Low interobserver agreement among subspecialised breast pathologists in evaluating HER2-low breast cancer
CONCLUSIONS: Subspecialised breast pathologists have suboptimal agreement for immunohistochemical evaluation of HLBC using the modified Ki-67 methodology. An urgent need remains for a new assay/algorithm to reliably evaluate HLBC.PMID:37714693 | DOI:10.1136/jcp-2023-209055

Modelling the rate of trainees transitioning to Fellowship before achieving competence under the RANZCP's Alternative Assessment Pathway to the Objective Structured Clinical Examination
CONCLUSIONS: Given possible increases in pre-competent trainees progressing to Fellowship while alternatives to the OSCE are finalised, confidence in the RANZCP's training program demands robust public analyses of those alternatives.PMID:37714141 | DOI:10.1177/10398562231202120

From the Flight Deck to the Bedside: Core Aviation Concepts Applied to Acute Care Physical Therapist Practice and Education
Phys Ther. 2023 Sep 15:pzad125. doi: 10.1093/ptj/pzad125. Online ahead of print.ABSTRACTThe health care industry has sought to reduce errors and patient harm by drawing upon human factors research developed largely in the aviation industry. However, literature that explicitly applies human factors concepts to physical therapist practice is scarce. Therefore, this clinical perspective highlights key concepts in aviation safety and training that should be considered for application to physical therapist education and practice in the acute care setting, including the law of primacy, pilot in command, crew resource management,...

Decision tree analysis accurately predicts discharge destination after spinal cord injury rehabilitation
CONCLUSIONS: D-CART and A-CART showed comparable prediction accuracies. This suggests that, even during the early stages of rehabilitation, it is possible to predict the discharge destination.PMID:37714507 | DOI:10.1016/j.apmr.2023.08.010

Functional outcome prediction after spinal cord injury using ensemble machine learning
CONCLUSIONS: Our study revealed that functional prognostication could be achieved using machine-learning methods with features present at the time of rehabilitation admission. Goals can be set at the beginning of rehabilitation. Moreover, our model can be utilized to evaluate advanced medical treatments, such as regenerative medicine.PMID:37714506 | DOI:10.1016/j.apmr.2023.08.011

The value of radiomics model based on ultrasound image features in the differentiation between minimal breast cancer and small benign breast masses
CONCLUSION: The radiomics model based on ultrasound image features has a satisfied predictive ability for small breast masses, and is expected to become a potential tool for the diagnosis of MBC, and it is a zero cost (in terms of patient participation and imaging time).PMID:37712556 | DOI:10.1002/jcu.23556

Cognitive-exercise dual-task intervention ameliorates cognitive decline in natural aging rats via inhibiting the promotion of LncRNA NEAT1/miR-124-3p on caveolin-1-PI3K/Akt/GSK3 β Pathway
This study aimed to investigate the effects of CEDI and the role of regulation of the lncRNA NEAT1/miR-124-3p on the caveolin-1-PI3K/Akt/GSK3β pathway in CEDI improving cognitive function. Forty 18-month-old natural aging rats were randomly assigned to four groups: exercise training group, cognitive training group, CEDI group, and aging control group, and underwent 12 weeks of intervention. A novel object recognition test was performed to determine the cognitive function, and the hippocampus was separated three days after the behavioral tests for further molecular detection. In an in vitro study, the mouse hippocampal neu...

Cardiomyocyte and endothelial cells play distinct roles in the tumor necrosis factor (TNF)-dependent atrial responses and increased atrial fibrillation vulnerability induced by endurance exercise training in mice
CONCLUSIONS: Our findings demonstrate that adverse atrial remodeling and AF vulnerability induced by intense exercise requires TNF in atrial cardiomyocytes whereas the impact of endothelial-derived TNF is limited to hypertrophy modulation. The implications of the cell autonomous effects of TNF and crosstalk between cells in the atria are discussed.TRANSLATIONAL PERSPECTIVE: Endurance sport is associated with atrial fibrillation (AF) and mouse models show intense exercise training promotes atrial hypertrophy, fibrosis, inflammation, and AF vulnerability, which requires the mechanosensitive inflammatory cytokine tumor necros...
