Standards of Practice in Acute Ischemic Stroke Intervention International Recommendations.
Yoshimura SI, Biscoito L, Pumar M, Diaz O, Fraser J, Lifante I, Liebeskind DS, Nogueira RG, Hacke W, Brainin M, Yan B, Soderman M, Taylor A, Pongpech S, Terbrugge K Abstract After five positive randomized controlled trials showed benefit of mechanical thrombectomy in the management of acute ischemic stroke with emergent large-vessel occlusion, a multi-society meeting was organized during the 17th Congress of the World Federation of Interventional and Therapeutic Neuroradiology in October 2017 in Budapest, Hungary. This multi-society meeting was dedicated to establish standards of practice in acute ischemic stroke ...

Clot Histopathology in Ischemic Stroke with Infective Endocarditis.
CONCLUSION: Detailed morphological and histopathological analysis of EVT-retrieved clots including Gram staining can assist in etiological classification of the clot. Understanding the composition of the clot may be of clinical value in early diagnostics and mapping treatment planning in IE. PMID: 30867075 [PubMed - as supplied by publisher]

The 2018 ter Brugge Lecture: Problems with the Introduction of Innovations in Neurovascular Care.
Abstract Most endovascular innovations have been introduced into clinical care by showing good outcomes in small enthusiastic case series of selected patients. Randomized clinical trials (RCTs) have rarely been performed, except for acute ischemic stroke, but even then most trial designs were too explanatory to inform clinical decisions. In this article, we review 2 × 2 tables and forest plots that summarize RCT results to examine methodological issues in the design and interpretation of clinical studies. Research results can apply in practice when RCTs are all-inclusive, pragmatic trials. Common problems inclu...

Effects of cold temperature and snowfall on stroke mortality: A case-crossover analysis.
CONCLUSION: Our results suggest that cold temperature and snowfall are independent risk factors for death from hemorrhagic stroke in men. These findings imply that interventions to prevent fatal hemorrhagic stroke during winter should include both cold temperature exposure and snowfall in men. Improving Door-to-needle Times in the Treatment of Acute Ischemic Stroke Across a Canadian Province: Methodology
Background: Alteplase is a proven medical treatment for acute ischemic stroke; however, the effectiveness of this treatment is highly time dependent. Therefore, it is imperative that hospitals treat acute ischemic stroke patients as quickly as possible. The measure, door-to-needle time, is the time from hospital arrival to when alteplase administration begins. Objective: The goal in the Canadian province of Alberta was to reduce the door-to-needle time to a median of 30 minutes and to increase the percent of patients treated within 60 minutes to 90%. Overview of Methodology: A modified version of Institute for Heal...

Importance of risk reassessment in patients with atrial fibrillation in guidelines: Assessing risk as a dynamic process
Publication date: Available online 2 February 2019Source: Canadian Journal of CardiologyAuthor(s): Ting-Yung Chang, Gregory Y.H. Lip, Shih-Ann Chen, Tze-Fan ChaoAbstractThe appropriate use of oral anticoagulants (OACs) for stroke prevention in atrial fibrillation (AF) relies on the convenient and accurate stroke risk prediction scheme, namely the CHA2DS2-VASc score. Since AF patients would become older and accumulate more comorbidities, their risk (for example, as reflected by the CHA2DS2-VASc scores) are not static and could increase over time. The available data demonstrated that follow-up and delta CHA2DS2-VASc scores p...

Recurrent stroke: the role of thrombophilia in a large international pediatric stroke population.
Abstract Risk factors for arterial ischaemic stroke in children include vasculopathy and prothrombotic risk factors but their relative importance to recurrent stroke is uncertain. Data on recurrent stroke from the databases held in Canada (Toronto), Germany (Kiel-Lubeck/Munster), and UK (London/Southampton) were pooled. Data were available from 894 patients aged 1 month to 18 years at first stroke (median age 6 years) with a median follow-up of 35 months. 160/894 patients (17.9%) had recurrence from 1 day to 136 months after first stroke (median 3.1 months). Among 288 children with vasculopathy, recurrence was sig...

Incidence and Current Treatment Gaps in Pediatric Stroke and TIA: An Ontario-Wide Population-Based Study
Conclusion: Our population-based study yielded a higher incidence rate than prior North-American studies. Important care gaps exist including delayed diagnosis, lack of expert care, and departure from published treatment guidelines. Variability in ICD PPV and yield underlines the need for prospective data collection and for improving the pediatric stroke and TIA coding processes.Neuroepidemiology 2019;52:119 –127

Modality selection for the revascularization of left main disease
Publication date: Available online 15 December 2018Source: Canadian Journal of CardiologyAuthor(s): Derrick Y. Tam, Faisal Bakaeen, Dmitriy N. Feldman, Philippe Kolh, Gaetano Antonio Lanza, Marc Ruel, Raffaele Piccolo, Stephen E. Fremes, Mario FL. GaudinoAbstractThe management of severe left main (LM) disease remains controversial and continues to evolve as new evidence emerges. Patient selection for CABG or PCI relies on both predicting mortality with CABG from clinical characteristics using the Society of Thoracic Surgeons risk score and anatomical complexity using the SYNTAX score. LM stenting techniques continue to evo...

Links Between Strokes and Hereditary Hemorrhagic Telangiectasia: A Population-Based Study.
This study found a higher HHT prevalence in Alberta (1 in 3800) compared to the world average of 1 in 5000. Women were also more likely to be diagnosed with HHT, with a 3.25:1 female gender preponderance in the yearly incidence.InterpretationThis study not only shows that HHT patients are at higher risk of having a stroke but also quantifies that risk using an age-adjusted metric in Alberta. This province has a higher than expected disease burden of HHT, with the majority of cases affecting women. Our study found that acute ischemic strokes and transient ischemic attacks are far more common than hemorrhage in HHT. As HHT i...

Nurse-staffing level and quality of acute care services: Evidence from cross-national panel data analysis in OECD countries
ConclusionA higher proportion of nurses-staffing level is associated with better performance of acute care services in OECD countries. Also, the nursing characteristics of Sweden, Denmark, Canada, Netherlands, Finland, Switzerland, Australia and United States would be good patterns for other countries to maximize nursing outcomes in the care of patients with acute and life-threatening conditions by reducing the risk of complication, mortality and adverse clinical outcomes.
