Efficacy and safety of ticagrelor versus aspirin and clopidogrel for stroke prevention in patients with vascular disease: a systematic review and meta-analysis
Introduction: Currently, it is still controversial to treat stroke with ticagrelor alone. The purpose of our study was to systematically review and analyze the efficacy and safety of ticagrelor on cerebrovascular outcomes in patients with vascular risk factors. Methods: The PubMed, Cochrane Library, and EMBASE databases were systematically searched using the keywords stroke, ticagrelor, clopidogrel, and aspirin to identify randomized controlled trials (RCT). Primary outcomes included reported stroke, ischemic stroke and complex events; the secondary outcome was hemorrhagic stroke. The safety outcomes included major bleedi...

Twenty-four-hour ambulatory blood pressure variability and association with ischemic stroke subtypes in the subacute stage
ConclusionThe results of this study indicate a discrepancy in blood pressure variability among different ischemic stroke subtypes during the subacute stage. Higher systolic blood pressure and systolic blood pressure variability during the 24 h, daytime, and nighttime, and nighttime diastolic blood pressure were independent predictors for large-artery atherosclerosis stroke. Increased nighttime diastolic BPV was an independent risk factor for cardioembolic stroke.

Follow-up Imaging After Thrombolysis: FIAT, A Randomized Trial
Current protocols for treatment of acute ischemic stroke with intravenous thrombolytics, such as alteplase (tPA) and tenecteplase (tNK), recommend the completion of a routine non-contrast head CT at 24 hours post treatment to evaluate for hemorrhage prior to the initiation of antiplatelet therapy for secondary stroke prevention. This guideline was instituted because it had been part of the protocol in the NINDS multicenter randomized placebo-controlled trial that showed the benefit of IV thrombolytics within 3 hours of stroke onset.

Outcome after endovascular treatment for acute ischemic stroke by underlying etiology: Tertiary experience and meta-analysis
ConclusionEtiology was not considered as an important factor in functional outcome, despite the differences in baseline characteristics and technical EVT approach. The current study of anterior circulation AIS-LVO patients supports that outcomes for those with ICAS are not significantly different from those with CE.

Stroke and cancer
Rev Neurol (Paris). 2023 Apr 4:S0035-3787(23)00891-3. doi: 10.1016/j.neurol.2023.03.009. Online ahead of print.ABSTRACTAssociation between stroke and cancer is well-established and has led to a growing literature over the past decades. Risk of ischemic and hemorrhagic stroke is increased among patients with newly diagnosed cancer and 5-10% of stroke patients bear an active cancer. All cancers are concerned, but hematological malignancy in childhood and adenocarcinoma from lung, digestive tract and pancreas in adults are most usually identified. Unique stroke mechanisms are dominated by hypercoagulation, a condition that ma...

Associations between computed tomography markers of cerebral small vessel disease and hemorrhagic transformation after intravenous thrombolysis in acute ischemic stroke patients
ConclusionIn patients with acute ischemic stroke, severe leukoaraiosis, brain atrophy and total CSVD burden may be risk factors for HT after IVT. These findings may help improve efforts to mitigate or even prevent HT in vulnerable patients.

Clinical and radiological characteristics of hiv-associated intracerebral haemorrhage in malawi
Stroke incidence is increasing in sub-Saharan African (SSA), especially due to intracerebral hemorrhage (ICH). We sought to describe the clinical and radiological characteristics of stroke due to ICH in people living with HIV (PLWH) in Malawi.

Time to ct scan imaging after symptom onset among ischeamic stroke patients presenting to a quaternary hospital in ghana
This study sought to determine the time of onset of symptoms to the time a CT imaging was done among patients presenting with acute stroke at the Korle-Bu Teaching Hospital.

Women's reproductive traits and cerebral small-vessel disease: A two-sample Mendelian randomization study
ConclusionOur findings do not convincingly support a causal effect of women's reproductive factors on CSVD. Future studies are warranted to investigate specific estrogen-related physiological changes in women, which may inform current researchers on the causal mechanisms involved in cerebral small-vessel disease progression.

Heart Rate Variability in Patients with Spontaneous Intracerebral Hemorrhage and its Relationship with Clinical Outcomes
ConclusionsHRV values were impaired significantly within 14  days after ICH. Furthermore, HRV indices measured 10–14 days after ICH were independently associated with 3-month outcomes.

Sex Differences in Outcomes of Intravenous Thrombolysis in Acute Ischemic Stroke Patients with Preadmission Use of Antiplatelets
ConclusionNo sex differences were observed in the safety of IVT regarding preadmission use of antiplatelets. Males showed more favourable 3-month functional independence than females; however, this sex difference was apparently not explained by a sex-specific mechanism related to preadmission use of antiplatelets.
