Total hip arthroplasty in active and advanced tubercular arthritis: a systematic review of the current evidence
ConclusionTHA is a safe and effective surgical intervention in patients with active and advanced TB arthritis of hip. It is recommended that the surgery be performed under cover of multi-drug anti-tubercular regimen. In patients with active sinus tracts, abscesses and TIS, surgery may be accomplished in a multi-staged manner. The clinical (range of motion, deformity correction, walking ability and pain scores), radiological (evidence of radiological reactivation and implant incorporation) and function outcome (as assessed by HHS) significantly improve after THA in these patients.

(  ±)-Dibrevianamides Q1 and Q2, the key precursors of asperginulin A from a marine-derived fungus
Appl Microbiol Biotechnol. 2023 Sep 2. doi: 10.1007/s00253-023-12739-2. Online ahead of print.ABSTRACTTwo pairs of new dimeric diketopiperazine alkaloids, ( ±)-dibrevianamides Q1 and Q2 (( ±)-1 and ( ±)-2), together with seven previously reported analogues (( ±)-3, 4-6, and ( ±)-7) were obtained from a marine-derived fungus Aspergillus sp. The structures of ( ±)-1 and ( ±)-2 were clarified using comprehensive spectroscopic analyses, the calculated ECD, and DP4 + probability methods. Speculated from the biogenesis, ( ±)-dibrevianamides Q1 and Q2 (( ±)-1 and ( ±)-2) might be the key precursor of [2 + 2] diketopiper...

Pyrrole-thiazolidinone hybrids as a new structural class of broad-spectrum anti-infectives
Eur J Med Chem. 2023 Aug 25;260:115757. doi: 10.1016/j.ejmech.2023.115757. Online ahead of print.ABSTRACTA series of pyrrole-thiazolidinone hybrids was designed, synthesized and evaluated for activities against ESKAP bacteria panel and mycobacterial pathogens. From the series, compound 9d showed prominent activity against S. aureus (MIC = 0.5 μg/mL) and compound 9k showed the most promising activity against M. tuberculosis H37Rv (MIC = 0.5 μg/mL). Potent derivatives were found to be non-toxic when tested against Vero cells. Compound 9d upon evaluation in vitro against several MRSA and VRSA strains produced activity compa...

Tofacitinib versus oral prednisolone for induction of remission in moderately active ulcerative colitis (ORCHID): A prospective, open-label, randomized, pilot study
CONCLUSION: In patients with moderately active ulcerative colitis, there was no difference in the efficacy and safety of tofacitinib and oral prednisolone for induction of remission at 8 weeks.PMID:37656880 | DOI:10.1093/ecco-jcc/jjad153

Factors that impact on recruitment to vaccine trials in the context of a pandemic or epidemic: a qualitative evidence synthesis
CONCLUSIONS: This review identifies many of the factors that influence a person's decision to take part in a vaccine trial, and these reflect findings from reviews that examine trials more broadly. However, we also recognise some factors that become more important in connection with a vaccine trial in the context of a pandemic or epidemic. These factors include the potential stigma of taking part, the possible adverse effects of a vaccine, the added motivation for helping society, the role of community leaders in trial dissemination, and the level of trust placed in governments and companies developing vaccines. These spec...

Clofazimine nanoclusters show high efficacy in experimental TB with amelioration in paradoxical lung inflammation
Biomater Adv. 2023 Aug 21;154:213594. doi: 10.1016/j.bioadv.2023.213594. Online ahead of print.ABSTRACTThe rise of tuberculosis (TB) superbugs has impeded efforts to control this infectious ailment, and new treatment options are few. Paradoxical Inflammation (PI) is another major problem associated with current anti-TB therapy, which can complicate the treatment and leads to clinical worsening of disease despite a decrease in bacterial burden in the lungs. TB infection is generally accompanied by an intense local inflammatory response which may be critical to TB pathogenesis. Clofazimine (CLF), a second-line anti-TB drug, ...

Therapeutic antibodies in rheumatology
Z Rheumatol. 2023 Sep 1. doi: 10.1007/s00393-023-01409-3. Online ahead of print.ABSTRACTEmil von Behring's serum therapy for diphtheria was the first therapeutic use of antibodies. More than 100 years later, a new era in the treatment of rheumatic diseases began in 1998 with the approval of infliximab, an antibody directed against tumor necrosis factor alpha (TNF alpha). The special feature of antibody therapy is the ability to bind and neutralize antigens in a highly specific manner. In addition, target cells can be eliminated by activation of the immune system. These properties of the immune system are exploited in rheum...

Deciphering the evolution of the temporal and geographic distribution of French Mycobacterium bovis genotypes using a high throughput SNP-targeted amplicon sequencing method
This study allowed classifying 618 different genotypic profiles (combination of a spoligotype and 8 loci-MIRU-VNTR profiles) into the nine previously identified clusters. A global analysis of the entire collection of the National Reference Laboratory has made it possible to represent the evolution of clonal complexes and clusters in time and space for better assessing epidemiological changes of bovine tuberculosis in France.PMID:37657678 | DOI:10.1016/j.meegid.2023.105497
