Renal Impairment and Ischemic Stroke Risk Assessment in Patients With Atrial Fibrillation: The Loire Valley Atrial Fibrillation Project
Conclusions: Renal impairment was not an independent predictor of IS/TE in patients with AF and did not significantly improve the predictive ability of the CHADS2 or CHA2DS2-VASc scores.

The impact of CHADS2 score on late stroke after the Cox maze procedure
Conclusions: The risk of stroke or transient ischemic attack in patients after a surgical Cox maze procedure was low and not associated with CHADS2 score or warfarin use. Given the known risks of warfarin, we recommend discontinuation of anticoagulation 3 months after the procedure if the patient has no evidence of atrial fibrillation, has discontinued antiarrhythmic medications, and is without any other indication for systemic anticoagulation.

Long-term Morbidity and Mortality of Carotid Endarterectomy in Patients with End-stage Renal Disease Receiving Hemodialysis
Conclusions: These data suggest that patients undergoing dialysis were at no greater risk for periprocedural complications when undergoing CEA. Thus, CEA may be effective for stroke prevention in hemodialysis patients.

Robert E. Lee's Last Stand: His Dying Words and the Stroke That Killed Him. (P1.294)
CONCLUSIONS: Lee suffered chronic angina and congestive heart failure, with a probable myocardial infarction during the war. His death stemmed from an acute, possibly cardioembolic stroke manifesting as expressive aphasia, and subsequent respiratory complications. Given his aphasia, Lee’s famous last words are questionable.Disclosure: Dr. Southerland has received personal compensation in an editorial capacity for Neurology Podcast.

Secondary prevention of stroke in elderly people in Poland-Results of PolSenior study.
CONCLUSIONS: Secondary cardiovascular preventive therapy in Poland is used too rarely in older people after stroke, and it especially concerns anticoagulant therapy in those with a history of atrial fibrillation. Structured educational programmes should be developed in Poland to improve usage of secondary preventive therapy in the elderly. PMID: 24821632 [PubMed - in process]

Study On Drug Utilization And Assessment Of Stroke Risk Using Chads2 And Cha2ds2-Vasc Scoring In Elderly Patients With Non-Valvular Atrial Fibrillation
Stroke Risk Stratification in AF patients of can be done using CHADS2 (Congestive heart failure, Hypertension, Age ≥75, DM, prior Stroke/TIA [2 points] ); or CHA2DS2VASc2 (Congestive heart failure/left ventricular ejection fraction ≤35%, Hypertension, Age ≥75 [2 points], DM, prior Stroke/TIA/thromboembolism [2 points], Vascular disease, Age 65–74, Sex- female). Treatment options for Prevention of stroke includes Anti-coagulants (Vitamin K Antagonist-Warfarin, Acenocoumarol; and Newer Oral Anticoagulant- Dabigatran) and anti-platelets (Aspirin and Clopidogrel).

Blood pressure and stroke risk in left ventricular assist devices
Ischemic and hemorrhagic stroke are one of the most devastating outcomes arising from treatment of end-stage congestive heart failure with a left ventricular assist device (LVAD)1. Stroke after LVAD is associated with increased risk of mortality and is a leading cause of not qualifying for transplantation. The risk of stroke is one of many factors limiting more widespread adoption of this technology for end-stage heart failure. The risk factors for stroke in LVAD have been the subject of several studies, with recent infection2 and changes in anti-thrombotics being commonly found3.

New-Onset Paroxysmal Atrial Fibrillation Diagnosis in Ischemic Stroke Patients
Conclusions: This risk prediction model can be helpful to estimate the risk of an underlying pAF within 3 months after suffering an IS/TIA, contributing to increased AF detection efforts, thereby starting the correct secondary prevention treatment.Eur Neurol 2015;74:211-217

Left Atrial Appendage Remodeling After Lariat Left Atrial Appendage Ligation Original Articles
Conclusions— Incomplete LAA ligation after Lariat is common. However, the remodeled LAA cavity is dramatically reduced. Diminished cavity size and tightening of the LAA orifice may play a role in the reduction of thrombus formation.

Atrial Fibrillations Effect on the Rate of Ischemic Stroke, Death and Disability in Subarachnoid Hemorrhage (P1.256)
Conclusion: There are significantly decreased rates of favorable outcomes in subarachnoid hemorrhage patients with atrial fibrillation. Further studies are warranted to better understand the associated factors.Disclosure: Dr. Hassan has nothing to disclose. Dr. Afzal has nothing to disclose. Dr. Chaudhry has nothing to disclose. Cost‐Effectiveness of High‐Dose Edoxaban Compared with Adjusted‐Dose Warfarin for Stroke Prevention in Non–Valvular Atrial Fibrillation Patients
ConclusionsHigh‐dose edoxaban appears to be an economically dominant strategy when compared with adjusted‐dose warfarin for the prevention of stroke in NVAF patients with a Clcr of 15–95 ml/minute and an appreciable risk of stroke.

Reply Comparing the ATRIA, CHADS 2 , and CHA 2 DS 2 -VASc Scores for Stroke Prediction in Atrial Fibrillation
We tested the ATRIA (Anticoagulation and Risk Factors in Atrial Fibrillation), CHADS2 (congestive heart failure, hypertension, age ≥75 years, diabetes mellitus, stroke), and CHA2DS2-VASc (congestive heart failure, hypertension, age ≥75 years, diabetes mellitus, prior stroke or transient ischemic attack, vascular disease, age 65 to 74 years, female) stroke risk scores in the CPRD (Clinical Practice Research Datalink) cohort of incident atrial fibrillation (AF) patients not using oral anticoagulants (OAC) because these are the patients for whom physicians must make the OAC treatment decision (1). The mean patient follo...

Risk of Ischemic Stroke in High Risk Atrial Fibrillation Patients during Periods of Warfarin Discontinuation for Surgical Procedures
Importance: The risk of ischemic stroke during periods of warfarin discontinuation for surgical procedures is recognized but not well characterized. Objective: The study aimed to quantitate the risk of ischemic stroke associated with high risk atrial fibrillation during periods of warfarin discontinuation. Design, Setting and Participants: A cohort of 4,060 patients (mean follow-up period of 3.5 ± 1.3 years) were randomized into the Atrial Fibrillation Follow-Up Investigation of Rhythm Management study. Patients enrolled in the study had atrial fibrillation plus at least one other risk factor for stroke or death: age ≥6...
