Transcatheter aortic valve implantation in patients on corticosteroid therapy
AbstractTranscatheter aortic valve implantation (TAVI) is recommended for patients who are inoperable or at high risk for surgical aortic valve replacement (SAVR). Corticosteroid therapy is considered to be a risk factor for SAVR, but there is a paucity of information about TAVI in patients taking corticosteroids. The aim of this study is to elucidate the outcome of TAVI in patients on chronic corticosteroid therapy, compared with SAVR. We retrospectively analyzed patients on corticosteroid therapy who underwent TAVI (n = 21) or SAVR (n = 30) for severe aortic stenosis in Sakakibara Heart Institute. Primary outcome was...

Coagulation Profile as a Risk Factor for 30-Day Morbidity and Mortality Following Posterior Lumbar Fusion
Study Design. A retrospective cohort study. Objective. The aim of this study was to identify associations between abnormal coagulation profile and postoperative morbidity and mortality in patients undergoing posterior lumbar fusion (PLF). Summary of Background Data. The literature suggests that abnormal coagulation profile is associated with postoperative complications, notably the need for blood transfusion. However, there is little research that directly addresses the influence of coagulation profile on postoperative complications following PLF. Methods. The American College of Surgeons National Surgical Quality Impro...

Transcatheter aortic valve implantation compared with surgical aortic valve replacement in patients with anaemia.
CONCLUSIONS: These results suggest that TAVI is not superior to SAVR in patients with anaemia. PMID: 28812435 [PubMed - as supplied by publisher]

Coronary Perforation Complicating Percutaneous Coronary Intervention in Patients With a History of Coronary Artery Bypass Surgery: An Analysis of 309 Perforation Cases From the British Cardiovascular Intervention Society Database Coronary Interventions
Conclusions— CP is an infrequent event during PCI-CABG but is closely associated with adverse clinical outcomes. A legacy effect of perforation on 12-month mortality was observed.

Comparative outcomes of transcatheter aortic valve replacement in African American and Caucasian patients with severe aortic stenosis
ConclusionsThere was no significant difference in the utilization rates, in‐hospital outcomes, and cost of TAVR between Caucasians and AA patients in contemporary US practice. Further comparative studies of surgical and TAVR in AAs and other racial minorities are warranted.

Clinical and hematological profile in a newborn cohort with hemoglobin SC.
CONCLUSIONS: Clinical severity is variable in SC hemoglobinopathy. Several children have severe manifestations similar to those with SS disease. PMID: 29195085 [PubMed - as supplied by publisher]

A model-based cost-effectiveness analysis of Patient Blood Management.
DISCUSSION: Our results indicate that PBM may be associated with fewer adverse clinical outcomes compared to control management and may, thereby, be cost-effective. PMID: 29517965 [PubMed - as supplied by publisher]

Transfusion-transmitted malaria masquerading as sickle cell crisis with multisystem organ failure.
CONCLUSIONS: Clinical suspicion for transfusion-related adverse events, including hemolytic transfusion reactions and transfusion-transmitted infections, should be high in typically and atypically immunocompromised patient populations (like SCD), especially those on chronic transfusion protocols. Manual blood smear review aids in the evaluation of patients with SCD presenting with severe vaso-occlusive crisis and MSOF and can alert clinicians to the need for initiating aggressive therapy like RBC exchange and artesunate therapy. In-hospital mortality and morbidity of pediatric scoliosis surgery in Japan: Analysis using a national inpatient database
Several previous reports have elucidated the mortality and incidence of complications after pediatric scoliosis surgery using nationwide databases. However, all of these studies were conducted in North America. Hence, this study aimed to identify the incidence and risk factors for in-hospital mortality and morbidity in pediatric scoliosis surgery, utilizing the Diagnosis Procedure Combination database, a national inpatient database in Japan. We retrospectively extracted data for patients aged less than 19 years who were admitted between 01 June 2010 and 31 March 2013 and underwent scoliosis surgery with fusion. The primar...

Unicompartmental knee replacement – An underrated alternative of total knee replacement: A matched comparative study analysing their benefits and risks in local population
Conclusion Because of less operative risk and faster rehabilitation, this study suggested that unicompartmental knee replacement is more cost effective and might be a better choice for patients with unicompartmental arthritis in local population.

Comparison of immediate extubation versus ultrafast tracking strategy in the management of off-pump coronary artery bypass surgery.
Conclusion: IE appears to be safe and effective in OPCAB patients without any major complications. It can be achieved after fulfilling traditional extubation criteria but is confined to highly selective group of patients. PMID: 29652272 [PubMed - in process]

Off-Pump Coronary Artery Bypass Grafting with Mini-Sternotomy in the Treatment of Triple-Vessel Coronary Artery Disease.
Authors: Su P, Gu S, Liu Y, Zhang X, Yan J, An X, Gao J, Xin Y, Zhou J Abstract We have developed off-pump coronary artery bypass approach with lower distal mini-sternotomy (TM-OPCAB) for multivessel coronary revascularization. The aim of this retrospective study is to provide evidence for the feasibility and safety of this technique in the treatment of triple-vessel diseases. Two hundred eighty-eight patients with triple-vessel coronary artery disease who underwent TM-OPCAB or standard off-pump coronary artery bypass surgery (S-OPCAB) were included in this study after propensity-score matching. We retrospectively r...

Gender Disparities with the Use of Percutaneous Left Ventricular Assist Device in Patients Undergoing Percutaneous Coronary Intervention Complicated by Cardiogenic Shock: From pLVAD Working Group
Conclusions Despite the small number of cohorts, this study did not reveal any significant differences among gender with the use of percutaneous left ventricular assist devices for PCI in patients with acute myocardial infarction complicated by CS. However, initiation of Impella prior to PCI may be associated with improved mortality and morbidity in both genders.
