Initiating anticoagulant therapy after ICH is associated with patient characteristics and treatment recommendations
ConclusionsClear treatment recommendations by attending stroke physicians significantly influence OAC initiation after ICH. OAC were more frequently recommended and started in younger patients with better functional recovery independent from intracranial complications. This might represent an important determinant of observed beneficial associations, hinting towards an indication bias which might affect observational analyses.

The 2017 hormone therapy position statement of The North American Menopause Society
The 2017 Hormone Therapy Position Statement of The North American Menopause Society (NAMS) updates the 2012 Hormone Therapy Position Statement of The North American Menopause Society and identifies future research needs. An Advisory Panel of clinicians and researchers expert in the field of women's health and menopause was recruited by NAMS to review the 2012 Position Statement, evaluate new literature, assess the evidence, and reach consensus on recommendations, using the level of evidence to identify the strength of recommendations and the quality of the evidence. The Panel's recommendations were reviewed and approved by...

A Novel Homozygous Non-sense Mutation in the Catalytic Domain of MTHFR Causes Severe 5,10-Methylenetetrahydrofolate Reductase Deficiency
Conclusion: We identified a novel non-sense mutation in MTHFR gene in a single Egyptian family with severe MTHFR deficiency. The present investigation is clinically important, as it adds to the growing list of MTHFR mutations, which might help in genetic counseling of families of affected children and proper genotype-phenotype correlation. Background Severe 5,10-Methylenetetrahydrofolate reductase (MTHFR; OMIM 236250) deficiency is a rare inborn error of metabolism and inherited in an autosomal recessive fashion. It is a very common disorder of folate metabolism and is clinically characterized with low plasma methion...

Challenge to the Intestinal Mucosa During Sepsis
Conclusion The impact of sepsis on the gut is manifold, e.g., sepsis mediated alteration of the gut-blood barrier and increase in the intestinal permeability, which may correlate with the phenomena of bacterial translocation and lymphatic activation (“toxic-lymph”). Systemic consequences of sepsis are widespread and concern to the coagulative system, the microbiome as well as enzymes, such as pancreatic proteases, MMPs and IAPs. Nevertheless, the therapeutic approaches for modulating the mucosal immune system are still rarely effective in daily routine. Recent published studies showing that treatment with ...

EP.69Disorders of coagulation in Duchenne muscular dystrophy? Results of a registry-based online survey
Several types of coagulation disorders are reported in patients with Duchenne muscular dystrophy (DMD). Those include repeated reports of increased rate of bleeding-complications following scoliosis surgery but also reports of thromboembolic complications. Personalized survey-links were forwarded to 682 registered patients with a genetically confirmed diagnosis of DMD via the German DMD-registry of the Treat-NMD-network. Data on the degree of mobility, disposition to hematoma, epistaxis and gum bleeding, occurrence of postsurgical hemorrhage, stroke, deep vein thrombosis, cardiac thromboembolism, long-term medication and a...

Permanent Implantable Cardiac Support Systems.
CONCLUSION: A VAD system can be implanted as an alternative to cardiac transplan- tation or as a bridging treatment until the patient can be listed for transplantation and receive the transplant. Because of the organ s , only a minority of VAD patients ever receive a transplant. PMID: 31931951 [PubMed - in process]

Coagulation disorders in Duchenne muscular dystrophy? Results of a registry-based online survey.
Authors: Schorling DC, Müller CK, Pechmann A, Borell S, Langer T, Thiele S, Walter MC, Zieger B, Kirschner J Abstract Different complications of hemostasis have been reported in patients with Duchenne Muscular Dystrophy (DMD). These comprise an increased rate of bleeding-symptoms during scoliosis surgery but also thromboembolic complications such as pulmonary embolism, cerebral infarction, deep vein thrombosis or cardiac thrombus. For this cross-sectional study, personalized online survey-links were forwarded to 682 registered patients with a genetically confirmed diagnosis of DMD via the German-Austrian DMD patie...

Bleeding Complications Drive In-Hospital Mortality of Patients with Atrial Fibrillation after Transcatheter Aortic Valve Replacement.
CONCLUSION:  The present study demonstrates that the incidence of bleeding defines the in-hospital outcome of patients with AF after TF-TAVR. Thus, the periprocedural phase demands particular care in bleeding prevention. PMID: 32828074 [PubMed - as supplied by publisher]

Intra-and postoperative complications in the treatment of spinal metastases. A multicentre surveillance study from the German Spinal Registry (DWG-Register).
CONCLUSIONS: This analysis provides predictive models for surgeons to identify patients who may benefit from transitional care programmes. Preoperative status, invasiveness, blood loss> 500 ml and blood transfusions are independent predictors associated with higher risk of complication. PMID: 33047579 [PubMed - as supplied by publisher]

Complications of left ventricular assist devices causing high urgency status on waiting list: impact on outcome after heart transplantation
ConclusionsLeft ventricular assist device therapy can impair the outcome after HTx. However, the occurrence of LVAD complications may not impact on outcome after HTx. Thus, we cannot support the prioritization or discrimination of HTx candidates according to distinct mechanical circulatory support ‐associated complications. Future allocation strategies have to respect that device‐related complications may define urgency but do not impact on the outcome after HTx.

Impact of Systemic Atherosclerosis on Clinical Characteristics and Short-term Outcomes in Patients with Deep Venous Thrombosis or Thrombophlebitis
CONCLUSIONS: Systemic atherosclerosis in DVT patients was accompanied by poorer outcomes. Systemic atherosclerosis was associated with higher bleeding rate and with isolated DVT (without concomitant PE).PMID:34551354 | DOI:10.1016/j.amjms.2021.09.002

Current status of antithrombotic therapy and in-hospital outcomes in patients with atrial fibrillation undergoing percutaneous coronary intervention in Germany
CONCLUSION: The preferred therapy after PCI in patients with AF is DAT with a NOAC and clopidogrel. In-hospital ischemic and bleeding events were rare. The recommended durations for combination therapy vary considerably.PMID:35243515 | DOI:10.1007/s00059-022-05099-6
