A Contemporary Review of Epidemiology, Risk Factors, Etiology, and Outcomes of Premature Stroke
AbstractPurpose of ReviewRecent data identifies increases in young ischemic and hemorrhagic strokes. We provide a contemporary overview of current literature on stroke among young patients or premature stroke along with directions for future investigation.Recent FindingsStrokes in the young are highly heterogenous and often cryptogenic. Sex distribution and risk factors shift from women among the youngest age groups (<  35) to men over the age of 45, with a coinciding rise in traditional vascular risk factors. Incidence is higher in minority and socioeconomically disadvantaged populations, and the impact of stroke amo...

Earlier Anticoagulation Better After Ischemic Stroke in AF? Earlier Anticoagulation Better After Ischemic Stroke in AF?
The observational ATTUNE study suggests that starting anticoagulation within 4 days of stroke leads to fewer new infarcts detected by imaging without an increase in hemorrhage compared with starting later.Medscape Medical News

Association of time to groin puncture with patient outcome after endovascular therapy stratified by etiology
CONCLUSION: Faster groin puncture has a more pronounced effect on the functional outcome in patients of CE subtype than those of LAA subtype. Reducing time to groin puncture is of great importance in improving the prognosis of patients after EVT, especially those of CE subtype, and reducing the incidence of any ICH in all patients.PMID:36299609 | PMC:PMC9590449 | DOI:10.3389/fnagi.2022.884087

Association between high-risk extracranial carotid plaque and covert brain infarctions and cerebral microbleeds
CONCLUSION: Both IPH and plaque ulceration are associated with total, cortical, and lacunar type CBIs but not CMBs suggesting that advanced atherosclerosis contributes predominantly to ischemic markers of subclinical vascular injury.PMID:36278979 | DOI:10.1007/s00234-022-03062-0

Elevated Levels of Serum Alkaline Phosphatase are Associated with Increased Risk of Cardiovascular Disease: A Prospective Cohort Study
CONCLUSIONS: Higher ALP levels, even within the normal range, were significantly associated with increased risks of CVD, in a dose-dependent manner. These findings suggested that regular monitoring of ALP levels may help in improving the early identification of the population at higher CVD risk.PMID:36261365 | DOI:10.5551/jat.63646

Transcranial Doppler ultrasound: Clinical applications from neurological to cardiological setting
J Clin Ultrasound. 2022 Oct;50(8):1212-1223. doi: 10.1002/jcu.23344.ABSTRACTTranscranial Doppler (TCD) ultrasonography is a rapid, noninvasive, real-time, and low-cost imaging technique. It is performed with a low-frequency (2 MHz) probe in order to evaluate the cerebral blood flow (CBF) and its pathological alterations, through specific acoustic windows. In the recent years, TCD use has been expanded across many clinical settings. Actually, the most widespread indication for TCD exam is represented by the diagnosis of paradoxical embolism, due to patent foramen ovale, in young patients with cryptogenic stroke. In addition...

Long-Term Effectiveness of Cilostazol in Patients with Hemodialysis with Peripheral Artery Disease
CONCLUSION: Continuous treatment of cilostazol in patients with HD with PAD significantly decreases the risk of ED visits, hemorrhagic stroke, and cardiovascular events and improves ED visit-free and cardiovascular event-free rates during long-term follow-up.PMID:36216573 | DOI:10.5551/jat.63404
