Benefit of Warfarin Compared With Aspirin in Patients With Heart Failure in Sinus Rhythm: A Subgroup Analysis of WARCEF, a Randomized Controlled Trial Original Articles
Conclusions— In patients <60 years, warfarin improved outcomes over aspirin with or without inclusion of major hemorrhage. In patients ≥60 years, there was no treatment difference, but the aspirin group had significantly better outcomes when major hemorrhage was included. Stroke/thromboembolism and intracranial hemorrhage in a 'real world' atrial fibrillation population: The CAFBO STUDY (Complications of Atrial Fibrillation in the BOlogna area).
CONCLUSION IEs occurred mainly in elderly patients who received ASA or no treatment. Half of anticoagulated patients with IEs had subtherapeutic INRs. Neck vessel dissection following whiplash-type neck injury
Arterial dissection is a recognised complication of neck manipulation and trauma. In a systematic audit of 148 complex medicolegal whiplash cases 131 had only neck pain, 8 had radiculopathic symptoms, and 9 had symptoms and signs suggesting an arterial dissection of a neck vessel with presumed secondary thromboembolism. Their ages ranged from 28–55. Three had hemipareses supported by scan abnormalities, and one had a probable posterior fossa stroke. Five cases had prolonged symptoms without imaging abnormalities. Anticoagulation Treatment for Stroke Prevention in Atrial Fibrillation Is Increasing, But Further Improvements Needed
A recent paper by Hsu et  al.(1) assessed antithrombotic treatment among atrial fibrillation (AF) patients. The data, from the American College of Cardiology PINNACLE (Practice Innovation and Clinical Excellence) registry between 2008 and 2012, showed that 61.8% of patients with moderate-to-high stroke risk received anticoagulant therapy with either warfarin or newer direct oral anticoagulants (DOACs). The investigators and an editorial commentary noted the alarming prevalence of aspirin-only treatment despite clear evidence that anticoagulants are superior for prevention of thromboembolism in AF (1,2).

Hyperacute Carotid Stenting for Acute Ischemic Stroke After Systemic Thrombolysis with IV rt-PA (P1.282)
Conclusions:These 2 cases demonstrate a successful use of hyperacute carotid stenting and antiplatelet load in patients who received systemic thrombolysis with IV rt-PA. Both had excellent outcomes and no complications. Predictors and Clinical Impact of Delayed Stent Thrombosis after Thrombectomy for Acute Stroke with Tandem Lesions INTERVENTIONAL

A case of ischemic stroke and transient thrombocytopenia in a young female following adenoviral vector-based COVID-19 vaccination: Was the association incidental or causal?
We report a 23-year-old healthy female who developed seizures, altered sensorium, and left hemiparesis, 20 days after receiving the first dose of adenoviral vector-based COVID-19 vaccine "Covishield™." The patient had transient thrombocytopenia. The D-dimer level was 2460 ng/mL. Magnetic resonance imaging (MRI) demonstrated occlusion of M2 segment of the middle cerebral artery and cerebral infarction. Platelet factor-4 antibodies level was normal.
