Association between obstructive sleep apnea and cardiovascular diseases
Acta Biochim Biophys Sin (Shanghai). 2022 Jul 25. doi: 10.3724/abbs.2022084. Online ahead of print.ABSTRACTObstructive sleep apnea (OSA) is a common respiratory disorder characterized by partial obstruction of upper respiratory tract and repetitive cessation of breathing during sleep. The etiology behind OSA is associated with the occurrence of intermittent hypoxemia, recurrent arousals and intrathoracic pressure swings. These contributing factors may turn on various signaling mechanisms including elevated sympathetic tone, oxidative stress, inflammation, endothelial dysfunction, cardiovascular variability, abnormal coagul...

Positive Airway Pressure Adherence, Mortality and Cardio-Vascular Events in Sleep Apnea Patients
CONCLUSIONS: These real-life clinical data demonstrate a dose-response relationship between PAP adherence and incident MACEs in OSA. Patients support program may help improve PAP adherence and CV outcomes in patients with OSA.PMID:35816570 | DOI:10.1164/rccm.202202-0366OC

Prevalence and associated factors of obstructive sleep apnea in refractory hypertension
Conclusion: In this large RHT cohort, resistant and refractory hypertensive patients have similar OSA prevalence, although refractory ones, which by definition use spironolactone, are younger and apparently have a better sleep pattern.

Stroke: What's Sleep Got to Do With It?
Summary: Ischemic strokes most often occur between 6 am and 12 am after awakening from sleep but up to 30% occur during sleep. Wake-up strokes (WUS) are new focal neurological deficit(s) persisting for ≥ 24 hours attributable to an ischemic event present on patient awakening. Obstructive sleep apnea (OSA) is a major risk factor for WUS because it compounds the instability of the morning environment and increases the likelihood of cardiovascular events, including hypertension, atrial fibrillation, right-to-left shunts, and stroke. Circadian-driven alterations in structural, homeostatic, and serological factors also pr...

Impact of sleep apnea and treatments on cardiovascular disease
Sleep Sci. 2022 Apr-Jun;15(2):250-258. doi: 10.5935/1984-0063.20220047.ABSTRACTCardiovascular diseases (CVDs) are the leading causes of mortality worldwide, accounting for nearly 18 million deaths per year. Among other considerations, treating CVDs requires better understanding their risk factors. Sleep-disordered breathing, especially obstructive sleep apnea (OSA), is a likely contributor to several CVDs. We review key epidemiological data that addresses the link between OSA and cardiovascular outcomes such as hypertension, atrial fibrillation (AF), stroke, atherosclerosis, and heart failure (HF), and proposed pathophysio...

Sleep and physical activity in relation to all-cause, cardiovascular disease and cancer mortality risk
Conclusion The detrimental associations of poor sleep with all-cause and cause-specific mortality risks are exacerbated by low PA, suggesting likely synergistic effects. Our study supports the need to target both behaviours in research and clinical practice.

Cardiovascular remodeling in obstructive sleep apnea: focus on arterial stiffness, left ventricular geometry and atrial fibrillation
We present an overview of the definition, prevalence, and risk factors of OSA and outline the association between OSA and cardiovascular complications. We then briefly discuss echocardiographic assessment in OSA with focus on the left atrium and LV. Finally, we highlight the importance of adherence to continuous positive airway pressure (CPAP) therapy with regard to reducing the risk of cardiovascular disease.EXPERT COMMENTARY: Although OSA has a strong association with cardiovascular complications, it is often underdiagnosed and undertreated. Patients with resistant hypertension and atrial fibrillation with poor therapeut...
