Consensus of an ACAAI, AAAAI, and ATS Workgroup on Definition of Clinical Remission in Asthma on Treatment
As more and better therapies for the treatment of asthma are developed, the approach to classification of asthma control has been in transition. With the availability of inhaled corticosteroids (ICS) as a treatment class came the introduction of control measures utilizing guideline-based criteria, which included validated tools such as the Asthma Control Test (ACT), Baseline Asthma Impairment and Risk Questionnaire (AirQ) and the Asthma Control Questionnaire (ACQ)1,2,3. Following the addition of long-acting bronchodilator (LABA) therapy, Bateman and colleagues described a measure of total control in the GOAL study, definin...

Managing Anaphylaxis – Epinephrine, Antihistamines, and Corticosteroids: Over 10 years of C-CARE Registry Data
Epinephrine is the first-line treatment for anaphylaxis but is often replaced with antihistamines or corticosteroids. Delayed epinephrine administration is a risk factor for fatal anaphylaxis. Convincing data on the role of antihistamines and corticosteroids in anaphylaxis management is sparse.

The chronic rhinosinusitis practice parameter
Ann Allergy Asthma Immunol. 2023 Sep;131(3):307-310. doi: 10.1016/j.anai.2022.12.022. Epub 2022 Dec 17.ABSTRACTThe Allergy-Immunology Joint Task Force on Practice Parameters has published the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) guidelines for the medical management of chronic rhinosinusitis with nasal polyposis (CRSwNP). The practice parameter provides evidence-based guidelines on the use of intranasal corticosteroids (INCS) and biologics for CRSwNP, and aspirin therapy after desensitization (ATAD) for the management of aspirin-exacerbated respiratory disease (AERD). Evidence on surge...

From the Pages of AllergyWatch
For this edition of From the Pages of AllergyWatch I've chosen articles from AllergyWatch focusing on atopic dermatitis, the main theme of this month's Annals. The first article, with comments from Dr. Spriet, investigates various biomarkers in stratum corneum tape strips to predict development of atopic dermatitis in infants and young children. The next article, with comments from Dr. Otani, evaluates the need for closer monitoring of the use of corticosteroids in patients with atopic dermatitis.

An Overview of Nanotechnological Approaches for the Diagnosis and Treatment of Allergic Illness
Curr Pharm Des. 2023 Aug 28. doi: 10.2174/1381612829666230828104015. Online ahead of print.ABSTRACTAllergies are a major health issue. Allergen avoidance, antihistamines, and corticosteroids do not treat the pathology's causes, therefore long-term therapy is essential. Long-term allergen-specific immunotherapy builds immune tolerance to the allergen. Unfortunately, immunotherapies for all allergens are not available, and adverse reactions during therapy, especially in severely allergic persons, remain a worry. In this regard, cell and bio- or nanomaterial-based allergy treatments are promising. This overview covers the mos...

Perception of burden of oral and inhaled corticosteroid adverse effects on asthma-specific quality of life
CONCLUSION: The most burdensome AEs may not be the AEs that would cause patients to stop therapy. Furthermore, qualitative focus groups suggest a psychosocial burden associated with ICS, which needs further investigation.PMID:37643678 | DOI:10.1016/j.anai.2023.08.595

Allergic rhinitis guidelines knowledge, attitudes, and practices among primary health care physicians: A national multicentre cross-sectional study
CONCLUSION: Our study demonstrates the importance of education and awareness for PHCPs managing AR. ARIA guidelines should be implemented as a standard of care for AR, as PHCPs are the first ones to encounter patients with AR, to improve outcomes and avoid undertreatment and complications.PMID:37636198 | PMC:PMC10451594 | DOI:10.4103/jfmpc.jfmpc_85_23

Analysis of comorbidities and multimorbidity in adult patients in the International Severe Asthma Registry
CONCLUSION: In a global study, comorbidity or multimorbidity is reported in most adults with severe asthma and is associated with poorer asthma-related outcomes.PMID:37640263 | DOI:10.1016/j.anai.2023.08.021

Perception of burden of oral and inhaled corticosteroid adverse effects on asthma-specific quality of life
: A multistakeholder core outcome set created for asthma trials showed that asthma specific quality of life (QoL) was a critically meaningful outcome. However, the definition and how to measure it was undetermined. The adverse effects (AEs) of corticosteroids may be a vital clinical trial outcome. Yet, the AE burden from the patient perspective has not yet been elucidated in an asthma population.
