Stroke after Aortic Valve Surgery: Results from a Prospective Cohort.
CONCLUSIONS: Clinical stroke after AVR was more common than previously reported, more than double for this same cohort in the STS database, and silent cerebral infarctions were detected in over half of patients undergoing AVR. Echocardiography Underestimates Stroke Volume and Aortic Valve Area: Implications for Patients With Small-Area Low-Gradient Aortic Stenosis
Conclusions Echocardiography underestimated LVOTarea, stroke volume, and therefore AVA, compared with MRI. The thresholds based on current guidelines were also inconsistent. In combination, these factors explain > 40% of patients with discordant small-area low-gradient AS.

Aortic Valve Area, Stroke Volume, Left Ventricular Hypertrophy, Remodeling, and Fibrosis in Aortic Stenosis Assessed by Cardiac Magnetic Resonance Imaging: Comparison Between High and Low Gradient and Normal and Low Flow Aortic Stenosis Valvular Heart Disease
Conclusions— CMR confirmed overall accuracy of echocardiographic classification of AS but demonstrated that LG/LF and LG/NF AS have larger AVA, less LV hypertrophy, and similar focal fibrosis compared with HG/LF AS. This challenges the view that LG/LF AS is a more advanced state of AS.

The assessment of Doppler-derived left ventricular outflow tract (LVOT) stroke volume (SV) is fundamental in outpatient, perioperative and critical care echocardiography.1 Calculation of SV allows assessment of the severity of aortic stenosis (AS) and evaluation of the haemodynamic state during undifferentiated hypotension.2 The use of focused transthoracic echocardiography (TTE) has appeal in a critical care environment, where after a single measurement of the LVOT area, pulsed wave Doppler velocity time integral (VTI) in the LVOT is used to calculate LVOT SV.

Postoperative Reverse Remodeling and Symptomatic Improvement in Normal-Flow Low-Gradient Aortic Stenosis After Aortic Valve Replacement Valvular Heart Disease
Conclusions— Patients with NFLG had less severe AS and LV remodeling than patients with normal-flow high-gradient.
