Days spent at home and mortality after critical illness: a cluster analysis using nationwide data
Chest. 2022 Oct 15:S0012-3692(22)03997-6. doi: 10.1016/j.chest.2022.10.008. Online ahead of print.ABSTRACTBACKGROUND: Beyond the question of short-term survival, days spent at home could be considered a patient-centered outcome in critical care trials.RESEARCH QUESTION: What are the days spent at home and healthcare trajectories during the year after surviving critical illness?STUDY DESIGN AND METHODS: Data were extracted on adult survivors spending at least two nights in a French intensive care unit (ICU) during 2018 who were treated with invasive mechanical ventilation and/or vasopressors or inotropes. Trauma, burn, orga...

Patient and aneurysm factors associated with aneurysm recanalization after coiling
Conclusions Several factors are identified by the ARETA study as playing a role in aneurysm recanalization after coiling: current smoking, aneurysm status (ruptured), aneurysm size (≥10 mm), neck size (wide neck), and aneurysm location (middle cerebral artery). This finding has important consequences in clinical practice. Trial registration number URL: http://www.clinicaltrials.gov; Unique Identifier: NCT01942512.

Atrial fibrillation detection with long-term continuous Holter ECG recording in patients with high cardiovascular risk and clinical palpitations: the prospective after study
ConclusionsThe systematic AF screening of patients with palpitations and high cardiovascular risk resulted in a diagnostic yield of AF in 14% of the population with a 14-day continuous ECG –Holter monitor. This strategy resulted in the prescription of anticoagulation and antiarrhythmic therapy in 90% of the AF detected population.Graphical abstract

Projected Impact of Treatment Intensification with Satin, Ezetimibe, and Statin plus Ezetimibe Fixed-Dose Combination on MACE Across Six Countries
CONCLUSION: The 2019 ESC/EAS guideline-based treatment intensification with strategies based on statin, ezetimibe, and statin plus ezetimibe FDC is estimated to result in a substantial population-level benefit in terms of MACE averted compared to BAU.PMID:36134461 | DOI:10.1093/eurjpc/zwac214

Symptomatic Isolated mesenteric artery dissection (IMAD): Insights from a conservative approach
Symptomatic isolated mesenteric artery dissection (IMAD) is a rare vascular disease. Therefore, data on its natural history are scarce. Treatment strategies on whether to stent symptomatic IMAD remain controversial, with recent observational reports advocating an aggressive approach, with a low threshold to stenting. In this issue, Abdallah et al1 provided alternative insights from a French Intestinal Stroke Centre, which adopts a conservative treatment algorithm for symptomatic IMAD.

Effects of healthcare system transformations spurred by the COVID-19 pandemic on management of stroke and STEMI: a registry-based cohort study in France
Conclusions The pandemic did not markedly alter the functioning of the emergency network. Although stroke patient management deteriorated, the resilience of the STEMI pathway was linked to its stronger structuring. Transversal reorganisations, aiming at concentrating resources on emergency care, contributed to maintenance of the quality of care. Trial registration number NCT04979208.

Transcatheter edge-to-edge repair following surgical valve repair with ring implantation: Results from the multicentre "Clip-in-Ring" registry
CONCLUSIONS: TEER in patients with failed mitral ring is feasible and safe. Further studies shoulddelineate its exact role in the therapeutic armamentarium for this medical issue.PMID:36115768 | DOI:10.1016/j.acvd.2022.05.009

Effect of angioplasty and stent placement on sensorineural hearing loss and pulsatile tinnitus in a patient with severe internal carotid artery stenosis
Pulsatile tinnitus occurs due to turbulent blood flow through a stenotic vessel including the internal carotid artery. The presence of sensorineural hearing loss suggests involvement of the posterior circulation vasculature.

Mortality and Morbidity Effects of Long-Term Exposure to Low-Level PM < sub > 2.5 < /sub > , BC, NO < sub > 2 < /sub > , and O < sub > 3 < /sub > : An Analysis of European Cohorts in the ELAPSE Project
CONCLUSIONS: Long-term exposure to PM2.5, NO2, and BC was positively associated with natural-cause and cause-specific mortality in the pooled cohort and the administrative cohorts. Associations were found well below current limit values and guidelines for PM2.5 and NO2. Associations tended to be supralinear, with steeper slopes at low exposures with no indication of a threshold. Two-pollutant models documented the importance of characterizing the ambient mixture with both NO2 and PM2.5. We mostly found negative associations with O3. In two-pollutant models with NO2, the negative associations with O3 were attenuated to esse...
