT-proBNP was superior to BNP for net risk reclassification for MACE.

Incremental Prognostic Significance of the Elevated Levels of Pentraxin 3 in Patients With Heart Failure With Normal Left Ventricular Ejection Fraction Heart Failure
Conclusions High plasma PTX3 levels, but not other inflammatory markers, are correlated with future cardiovascular events in patients with HFNEF. PTX3 may be a useful biomarker for assessment of risk stratification in HFNEF. Clinical Trial Registration URL: http://www.umin.ac.jp; Unique identifier: UMIN000002170.

Prognostic Implications of Left Ventricular Dyssynchrony for Major Adverse Cardiovascular Events in Asymptomatic Women and Men: The Multi-Ethnic Study of Atherosclerosis Heart Failure
Conclusions In an asymptomatic cohort, greater LV dyssynchrony determined by cardiac magnetic resonance imaging predicts adverse cardiovascular outcome in women but not in men. Clinical Trial Registration URL: http://clinicaltrials.gov. Unique identifier: NCT00005487.

Choosing Wisely for Syncope: Low-Value Carotid Ultrasound Use Health Services and Outcomes Research
Conclusions These data suggest that carotid ultrasound for patients with uncomplicated syncope are still commonly ordered and may be an easy target for institutions striving to curtail low-value care.

Obesity, Physical Activity, and Their Interaction in Incident Atrial Fibrillation in Postmenopausal Women Arrhythmia and Electrophysiology
Conclusions Greater physical activity is associated with lower rates of incident AF and modifies the association between obesity and incident AF.

Prognostic Value of Estimated Functional Capacity Incremental to Cardiac Biomarkers in Stable Cardiac Patients Coronary Heart Disease
Conclusion A simple self-assessment tool of functional capacity in stable patients undergoing elective diagnostic cardiac evaluation provides independent and incremental prognostic value for prediction of both significant coronary angiographic disease and long-term adverse clinical events.

Short-Term Change in eGFR and Risk of Cardiovascular Events Health Services and Outcomes Research
Conclusion Clinically relevant changes in eGFR are associated with increased risk of cardiovascular events. However, most of the apparent increase in risk can be accounted for by assessing comorbidity and baseline kidney function.

Importance of Exercise Capacity in Predicting Outcomes and Determining Optimal Timing of Surgery in Significant Primary Mitral Regurgitation Valvular Heart Disease
Conclusion In patients with primary MR that underwent exercise echocardiography followed by MV surgery, lower achieved metabolic equivalents were associated with worse long-term outcomes. In those with preserved exercise capacity, delaying MV surgery by ≥1 year did not adversely affect outcomes.

Anti-Inflammatory Immune Skewing Is Atheroprotective: Apoe-/-Fc{gamma}RIIb-/- Mice Develop Fibrous Carotid Plaques Vascular Medicine
Conclusions We hypothesized that removal of FcRIIb would exacerbate atherosclerosis and generate unstable plaques. However, we found that deletion of FcRIIb on a congenic C57BL/6 background induces an anti-inflammatory Treg/M2 polarization that is atheroprotective.
