Proximalization of Inflow With Adjunct Arterial Pressure Measurement for Hemodialysis Access-induced Digital ischemia
Hemodialysis access-induced distal ischemia (HAIDI) is an uncommon, yet potentially devastating complication of hemodialysis access surgery. Symptomatic HAIDI affects about 10% of dialysis patients and can present with mild hand coolness, rest pain, or tissue loss. Volume flow determines the management of HAIDI and may involve banding; revision using distal inflow; proximalization of inflow, distal revascularization; interval ligation; or access ligation. This case report highlights the use of proximalization of inflow with adjunctive intraoperative measurement of distal perfusion to treat severe HAIDI.

62-Year-Old Man With Abdominal Pain and Cloudy Peritoneal Dialysate
A 62-year-old man presented to the emergency department for evaluation for abdominal pain. He has a history of end-stage renal disease (ESRD) secondary to lithium toxicity, now on peritoneal dialysis (PD) for 2 years. His past medical history was also notable for hypertension, obstructive sleep apnea (OSA) adherent to continuous positive airway pressure (CPAP) ventilation, heart failure with reduced ejection fraction caused by idiopathic dilated cardiomyopathy, bipolar disorder, and ongoing tobacco use.

Acute tubular injury in a patient with acetaminophen toxicity
A 31-year-old woman with a history of alcohol abuse presented with nonoliguric acute kidney injury, mild pancreatitis, and diarrhea. Her only medication was acetaminophen, 1 to 2 g every 4 to 6 hours for 4 days for abdominal pain. Admission blood pressure was 122/74 mm  Hg with serum creatinine of 9.16 mg/dl (baseline, 0.5 mg/dl), requiring dialysis. Urine sediment was inactive. Blood ethanol was undetectable. Aspartate and alanine transaminases were>20,000 U/L and 3799 U/L, respectively. Creatine kinase level was normal.

Electronic Decision Support for Deprescribing in Patients on Hemodialysis: Clinical Research Protocol for a Prospective, Controlled, Quality Improvement Study
CONCLUSIONS: Electronic decision support can facilitate deprescribing for the clinical team by providing a nudge reminder, decreasing the time it takes to review and effectuate guideline recommendations, and by lowering the barrier of when and how to taper. Guidelines for deprescribing in the dialysis population have recently been published and incorporated into the MedSafer software.
