Postapproval Outcomes: The Berlin Heart EXCOR Pediatric in North America
The Berlin Heart EXCOR Pediatric Ventricular Assist Device (BH) was approved for use in the United States in December 2011, based on a prospective investigational device exemption (IDE) trial. Strict exclusion criteria for the trial selected a low-risk “ideal” cohort. We sought to determine whether postapproval usage of the BH in a “real world” cohort of recipients would result in similar outcomes. Preimplant diagnostic information was collected for all patients. Efficacy was evaluated by comparison of all children (efficacy group, n = 247) implanted between FDA approval and April 2015 to those in the IDE trial (ID...

Left ventricular wall motion abnormalities: What can they tell us about stroke recurrence?
Left ventricular wall motion abnormalities (LVWMAs) are commonly observed in a variety of medical conditions, including coronary artery disease, congestive heart failure, stress-induced cardiomyopathy, myocarditis, chronic renal disease, and stroke.1–4 Their underlying disease mechanisms and their potential causative role for stroke remain inadequately elucidated. LVWMAs may directly increase stroke risk through thrombus formation in the left ventricle5 or may merely indicate the burden of systemic atherosclerosis underlying stroke, and not the primum movens.

Galectins - a potential target for cardiovascular therapy.
Abstract Cells constantly adapt to external humoral cues like cytokines and hormones, but practically most cellular behavior is under locally guided control via cell-cell interactions. Galectins (Gals) are one of the most prominent members of the group of molecules involved in this intercellular signaling. They are the family of β-galactoside specific lectins and consist of 15 different types, each with a specific function. They play crucial role in the immune system, inflammation, wound healing and carcinogenesis. In recent times, the role of Gals in the development of cardiovascular disease (CVD) has gained att...

Myocardial fibrosis detected with Gadolinium Delayed Enhancement in Cardiac Magnetic Resonance Imaging and Ventriculoarterial Coupling alterations in patients with Acute Myocarditis.
CONCLUSION: In acute myocarditis patients, DGE is related to neither EF nor VAC; however, EF significantly affects the VAC status. Further studies are needed to investigate the potential quantitative, rather than qualitative, relationships between these variables. PMID: 28081949 [PubMed - as supplied by publisher]

Application of Cardiac Troponin in Cardiovascular Diseases Other Than Acute Coronary Syndrome Reviews
BACKGROUND: Increased cardiac troponin concentrations in acute coronary syndrome (ACS) identify patients with ongoing cardiomyocyte necrosis who are at increased risk. However, with the use of more precise assays, cardiac troponin increases are commonly noted in other cardiovascular conditions as well. This has generated interest in the use of cardiac troponin for prognostic assessment and clinical management of these patients. In this review, we have summarized the data from studies investigating the implications of cardiac troponin concentrations in various acute and chronic conditions beyond ACS, i.e., heart failure, my...

Experiences With Aggressive Cardiac Rehabilitation in Pediatric Patients Receiving Mechanical Circulatory Supports.
Authors: Amao R, Imamura T, Sawada Y, Endo S, Ozaki S, Okamura K, Masuzawa A, Takaoka T, Hirata Y, Shindo T, Ono M, Haga N Abstract Although some patients with fulminant myocarditis can be rescued owing to the improvements in mechanical circulatory support therapy, there are few reports providing evidence of cardiac rehabilitation during mechanical circulatory supports, particularly among pediatric patients. We treated two pediatric patients who underwent aggressive cardiac rehabilitation during mechanical support. Five days after the initiation of extracorporeal membrane oxygenation therapy aggressive cardiac reha...

Takotsubo-Like Myocardial Dysfunction in Ischemic Stroke Clinical Sciences
This study included 23 patients (0.42%). The mean age was 70.7±13.9 years, with predominance of women (73.9%) and typical pattern of Takotsubo-like myocardial dysfunction (91.3%). They were associated with short-term poor functional outcomes, including high mortality, neurological deterioration, and functional status at discharge, compared with those without (39.1% versus 2.4%, 47.8% versus 7.4%; and median [interquartile range], 5 [5–6] versus 3 [2–4]; all P

Alcoholic cardiomyopathy : The result of dosage and individual predisposition.
Authors: Maisch B Abstract The individual amount of alcohol consumed acutely or chronically decides on harm or benefit to a person's health. Available data suggest that one to two drinks in men and one drink in women will benefit the cardiovascular system over time, one drink being 17.6 ml 100 % alcohol. Moderate drinking can reduce the incidence and mortality of coronary artery disease, heart failure, diabetes, ischemic and hemorrhagic stroke. More than this amount can lead to alcoholic cardiomyopathy, which is defined as alcohol toxicity to the heart muscle itself by ethanol and its metabolites. Historical exa...

In the heart of MELAS syndrome
With interest we read the article by Youssef et al. about a 25year old male with mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) syndrome who experienced an acute deterioration of left ventricular function being attributed to myocarditis from a presumed parvovirus B19 infection [1]. We have the following comments and concerns.

The emerging role of galectins in cardiovascular disease.
Abstract Galectins are an ancient family of β-galactoside-specific lectins and consist of 15 different types, each with a specific function. They play a role in the immune system, inflammation, wound healing and carcinogenesis. In particular the role of galectin in cancer is widely studied. Lately, the role of galectins in the development of cardiovascular disease has gained attention. Worldwide cardiovascular disease is still the leading cause of death. In ischemic heart disease, atherosclerosis limits adequate blood flow. Angiogenesis and arteriogenesis are highly important mechanisms relieving ischemia by rest...

Cardiac eosinophilic granulomatosis with polyangiitis: rapid imaging with contrast CT and contrast echo aids early diagnosis
A 49-year-old male asthmatic (on Montelukast) with multiple sclerosis presented with shortness of breath and generalised weakness. On examination, he was in cardiac failure with a systolic murmur, rapid atrial fibrillation and weakness in all four limbs with slurred speech. His white cell count was 37x109/L, eosinophils 25x109/L, urea 13.6 mmol/L, creatinine 131 μmol/L, troponin I 4448 ng/L, anti-neutrophil cytoplasmic antibodies (ANCA) weakly positive (negative myeloperoxidase (MPO) and proteinase 3 (PR3)). Differential diagnoses included stroke, myocarditis and eosinophilic granulomatosis with polyangii...

Atypical Presentation of Intracardiac Floating Thrombi in Hypereosinophilic Syndrome Complicated With Stroke and Systemic Embolization: A Case Report
We present a case with atypical cardiac presentation with massive intracardiac fragile thrombi, causing peripheral emboli and strokes. HES can present as floating thrombi with thin attachment to the left ventricle, and clinicians should also be vigilant of thromboembolic complications and initiate early therapy to prevent or reduce the potential complications of HES.

A Technique for Implantation of the CentriMag Left Ventricular Assist Device to Allow Ambulation and Rehabilitation in Patients with Heart Failure.
CONCLUSIONS: CentriMag LVAD implantation via a right thoracotomy is a feasible approach that provides adequate hemodynamic support while allowing patients to ambulate, making subsequent cardiac transplantation less complicated by allowing the avoidance of a repeat sternotomy.
