Carotid artery stenting using a 7 French Optimo balloon guide catheter combined with a distal filter

Mechanical thrombectomy is cost-effective versus medical management alone around Europe in patients with low ASPECTS
Conclusions MT is efficient versus MM alone for patients with low ASPECTS in eight countries across Europe. Patients with a large ischemic core could be treated with MT because it is both clinically beneficial and economically sustainable.

Effect of pharmacologic anti-atherosclerotic therapy on carotid intraplaque neovascularization: a systematic review
Atherosclerotic cardiovascular (CV) disease is one of the leading causes of global mortality. It is a progressive inflammatory disease that involves lipid accumulation in the sub-intimal layer of the arterial wall, resulting in gradual plaque formation1. Plaque rupture is implicated in a large proportion of major adverse cardiovascular events (MACE), including acute coronary syndrome and ischemic stroke. The presence of intraplaque neovascularization (IPN) in large arterial plaques have been associated with an increased risk of rupture and plaque vulnerability.

Poor clinical outcome despite successful basilar occlusion recanalization in the early time window: incidence and predictors
Conclusions Early successful reperfusion with EVT occurred in 80.5% of patients, and the only treatment-related factor predictive of clinical outcome was first pass mTICI 2b-3 reperfusion. Further research is warranted to identify the optimal techniques and devices associated with first pass reperfusion in the posterior circulation.

Thrombectomy in basilar artery occlusions: impact of number of passes and futile reperfusion
Conclusions We found that BAO patients had a significant relationship between the number of MT passes and both recanalization and favorable functional outcome. We further found that the benefit of recanalization in BAO patients was significant only when recanalization was achieved within three passes, encouraging at least three passes before stopping the procedure.
