Neurological Involvement in Primary Systemic Vasculitis
Conclusion Neurological involvement is a common complication of PSV (Table 1), and neurologists play an important role in the identification and diagnosis of PSV patients with otherwise unexplained neurological symptoms as their chief complaint. Progeria syndrome in CKD/ESRD
Conclusions: Renal impairment independently associates with greater stroke disability at presentation and worse functional outcomes despite modern stroke care. Nonetheless these patients are also less likely to be thrombolysed for acute ischaemic stroke, which suggests a possible inequity in access to healthcare that requires urgent study.

HVAD: The ENDURANCE Supplemental Trial
ConclusionsThe ENDURANCE Supplemental Trial failed to demonstrate noninferiority of HVAD versus control regarding the pre-specified primary endpoint. However, the trial confirmed that BP management is associated with reduced stroke rates in HVAD subjects. HVAD subjects, relative to control subjects, more commonly achieved the composite endpoint (freedom from death, disabling stroke, and device replacement or urgent transplantation). Evaluation of Neuroimaging Findings of Central Nervous System Complications in Heart Transplant Recipients.
CONCLUSIONS: Posterior reversible leukoencephalopathy syndrome and ischemic stroke were the most common neurologic complications in our heart transplant recipients. The other complications were hemorrhagic stroke, intracranial abscess, and intracranial dissemination of sinusoidal fungal infection. Neurologic complications are common in heart transplant recipients and should be identified promptly for early treatment. Use and safety of heparin-free maintenance hemodialysis in the USA
Conclusions Patient markers of increased risk of bleeding and facility region associated with heparin-free HD use. Despite the potential benefits of avoiding heparin use, heparin-free HD was not significantly associated with decreased hazards of death, bleeding or thrombosis, suggesting that it may be no safer than HD with heparin.

Carotid-bulb thrombus and continuous-flow left ventricular assist devices: A novel observation
The continuous-flow left ventricular assist device (LVAD) has emerged as a clinically important treatment strategy for patients with advanced-stage heart failure. However, LVADs are not without associated neurologic complications, including stroke. In one review, the incidence of stroke (both ischemic and hemorrhagic) with the HeartMate II (HM II; Thoratec, Pleasanton, CA) ranged from 2% to 18%. In another recent single-center study, the incidence was 10%. In that study, previous stroke, persistent malnutrition and inflammation, severity of heart failure and post-LVAD infections were associated with neurologic complications.

Cerebrovascular disease in the era of left ventricular assist devices with continuous flow: Risk factors, diagnosis and treatment
Cerebral infarction and hemorrhage are among the principal sources of morbidity and mortality associated with mechanical circulatory support. There has been particular concern with the recently reported increased rate of device thrombosis in continuous-flow left ventricular assist devices. The neurologic management of stroke among these patients has limited data to inform current clinical practices. In this investigation we review the available literature on stroke in patients with continuous-flow left ventricular assist devices, with a focus on treatment algorithms in the acute setting and for secondary stroke prevention.

How I manage cerebral vasculopathy in children with sickle cell disease
Summary Sickle cell disease induces specific brain alterations that involve both the macrocirculation and the microcirculation. The main overt neurovascular complications in children are infarctive stroke, transient ischaemic attack and cerebral haemorrhage. Silent cerebral infarction, cognitive dysfunction and recurrent headache are also common. Cerebrovascular disease selectively affects children with the HbSS or HbS‐β0 genotypes (i.e. sickle cell anaemia). Neurological complications in renal transplant patients: A single-center experience
Conclusion We conclude that complications involving the neurological system occur in 10.5% of all transplant patients with 8% involving CNS and 7% involving the PNS. The high mortality rates associated with CNS complications warrant early diagnosis and aggressive treatment in renal transplant recipients.

Propensity score ‐based analysis of long‐term outcome of patients on HeartWare and HeartMate 3 left ventricular assist device support
ConclusionsSurvival was not significantly different between both groups after PS matching, but was better for HM3, with a significantly lower incidence of haemorrhagic stroke and pump thrombosis for HM3. These results need to be interpreted carefully, because matching may have introduced greater imbalance on unmeasured covariates. A multicentre approach of carefully selected centres is recommended to enlarge the number of matched patients.
