SemEHR: A general-purpose semantic search system to surface semantic data from clinical notes for tailored care, trial recruitment, and clinical research*
ConclusionResults from the multiple case studies demonstrate SemEHR ’s efficiency: weeks or months of work can be done within hours or minutes in some cases. SemEHR provides a more comprehensive view of patients, bringing in more and unexpected insight compared to study-oriented bespoke IE systems. SemEHR is open source, available athttps://github.com/CogStack/SemEHR.

Next-Generation Sequencing of 5' Untranslated Region of Hepatitis C Virus in Search of Minor Viral Variant in a Patient Who Revealed New Genotype While on Antiviral Treatment.
In conclusion, longitudinal analysis of HCV variants, employing two independent deep sequencing methods, suggests that the transient presence of a different genotype strain in PBMC was a result of superinfection and not a selection of pre-existing minor variant. Hepatitis C point-of-care diagnostics: in search of a single visit diagnosis.
Authors: Grebely J, Applegate TL, Cunningham P, Feld JJ Abstract INTRODUCTION: The availability of simple, tolerable, therapies for hepatitis C virus (HCV) infection with responses >95% is one of the greatest medical advances in decades, offering an opportunity to reverse the rising burden due to HCV and strive towards HCV elimination. A key challenge moving forward will be to ensure that those who are undiagnosed are made aware of their infection, receive HCV therapy and achieve viral cure. The availability of point-of-care tests for HCV infection has the potential to simplify testing algorithms, increase diagn...

An update on recent developments in the search for hepatitis C virus therapies with pan-genotypic efficacy.
Authors: Borgia G, Scotto R, Buonomo AR Abstract INTRODUCTION: Pangenotypic drugs against HCV infections led to high rates of virologic response with very limited adverse events. Moreover, they are easy to administer among all HCV genotypes with minimal changes in treatment schedule. For this reason, the demand for new antivirals is greatly lowered. Areas covered. We discuss results from trials assessing safety and efficacy of Ravidasvir, which is the only direct-acting antiviral currently in clinical development. Expert opinion. The currently available pangenotypic drug combinations are very effective in HCV eradi...

Vertical transmission of hepatitis C virus: Current knowledge and perspectives.
Authors: Yeung CY, Lee HC, Chan WT, Jiang CB, Chang SW, Chuang CK Abstract Hepatitis C virus (HCV) infection is a major global health issue. Infection by the HCV can cause acute and chronic liver diseases and may lead to cirrhosis, hepatocellular carcinoma or liver failure. The World Health Organization estimates that approximately 3% of the world population have been infected with HCV and the worldwide prevalence is between 1% and 8% in pregnant women and between 0.05% and 5% in children. Following the introduction of blood product screening, vertical transmission becomes the leading cause of childhood HCV infecti...

Antiviral therapy for hepatitis C: Has anything changed for pregnant/lactating women?
Authors: Spera AM, Eldin TK, Tosone G, Orlando R Abstract Hepatitis C virus (HCV) affects about 3% of the world's population, with the highest prevalence in individuals under 40. The prevalence in pregnant women varies with geographical distribution (highest in developing countries). Prevalence also increases in sub-populations of women at high risk for blood-transmitted infections. HCV infection in pregnancy represents a non-negligible problem. However, most of the past antiviral regimens cannot be routinely offered to pregnant or breastfeeding women because of their side effects. We briefly reviewed the issue of ...

Detection of Hepatitis C virus and the risk of transmission among pregnant and nursing mothers from rural and urban communities in Kogi State, Nigeria.
This study found an overall rate of 4.6% for HCV and 2.2% (4/176) viremia indicating both active and passive infections. HCV rate was higher among the civil servants (2.3%; CI = -0.25-2.91; P= 0.241) and peaked among the age group 31-35 years (2.3%; CI = 0.183-2.182; P= 0.293). Various risk factors identified included, relatively high HCV rates during first trimester (1.7%; CI = -2.2-3.61; P= .047), ear/nose piercing (4.6%; CI = -46.83-54.82; P= 0.157), seropositivity among the married (3.9%; CI = -3.36-7.3567; P= 0.238) and urban dwellers (2.8%; CI = -8.71-16.71; P= 0.157). None of the bio-socio-demographic variables exce...

Methadone Treatment of Opiate Addiction: A Systematic Review of Comparative Studies
Conclusion and Recommendations More intervention-based clinical trials are needed to provide a strong alternative candidate therapy for management of opiate use disorder. Buprenorphine, although a promising candidate, should be researched more regarding treatment retention and patient satisfaction before any conclusions can be made regarding its standard use for opiate substitution maintenance treatment of pregnant patients with opiate use disorder. Heroin-assisted treatment still cannot be considered in opiate addicts refractory to MMT; more data should be collected regarding efficacy in larger participant populations un...

Diagnostic Biomarkers to Diagnose Acute Allograft Rejection After Liver Transplantation: Systematic Review and Meta-Analysis of Diagnostic Accuracy Studies
Conclusion: IL-2 receptor expression demonstrated the highest diagnostic accuracy, while the peripheral eosinophil count was the only marker tested in more than one study. Presently, liver biopsy remains superior to noninvasive diagnostic biomarkers as most studies exhibited inferior designs, hindering possible translation into clinical application. Introduction Liver transplantation (LT) exhibits 20-year survival rates of up to 50% (1) and is of great clinical importance. LT is the treatment of choice for acute or acute-on-chronic liver failure, while organ replacement therapies are still waiting for the break (2)...
