Dysphagia screening in subacute care settings using the Italian version of the Royal Brisbane and Women's Hospital (I-RBWH) dysphagia screening tool.
Authors: Mozzanica F, Scarponi L, Pedrali S, Pizzorni N, Pinotti C, Foieni F, Zuccotti G, Schindler A Abstract The large majority of the available dysphagia screening tools has been developed for the stroke population. Only few screening tools are suitable for heterogeneous groups of patients admitted to a subacute care unit. The Royal Brisbane and Women's Hospital (RBWH) dysphagia screening tool is a nurse-administered, evidence-based swallow screening tool for generic acute hospital use that demonstrates excellent sensitivity and specificity. No Italian version of this tool is available to date. The aim of this s...

Parameters Influencing Tracheostomy Decannulation in Patients Undergoing Rehabilitation after severe Acquired Brain Injury (sABI)
Conclusions Parameters that could be used as positive predictors of weaning are: mean expiratory pressure, presence of spontaneous cough, and cough strength. Provoked cough and GCS were not predictive of weaning success. Bilateral lingual artery stenosis: A rare, late complication of chemoradiotherapy
Discussion Small vessel disease is a possible complication in patients treated by RT for head and neck cancer. Although the risk of these complications is not directly related to the total radiation dose, higher doses appear to accelerate the development of vascular lesions. Practitioners must be aware of the possibility of these complications, especially in patients surviving more than 5 years.

Acute vestibular syndrome in  cerebellar stroke : A case report and review of the literature.
Acute vestibular syndrome in cerebellar stroke : A case report and review of the literature. HNO. 2017 Mar 07;: Authors: Volgger V, Gürkov R Abstract The current paper reports on a patient with recurrent rotational vertigo and persistent dizziness and imbalance lasting several weeks, who underwent extensive neuro-otological and radiological examinations. Pathological findings initially included right-sided benign paroxysmal positional vertigo (BPPV), persistent horizontal spontaneous nystagmus (SPN) to the left, and a pathological bedside and video head impulse test (HIT) on the left. The patholog...

Acute vestibular syndrome following cerebellar stroke : Case report and literature review. German version.
Authors: Volgger V, Gürkov R Abstract The current paper reports on a patient with recurrent rotational vertigo and persistent dizziness and imbalance lasting weeks, who underwent extensive neurootological and radiological examination. Pathological findings initially included right-sided benign paroxysmal positional vertigo (BPPV), persistent horizontal spontaneous nystagmus (SPN) to the left and a pathological bedside and video head impulse test (HIT) on the left. The pathological HIT on the left and SPN to the left indicated a central genesis. Therefore, cMRI was performed and revealed a left-sided cerebral i...

Reconstruction Outcomes Following Lateral Skull Base Resection
Objective: Compare reconstruction outcomes for various lateral skull base closure techniques. Study Design: Retrospective medical records review. Setting: University-based tertiary referral center. Patients: Patients who underwent resections of tumors involving the lateral skull base requiring reconstruction beyond primary closure. Intervention(s): Reconstructive techniques, from rotational flaps to free tissue transfer. Main Outcome Measure(s): Outcome data including wound complications, cerebrospinal fluid (CSF) leakage, and need for surgical revision were tabulated. Results: Eighty-six patients underwent lateral s...

Pp - the importance of dental care and drug therapy in hospitalized patients with oral myiasis: a case report
Myiasis is a condition caused by the invasion of fly larvae from both living and dead tissues. It rarely affects the human oral cavity, but it may be found in neurological or dependent care patients with limited lip seal and inadequate oral condition. Its treatment consists of the use of Ivermectin, removal of larvae, and improved oral hygiene. Patient, SRS, male, 56, hospitalized due to sequelae caused by a stroke. During the oral hygiene, swelling in the middle third of the left cheek and lack of lip veiling were observed.

Relationship between increased carotid artery stiffness and idiopathic subjective tinnitus
AbstractTinnitus is defined as perception of sound with no external stimulus, and can separate into pulsatile and non-pulsatile types. Arterial stiffness is a parameter that can predict the cardiovascular event and associated with incidence of stroke. It has been shown that increased arterial stiffness may lead to microvascular damage in brain. Our aim was to assess the arterial stiffness of the carotid system in the development and severity of idiopathic subjective tinnitus. Forty subjective tinnitus patients and 40 age- and sex-matched controls were enrolled in the study. The parameters obtained from the participants inc...

Brainstem infarct after Le Fort I osteotomy: A morbid complication
With the evolution of advanced surgical skills, the Le Fort I osteotomy is commonly used to correct maxillofacial deformities, and the complications are well described, the most serious and life-threatening of which involve operative injury to the nerves and vital blood vessels.1 –3 Iatrogenic injury to blood vessels may lead to massive blood loss and result in ischaemia of vital structures of the brain. We would like to report a brainstem stroke, a rare, unpreventable complication encountered during such a procedure.

Arterial ligation versus embolization in epistaxis management: Counterintuitive national trends
ConclusionCompared to embolization, ligation is associated with significantly decreased hospital charges and shorter hospital stay, without an increase in complication rates. Counterintuitively, ligation appears to be trending downward nationally in its use relative to embolization. Level of Evidence2C. Laryngoscope, 2016

Impact of malignant cerebral artery infarction guidelines on the profile of the cranioplasty service
Background: Early decompressive craniectomy (DC) for malignant middle cerebral artery (MCA) infarction leads to improved survival, as demonstrated by three multicenter randomized controlled trials (DECIMAL, DESTINY, HAMLET). The survivors of a malignant MCA stroke manifest complex medical and neurological problems along with extensive calvarial defects that require cranioplasty.

Pre-treatment clinical assessment in head and neck cancer: United Kingdom National Multidisciplinary Guidelines.
Authors: Robson A, Sturman J, Williamson P, Conboy P, Penney S, Wood H Abstract This is the official guideline endorsed by the specialty associations involved in the care of head and neck cancer patients in the UK. This paper provides recommendations on the pre-treatment clinical assessment of patients presenting with head and neck cancer. Recommendations • Comorbidity data should be collected as it is important in the analysis of survival, quality of life and functional outcomes after treatment as well as for comparing results of different treatment regimens and different centres. (R) • Patients with hypertens...

Oral health status in patients with acquired brain injury: a systematic review
Conclusion Stroke was the most predominant brain injury condition studied in the literature with few publications focusing on other form of brain injury diseases. Overall, oral health has been noted to be poor in patients with ABI, but oral hygiene, and OHrQoL, has been found to be improved when oral hygiene interventions are provided to the patients.
