Cross-jurisdictional pandemic management: providers speaking on the experience of Nunavut Inuit accessing services in Manitoba during the COVID-19 pandemic
Int J Circumpolar Health. 2023 Dec;82(1):2259122. doi: 10.1080/22423982.2023.2259122. Epub 2023 Sep 20.ABSTRACTAcross Canada, the COVID-19 pandemic placed considerable stress on territorial and provincial healthcare systems. For Nunavut, the need to continue to provide access to critical care to its citizens meant that medical travel to provincial points of care (Edmonton, Winnipeg and Ottawa) had to continue through the pandemic. This complexity created challenges related to the need to keep Nunavut residents safe while accessing care, and to manage the risk of outbreaks in Nunavut resultant from patients returning home. ...

Chronic Respiratory Diseases as a Risk Factor for Herpes Zoster Infection
CONCLUSIONS: In a standard clinical practice setting, the most prevalent respiratory diseases (asthma, COPD and lung cancer) are related to a higher risk of HZ and PHN. These data are fundamental to assess the potential impact of vaccination in this population.PMID:37734964 | DOI:10.1016/j.arbres.2023.08.010

Machine Learning Algorithms Predict Long-Term Postoperative Opioid Misuse: A Systematic Review
CONCLUSIONS: ML algorithms have demonstrated promising potential as a decision-supportive tool in predicting prolonged opioid use in post-surgical patients. Further validation studies would allow for their confident incorporation into daily clinical practice.PMID:37732536 | DOI:10.1177/00031348231198112

Tapentadol versus Oxycodone for Opioid-related Adverse Drug Events and Clinical Outcomes After Inpatient Surgery
Oxycodone is a commonly prescribed opioid for postoperative pain. However, there has been a marked increase in the use of tapentadol over the previous decade due to a perceived superior safety profile of tapentadol compared to oxycodone. There is limited real-world evidence on the safety of tapentadol compared to oxycodone after surgery. The primary objective was to examine the impact of tapentadol compared to oxycodone use on the incidence of opioid-related adverse drug events after surgery. Data for adult surgical patients receiving tapentadol or oxycodone during hospitalisation between 1 January 2018 and 31 December 202...

Echocardiographic Evaluation of Cardiac Masses
AbstractPurpose of ReviewCardiac masses encompass a broad range of etiologies and are often initially revealed by echocardiography. The differential may change depending on the location of the mass and patients ’ medical history or presentation. It is important for clinicians to be aware of subtle visual characteristics on echocardiography in order to correctly diagnose the pathology.MethodsPatients who underwent transthoracic echocardiography and were found to have one or more cardiac masses between January 1, 2020, and May 15, 2023, were reviewed. Their demographic data, clinical presentation, medical history, imaging,...

Pharmacokinetics/Pharmacodynamics of dabrafenib and trametinib for redifferentiation and treatment of radioactive-iodine-resistant mutated advanced differentiated thyroid cancer
CONCLUSIONS: Our data suggest a relationship between DLT onset and trametinib plasma exposure, as well as an association between achievement of RAI reuptake and dabrafenib plasma exposure (AUC and ratio of AUCOHD/DAB). These data imply that the use of plasma drug monitoring could be helpful in guiding clinical practice in redifferentiation treatment.PMID:37725566 | DOI:10.1089/thy.2023.0228

Disparities in misclassification of race and ethnicity in electronic medical records among patients with traumatic injury - Conrick KM, Mills B, Schreuder AB, Wardak W, Vil CS, Dotolo D, Bulger EM, Arbabi S, Vavilala MS, Moore M, Rowhani-Rahbar A.
Systems-level barriers to self-reporting of race and ethnicity reduce the integrity of data entered into the medical record and trauma registry among patients with injuries, limiting research assessing the burden of racial disparities. We sought to charact...

Routine breast milk monitoring using automated molecular assay system reduced postnatal CMV infection in preterm infants
In this study, we evaluated the analytical and clinical performance of the measurement procedure of CMV load in breast milk utilizing the Cobas CMV test on the Cobas 6,800 system. This was enabled by incorporating a simple independent sample preparation procedure before the application of samples on the automated assay system. Clinical data from electronic medical records were retrospectively analyzed. Breast milk samples from mothers of preterm infants born before 33 weeks of gestation were screened for CMV using the automated assay system. CMV positivity rates in breast milk and neonatal samples and the CMV transmissio...

COVID-associated complications after reconstructive breast surgery: a retrospective cohort study
ConclusionPerioperative COVID-infection increases susceptibility to complicated wound healing after reconstructive breast surgery. A possible explanation lies in the dysregulation of haemostasis by the virus, and its direct effects on microvasculature. A hypercoagulable state results. We recommend to postpone elective breast surgery for 4 –6 weeks after COVID-19 infection. Also, precautionary measures remain important to minimize the risk of perioperative COVID-19 infection.

Third-line treatment patterns and clinical outcomes for metastatic colorectal cancer: a retrospective real-world study
CONCLUSION: Chemotherapy combined with or without targeted therapy remained dominated third-line choice and showed favorable efficacy compared with anti-angiogenic monotherapy. With the application of more types and quantities of effective drugs, patients would achieve better survival.PMID:37720594 | PMC:PMC10501067 | DOI:10.1177/20406223231197311
