Food Protein-induced Enterocolitis Syndrome Due to Cuttlefish in a Child with Anaphylaxis to Crustaceans
This report illustrates a rare case of a 6-year-old girl with challenge-proven acute food protein-induced enterocolitis syndrome (FPIES) to cuttlefish (phylum Mollusca, class Cephalopoda), anaphylaxis to crustaceans (phylum Arthropoda), and tolerance to other mollusks, including clams and mussels (phylum Mollusca, class Bivalvia). Clarification of food allergy diagnosis at a military medical center using oral food challenges
Food allergies have risen in the past decades, affecting 8% of children in Western countries and 4% of adults.1-3 An increased prevalence of food allergies has also been found in the military population, which has career implications including disqualification from service or specific military occupations.1 An accurate food allergy diagnosis is vital to provide appropriate education and management recommendations and limit life-threatening reactions or unnecessary dietary restrictions. Oral food challenges (OFCs) are important for the military population to clarify eligibility for entry and retention for service.

Data Extraction and Handling Issues on Evidence Synthesis of Risk of Immunoglobulin E –Mediated Food Allergy
To the Editor The systematic review and meta-analysis by Scarpone and colleagues exploring the association between earlier introduction of allergenic foods and risk of immunoglobulin E (IgE) –mediated food allergy is interesting but not scientifically sound. This may result in less reliable results and conclusions.

In Reply We are grateful to Xiao et al for raising some methodological issues about our recent systematic review and meta-analysis. Four separate issues are raised. The first is the definition of immunoglobulin E (IgE) –mediated food allergy. The authors are correct that sensitization is often used as part of an assessment of IgE-mediated food allergy. Cortisol synergizing with endoplasmic reticulum stress induces regulatory T-cell dysfunction
In this study, blood samples were collected from patients with food allergy (FA). Immune cells were purified from blood specimens by flow cytometry. A mouse model of FA was established with ovalbumin as a specific antigen. We observed that serum cortisol levels of FA patients were negatively correlated with peripheral Treg counts. Overwhelmed ER stress status was detected in Tregs of FA patients. The antigen-specific immune response induced ER stress in Tregs, which was exacerbated by concurrent cortisol exposure.
