Hard Times in the Country
The AP recently reported onthe disappearance of maternity wards in rural areas: Alisha Alderson placed her folded clothes and everything she needed for the last month of her pregnancy in various suitcases. She never imagined she would have to leave the comfort of her home in rural eastern Oregon just weeks before her due date. But following the abrupt closure in August of the only maternity ward within 40 miles, she decided to stay at her brother ’s house near Boise, Idaho — a two-hour drive through a mountain pass — to be closer to a hospital. . . .A growing number of rural hospitals have been shutt...

Telephonic intervention to combat non-adherence to oral iron-folic acid supplementation in pregnancy: A randomized controlled trial
CONCLUSION: In addition to improving adherence to oral IFAS, telephonic intervention mitigates side effects and enhances hemoglobin in anemic pregnant women. The increase in adherence was threefold in the experimental group compared to a marginal rise in the control group. This study recommends the implementation of a telephonic intervention to promote adherence to oral IFAS among anemic pregnant women.PMID:37736306 | PMC:PMC10509657 | DOI:10.1016/j.eurox.2023.100235

Plasticity of gastrointestinal vagal afferents in terms of feeding-related physiology and pathophysiology
J Physiol. 2023 Sep 22. doi: 10.1113/JP284075. Online ahead of print.ABSTRACTGastrointestinal vagal afferents play an important role in communicating food related information from the gut to the brain. This information initiates vago-vagal reflexes essential for gut functions, including gut motility and secretions. These afferents also play a role in energy homeostasis, signalling the arrival, amount and nutrient composition of a meal to the central nervous system where it is processed ultimately leading to termination of a meal. Vagal afferent responses to food related stimuli demonstrate a high degree of plasticity, resp...

Pregnancy and progression of differentiated thyroid cancer: A propensity score-matched retrospective cohort study
CONCLUSIONS: Pregnancy seemed to be irrelevant to DTC progression-free survival before surgical treatment. Further prospective cohort studies are needed to translate this finding into clinical practice.PMID:37738427 | DOI:10.1210/clinem/dgad557

Predictors of compliance with higher dose omega-3 fatty acid supplementation during pregnancy and implications for the risk of prematurity: exploratory analysis of the ORIP randomised trial
Conclusions Results in compliers were similar to those from intention-to-treat analyses, suggesting that non-compliance was not a major factor in explaining outcomes from the ORIP trial. Trial registration number ACTRN12613001142729.

Association of maternal thyroid peroxidase antibody during pregnancy with placental morphology and inflammatory and oxidative stress responses
ConclusionThere may be trimester-specific associations between maternal TPOAb levels and placental morphology and inflammatory and oxidative stress responses. The effect of maternal TPOAb levels on placental morphology is present throughout pregnancy. Early pregnancy may be the critical period for the association between maternal TPOAb levels and placental inflammatory and oxidative stress responses.

Assisted reproductive technology outcomes and management considerations for people with epilepsy
Curr Opin Endocrinol Diabetes Obes. 2023 Sep 22. doi: 10.1097/MED.0000000000000838. Online ahead of print.ABSTRACTPURPOSE OF REVIEW: Epilepsy is a common neurologic condition amongst people with reproductive capacity, and assisted reproductive technology (ART) is being increasingly utilized. A recent population-based study described favorable outcomes of ART in this population, but other work shows how fluctuating sex hormones may influence medication concentrations and seizures.RECENT FINDINGS: This review discusses hormonal influences on epilepsy and catamenial epilepsy, considerations regarding fertility in women with e...

Prediction of cytology-histology discrepancy when Bethesda cytology reports benign results for thyroid nodules in women: with special emphasis on pregnancy
Conclusions: Risk of malignancy in a presumably benign thyroid disease should not be ignored. Radiology-cytology correlation by an experienced dedicated team may help in decreasing sampling errors. Physiologic changes caused by pregnancy may shade malignant transformation in thyrocytes, and it would be appropriate to be cautious about benign FNA taken during this period.PMID:37731357 | DOI:10.1080/19932820.2023.2258670

Relationships of pregnancy and postpartum diet quality with offspring birth weight and weight status through 12  months
CONCLUSIONS: Maternal diet quality during pregnancy (and during postpartum in mothers who breastfed for a longer duration) was inversely related to LGA and weight status indicators from birth through 12 months. Increasing maternal diet quality may have use for promoting healthy infant weight development.PMID:37731285 | DOI:10.1002/oby.23891
